A case report of neurologically unstable fracture of the lumbosacral spine in a patient with ankylosing spondylitis.

INTRODUCTION: Fracture/dislocation is uncommonly reported in ankylosing spondylitis involving the lumbosacral spine. CLINICAL PICTURE: We report an 18-month follow-up of a case of neurologically unstable traumatic fracture of the lumbosacral spine in ankylosing spondylitis. TREATMENT/OUTCOME: Posterior decompression, alar-transverse fusion and instrumentation were performed. Anterior diskectomy and fusion were done 6 weeks later. There was solid bony fusion on follow-up and the patient had improvement of 2 Frankel grades and was able to ambulate. CONCLUSION: Combined approaches and longer f

The surgical management of ossification of the posterior longitudinal ligament in 51 patients.

Ossification of the posterior longitudinal ligament (OPLL) in the cervical spine warrants unique clinical, radiographic, and surgical management. OPLL patients presenting with severe myelopathy require full assessment with both magnetic resonance imaging (MRI) and computed tomography-based (noncontrast CT, myelo-CT, three-dimensional CT) examinations to document the full extent of their disease. Whether better surgical outcomes are attained after anterior resection (diskectomy/corpectomy), rather than posterior decompression (laminectomy/laminoplasty) of OPLL remains controversial. However, ou

Evaluation and treatment of chemonucleolysis failures.

Fifty patients underwent chemonucleolysis for the treatment of lumbar disk herniations unresponsive to conservative therapy. In patients treated with chymopapain, unrelieved sciatica was the most common cause of clinical treatment failure. Eight patients (16%) experienced no relief or only a transient reduction in their radicular symptoms following chymopapain injection. All eight patients were clinically reevaluated and underwent repeat neuroradiographic studies. Computed axial tomography and lumbar myelography demonstrated persistent nerve root compromise at the level of the injected disk sp

Clinical outcomes in patients after lumbar disk surgery with annular reinforcement device: two-year follow up.

Annular Reinforcement Device represents a modification of operative treatment of intervertebral disk herniation. It is a prosthesis that is anchored into the body of the vertebra. The intradiscal part of the implant is placed in the inner part of the annulus fibrosus defect. The aim of this technique is to reduce the incidence of reherniation and the degree of intervertebral space collapse, which is the most frequent adverse effect of diskectomy. Clinical outcomes of the treatment group indicated a statistically significant improvement with respect to the control group. Furthermore, over the p

Synovial chondromatosis of the left temporomandibular joint superficially resembling chondrosarcoma: a case report.

A 25 year-old woman was referred to the clinic complaining of pain in the left temporomandibular joint (TMJ) and trismus. According to the x-ray images, a solitary mass was observed in the anterior pouch of the lower joint cavity. The mass was removed by means of a synovectomy and a diskectomy. Upon light microscopic examination, the tissue removed showed high cellular activity. As a differential diagnosis, we had to consider the possibility of chondrosarcoma based on the histopathological features; however, since no invasive nor metastatic finding was recognized, we made a diagnosis of synovi

Burnei's anterior transthoracic retropleural approach of the thoracic spine: a new operative technique in the treatment of spinal disorders.

BACKGROUND: Up to the middle of the last century, the thoracic spine, especially in its upper part, has been considered an unapproachable site, a no-man's land, but the constant evolution of medicine imposed techniques of the spine at these levels in order to solve a large area of pathology (infectious, tumoral, traumatic, and last but not least, deformative). This way, a series of anterior approaches allowed surgeons to gain access to the anterior part of the spine and the posterior mediastinum. The approaches described by Hodgson, Mirbaha or transthoracic transpleural approach (T4-T11), are

Nontraumatic acute paraplegia associated with cervical disk herniation.

BACKGROUND: Acute paraplegia is a true emergency. It is often the result of trauma but is rarely reported in association with cervical disk herniation in patients without antecedent injury. METHODS: Case report. FINDINGS: This 75-year-old man presented with acute paraplegia due to severe compression of the spinal cord by herniation of the C4-C5 cervical disk. He underwent emergency diskectomy and anterior fusion. Postoperatively, his neurologic functions improved gradually. CONCLUSIONS: Cervical disk herniation should be considered in the differential diagnosis of nontraumatic acute paraple

Short-term effect of fresh disk allograft on the osteoarthrotic sheep temporomandibular joint.

