Updated Dietetic Prescribing Guidelines When Naso-Gastric Tube Feeding Under Physical Restraint Is Required: A Modified Nominal Group Technique Study.

INTRODUCTION: Naso-gastric tube (NGT) feeding under physical restraint is a lifesaving clinical intervention that is sometimes needed in mental health patients, particularly those with restrictive eating disorders. Initial guidance was published in 2020 and subsequently reviewed and updated to support dietitians in this specialist area. METHODS: Using purposeful recruitment, we brought together specialist mental health dietitians from England, Wales, Scotland and Northern Ireland who work in inpatient child and adolescent mental health services or adult inpatient mental health services. We fo

Effectiveness of dietetic consultation for lowering blood lipid levels in the management of cardiovascular disease risk: A systematic review and meta-analysis of randomised controlled trials.

AIM: Evidence of the effectiveness of dietetic consultation for the management of cardiovascular disease (CVD) risk factors has not been previously synthesised. A systematic review and four meta-analyses evaluated the effectiveness of dietetic consultation for lowering blood lipid levels in high-risk individuals in primary health-care settings. METHODS: Of the 4860 records identified, 10 eligible randomised controlled trials (RCTs, n = 1530) were evaluated for reporting blood lipid outcomes following dietetic consultation (DN)-defined as at least one exclusive individual face-to-face consulta

A single-centre retrospective study investigating changes in nutritional status and use of dietetic interventions in children receiving chimeric antigen receptor T-cell therapy.

BACKGROUND AND AIMS: Chimeric antigen receptor T-cell therapy (CAR-T) is a novel treatment for children with relapsed or refractory acute lymphoblastic leukaemia. Preferential outcomes have been shown when children maintain their nutritional status during cancer treatment. Few studies have investigated the impact of CAR-T on the nutritional status of children. This study aimed to examine the nutritional status and use of dietetic interventions in children having CAR-T. METHODS: Electronic records of all children who had CAR-T between June 2015-June 2024 at Great Ormond Street Hospital in Lond

Evaluation of a weekly speech pathology/dietetic service model for providing supportive care intervention to head and neck cancer patients and their carers during (chemo)radiotherapy.

PURPOSE: Dysphagia is a common and debilitating side effect for head and neck cancer (HNC) patients undergoing radiotherapy (RT) with or without chemotherapy ([C]RT) and is associated with nutritional and emotional comorbidities. Emotional sequelae and distress are also known to affect carers of HNC patients. A weekly, joint speech pathology/dietetic (SP/DN) service-delivery model has been employed to manage swallowing/nutritional and associated emotional issues during (C)RT. This study aimed to conduct a service evaluation of the weekly SP/DN clinical model. METHODS: Cross-sectional sampling

Implementation of clinical practice guidelines using the Plan-Do-Study-Act framework: The methodology and experiences of the Academy of Nutrition and Dietetics Health Informatics Infrastructure Registry Study on gestational diabetes mellitus.

BACKGROUND: Registered dietitian nutritionists (RDNs) use clinical practice guidelines (CPGs) to inform evidence-based practice. Despite the availability of CPGs, guidelines are not always translated into practice. This challenge is central to implementation science (IS), which seeks to understand how evidence can be adopted and sustained. METHODS: The Gestational Diabetes Mellitus (GDM) Registry Study was a multiphase, multisite hybrid implementation study that explored guideline implementation using quality improvement (QI) methods grounded in the Model of Improvement and guided by Plan-Do-

Do Patients Receiving GLP-1 Receptor Agonists for Weight Loss Require Structured Dietetic Care? Lessons from Metabolic and Bariatric Surgery (MBS) Pathways.

BACKGROUND: Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) and dual incretin therapies have significantly advanced obesity management by facilitating clinically significant weight loss and enhancing cardiometabolic outcomes. Nevertheless, their growing application calls for careful consideration of nutritional and behavioural factors, including decreased dietary intake, gastrointestinal symptoms, loss of lean mass, micronutrient deficiencies, and potential weight regain following treatment cessation. METHODS: This narrative review synthesises evidence from clinical trials, observationa

The selenium state of children. II. Selenium content of serum, whole blood, hair and the activity of erythrocyte glutathione peroxidase in dietetically treated patients with phenylketonuria and maple-syrup-urine disease.

