Rescue sedation with dexmedetomidine for diagnostic imaging: a preliminary report.

BACKGROUND: Sedation is frequently required during noninvasive radiological imaging in children. Although commonly used agents such as chloral hydrate and midazolam are generally effective, failures may occur. The authors report their experience with dexmedetomidine for rescue sedation during magnetic resonance imaging. METHODS: A retrospective chart review was undertaken. RESULTS: The cohort included five patients ranging in age from 11 months to 16 years. Following the failure of other agents (chloral hydrate and/or midazolam), dexmedetomidine was administered as a loading dose of 0.3-1.0

Hemodynamic and coronary vascular effects of dexmedetomidine in the anesthetized goat.

BACKGROUND: In phase III trials, the hemodynamic stabilising effect of the alpha 2-adrenergic agonist dexmedetomidine (DEX) is being investigated in patients with coronary artery disease, Coronary vascular effects of alpha 2-agonists have been studied in dogs and pigs, but both species have a different hemodynamic response to DEX than man. The aim of this study was to investigate the hemodynamic and coronary vascular effects in goats. METHODS: In 6 open-chest goats anesthetized with halothane, central and coronary hemodynamics and oxygen supply and demand were measured before and following IV

Effect of dexmedetomidine on lumbar cerebrospinal fluid pressure in humans.

UNLABELLED: Dexmedetomidine's potential for analgesia without respiratory depression and its opioid- and anesthetic-sparing properties make it an attractive choice as an anesthetic adjunct for patients undergoing neurosurgery. However, the effects of dexmedetomidine on intracranial pressure are not known. We therefore studied the effect of dexmedetomidine on lumbar cerebrospinal fluid (CSF) pressure in patients after transphenoidal pituitary tumor surgery. Sixteen transphenoidal pituitary tumor surgery patients were randomized to receive placebo (n = 9) or dexmedetomidine (n = 7) for 60 min in

Dexmedetomidine modulates peritoneal macrophage to attenuate lipopolysaccharide-induced inflammation.

PURPOSE: To investigate how Dexmedetomidine (Dex) modulates the function of peritoneal macrophages (PMs) to reduce lipopolysaccharide (LPS)-induced inflammation. METHODS: The anti-inflammatory effect of Dex on LPS-stimulated PMs was assessed by examining its impact on their proliferation, phagocytosis, and polarization. Proliferation and phagocytic activity were measured using CCK-8 and Neutral Red staining assays, respectively. The levels of inflammatory mediators were quantified using ELISA. Additionally, macrophage polarization was evaluated via ELISA, flow cytometry, and Western blot anal

Dexmedetomidine for cancer pain: mechanisms and opioid-sparing effects.

BACKGROUND AND OBJECTIVE: Cancer pain treatment faces challenges such as ineffective pain management, high-dose opioid use, and insufficient analgesia. Dexmedetomidine (DEX), a novel α2 receptor agonist, is a potential adjuvant analgesic. Its analgesic mechanism involves central coeruleus cell hyperpolarization, activation of peripheral, spinal cord, and spinal α2 receptors, and regulation of cellular signaling pathways and inflammatory factors. DEX reduces harmful neurotransmitter production and pain signal transmission and enhances opioid analgesia while decreasing opioid use and tolerance.

Intravenous dexmedetomidine use in obstetric anesthesia: a focused review.

Dexmedetomidine is an alpha-2 adrenergic receptor agonist with analgesic properties. Dexmedetomidine is currently U.S. Food and Drug Administration (FDA) approved for intravenous (IV) administration in non-pregnant patients. However, it has shown promise for various off-label indications in obstetric anesthesia. This review focuses on reported uses for IV dexmedetomidine in obstetric anesthesia. Intravenous dexmedetomidine has reported efficacy for producing light sedation, analgesia, and anxiolysis in the parturient. In addition, the use of IV dexmedetomidine during cesarean delivery has been

Antianxiety effects of dexmedetomidine: systematic review and meta-analysis.

BACKGROUND: Despite numerous studies of the anxiolytic effects of dexmedetomidine compared with those of other drugs or saline, the results have been inconsistent. Here we report a systematic review and meta-analysis to comprehensively evaluate the evidence of the anxiolytic effects of dexmedetomidine. METHODS: This research has been registered in the International Prospective Register of Systematic Reviews. The PubMed, Embase, Cochrane Library, Web of Science, and ClinicalTrials.gov database were searched for clinical trials that compared the anxiolytic effects of dexmedetomidine with those

Maximal Tizanidine withdrawal managed with dexmedetomidine: a vital intervention.

Tizanidine withdrawal is a rare and complex phenomenon characterized by a surge in adrenergic activity upon abrupt discontinuation of the drug. We present a unique case of a 41-year-old male with multiple comorbidities who self-administered an exceptionally high daily dose of Tizanidine, leading to severe withdrawal symptoms. This case report highlights the challenges in managing such cases. The patient, with a history of myofascial pain syndrome, hypertension, anxiety, and depression, experienced distressing symptoms, including tachycardia, rebound hypertension, neuropsychiatric manifestation

Hypothermia Inhibits Dexmedetomidine-Induced Contractions in Isolated Rat Aortae.

Dexmedetomidine is widely used to induce sedation in the perioperative period. This study examined the effect of hypothermia (33 and 25 °C) on dexmedetomidine-induced contraction in an endothelium-intact aorta with or without the nitric oxide synthase inhibitor NW-nitro-L-arginine methyl ester (L-NAME). In addition, the effect of hypothermia on the contraction induced by dexmedetomidine in an endothelium-denuded aorta with or without a calcium-free Krebs solution was examined. The effects of hypothermia on the protein kinase C (PKC), myosin light chain (MLC20) phosphorylation, and Rho-kinase m

Dexmedetomidine alleviates propofol-induced neural injury in developing rats.

Propofol, a commonly used intravenous anesthetic, has been associated with neurodegeneration in the developing brain upon repeated exposure. Dexmedetomidine is an α2 adrenoceptor agonist that was previously reported to possess neuroprotective properties. Here, we confirmed the impacts of dexmedetomidine on propofol-induced neuroapoptosis and subsequent spatial learning and memory deficits in neonatal rats. We found that dexmedetomidine effectively mitigated propofol-induced spatial learning and memory impairments and improved aversive memory in developing rats. Dexmedetomidine reduced propofol

Hypotensive Events in Pediatric Patients Receiving Dexmedetomidine for MRI.

PURPOSE: Dexmedetomidine, the preferred pediatric sedating agent for magnetic resonance imaging (MRI), has the side effect of hypotension. Newer recommendations for reporting adverse events in pediatric procedural sedation include using a two-pronged definition. Our aim was to describe the incidence of hypotension in patients undergoing sedated MRI and to identify demographic and clinical factors associated with hypotension, applying a two-pronged definition, where a numerical threshold/clinical criterion must be met as well as at least one clinical intervention performed. DESIGN: An observat

Dexmedetomidine Utilization During Air Medical Transport for Agitated Patients.

Fixed Wing Air Ambulance providers routinely transport patients agitated from traumatic brain injury sequelae across long distances in a unique environment. The current paradigm limits options available to air medical clinicians to those routinely found on ground based, short distance vehicles, plus whatever a sending facility might be willing to provide. We postulate that dexmedetomidine offers a safe, effective alternative to improve patient care and enhance the safe operation of aircraft.

EEG response of dexmedetomidine during drug induced sleep endoscopy.

INTRODUCTION: Dexmedetomidine is one of the anesthetics of choice for drug induced sleep endoscopy (DISE), with advantages including limited respiratory depression, analgesia, and decreased incidence of emergence delirium. However, challenges with determining sedation levels and prolonged recovery have limited its usage. An improved understanding of the effect of dexmedetomidine on the level of sedation and the corresponding electroencephalographic (EEG) changes could help overcome these barriers. METHODS: Fifty-one patients received dexmedetomidine sedation with Richmond Agitation-Sedation S

Acute Colonic Pseudo-Obstruction Following the Use of Dexmedetomidine.

Dexmedetomidine is a preferred agent for light sedation with minimal adverse effects. We report a case of acute colonic pseudo-obstruction following dexmedetomidine use in a patient with alcohol withdrawal. He was treated with benzodiazepines first to control the withdrawal symptoms, then escalated to dexmedetomidine once delirium tremens ensued. Later on, the patient developed abdominal distension and vomiting. Imaging showed dilated bowel loops and absence of peristalsis on ultrasound. Decompression with the nasogastric (NG) tube was done, with high output from the NG tube. Dexmedetomidine i