The selenium content of serum, whole blood and hair was measured by neutron activation analysis in dietetically treated patients with phenylketonuria (PKU) and maple-syrup-urine disease (MSUD). Follow-up studies showed a decrease of the serum selenium content and the glutathione peroxidase activity of erythrocytes--a selenoenzyme--from normal values at the beginning of the diet to 20% (selenium) and 50% (gluthione peroxidase) of normal within 10--12 weeks of dietary treatment. In 36 patients the serum selenium content was lower at 6.7--28 X 10(-9) g/ml, independent of the age of the patients (

You Are Not an Impostor: The Registered Dietitian Nutritionist and Impostor Phenomenon.

Impostor phenomenon (IP) interferes with the desire for taking advantage of career opportunities and pursuing career advancement. There is robust literature describing the effects of IP in other health care professions, yet an absence of research exists on IP within the nutrition and dietetics profession. This perspective will explore why nutrition and dietetics professionals may experience IP, specify what might be done to mitigate the negative effects of IP, and describe future research directions. Impostorism is an understudied phenomenon that could have significant implications for the pro

Nutrition care and Dietetic Practice in Managing Metabolic and Endocrine Dysfunction in Polyendocrine Metabolic Ovarian Syndrome (PMOS): Implications and Opportunities in the Context of the Name Change of Polycystic Ovarian Syndrome (PCOS) to PMOS.

Polyendocrine metabolic ovarian syndrome (PMOS) affects 8-13% of women across the lifespan. Those with PMOS have reduced quality of life and increased risk of co-morbidities including elevated weight, eating disorders, gestational diabetes, type 2 diabetes and cardiovascular disease. PMOS is a major population in clinical dietetic practice and dietary intervention is the first-line approach recommended to target metabolic risk in the international evidence-based guidelines for assessment and management of PCOS. The past name, polycystic ovary syndrome (PCOS) was ovarian-centric and did not acc

Effect of time restricted eating versus current practice in dietetics on glycaemic control and cardio-metabolic outcomes in individuals at risk of developing type 2 diabetes: Protocol for a multi-centre, parallel group, non-inferiority, randomised controlled trial.

BACKGROUND: Time restricted eating (TRE) is a dietary strategy that may improve metabolic health. However, no studies have compared TRE with current practice (CP) in dietetics. HYPOTHESIS: TRE will not be inferior to CP to improve glycaemic control in individuals at risk of type 2 diabetes (T2D). METHODS: This parallel group, randomised, non-inferiority, controlled trial randomised 247 participants by site and glycated haemoglobin (HbA1c) into TRE or CP (1:1) for 12 months. Participants were aged 35-70 years, with a body mass index (BMI) >25 but <45 kg/m2, and score ≥15 on the Australian typ

Is vegetarianism healthy for children?

According to the Academy of Nutrition and Dietetics' influential position statement on vegetarianism, meat and seafood can be replaced with milk, soy/legumes, and eggs without any negative effects in children. The United States Department of Agriculture endorses a similar view. The present paper argues that the Academy of Nutrition and Dietetics ignores or gives short shrift to direct and indirect evidence that vegetarianism may be associated with serious risks for brain and body development in fetuses and children. Regular supplementation with iron, zinc, and B12 will not mitigate all of thes

Symposium on 'The challenge of translating nutrition research into public health nutrition'. Session 3: Joint Nutrition Society and Irish Nutrition and Dietetic Institute Symposium on 'Nutrition and autoimmune disease'. Recent advances in genetic understanding of coeliac disease.

Over the past 20 years major advances have been made in the diagnosis and understanding of pathogenic mechanisms relating to coeliac disease. Recently-identified genetic markers support the immunological-inflammatory nature of the disease. It is hoped that these newly-identified genes will assist further dissection of the inflammatory pathways in coeliac disease and give insight into why certain individuals develop intolerance to dietary gluten.