UNLABELLED: The aim of this experimental study is to evaluate the effect of fresh disk allograft on the osteoarthrotic temporomandibular joints in four sheep. STUDY DESIGN: Bilateral osteoarthrosis was induced in the sheep's temporomandibular joint. Three months later, unilateral diskectomy and fresh disk allograft repair were performed, with the contralateral joint being left as a control. At 6 months after the initial procedure, 3 months after the repair, the four sheep were sacrificed. RESULTS: It was found that the operated joint was fibrously repaired without ankylosis, whereas the cont

Indications for lumbar microdiskectomy.

A very high percentage of patients coming to surgery for large disk extrusions and sciatica do very well with minimally invasive diskectomy. In most patients given relatively early surgical treatment, the primary predictor of outcome is the size of the disk herniation and the remaining competency of the anulus fibrosus. With the passage of time and with prolonged disability before surgery, psychosocial factors become increasingly important in determining outcomes. Factors such as psychological distress, depression, involvement with workers' compensation claims and disability claims, drug and a

Expression of proteoglycan mRNA in human temporomandibular joint disks in temporomandibualr disorders.

The purpose of this study was to investigate the expression of PG mRNA in human temporomandibular joint (TMJ ) disks from patients with temporomandibular disorders (TMD). Total RNA was isolated from the TMJ disks. All patients had undergone diskectomy under general anethesia. RT-PCR was performed using primers for the proteoglycans, aggrecan and decorin. On electrophoresis of a lysate of human TMJ disks, decorin was recognized in each sample. In the calcified part of the disk which we recognized visually, aggrecan was also recognized. These results suggest that TMJ disks contain decorin, and a

Preoperative Electrodiagnostic Study Findings Differ Between Patients With Double-crush Syndrome and Carpal Tunnel Syndrome: A Propensity Matched Analysis.

INTRODUCTION: Double-crush syndrome (DCS) represents a condition that involves peripheral nerve compression in combination with spinal nerve root impingement. The purpose of this study was to compare electrodiagnostic study (EDS) results in patients undergoing carpal tunnel release (CTR) for carpal tunnel syndrome with those undergoing both CTR and anterior cervical diskectomy and fusion for DCS. METHODS: Patients receiving an isolated CTR were compared with those undergoing CTR and anterior cervical diskectomy and fusion within two years of CTR. The latter group was defined as our DCS cohort

Pseudarthrosis failures of anterior subaxial cervical spine fusion using a plate with a single screw per vertebral body: a case series.

INTRODUCTION: The UNIPLATE was developed to improve operative times and limit dissection at the lateral margins of the vertebral bodies. The distinguishing character of this plate is its thin design, which requires only one screw per vertebral level (monovertebral screw plate). Most cervical spine plates, in contrast, are designed for two screws per vertebral level (bivertebral screw plate). Limited reports of the biomechanical efficacy of the UNIPLATE are available, and to the authors' knowledge, this report represents the largest clinical study of its use. METHODS: This is a retrospective c

[Imaging with magnetic resonance imaging of the operated cervical spine].

The Authors report their personal experience with postoperative neuroradiologic studies in surgical patients with a herniated disk and/or degenerative cervical spondylosis. Twenty-six patients were enrolled: 16 of them underwent anterior diskectomy, 7 posterior decompressive laminectomy and 3 vertebral body removal. The patients were submitted to pre- and postoperative MRI with high field equipment (Signa 1.5 T, General Electrics), using sagittal SE T1- (TR/TE 360/15), PD and T2-weighted (TR/TE/FA 2000/40/120) sequences and T2-weighted GE axial sequences (TR/TE/FA 400/34/30). Clinical and Radiological Outcomes of Skip-Level Cervical Disk Arthroplasty.

BACKGROUND AND OBJECTIVES: The skipped, in-between indexed, level theoretically has higher chances of adjacent segment disease (ASD), especially in between 2 anterior cervical diskectomy and fusion (ACDF) constructs. However, this inference remains unclear. The objective was to compare cervical disk arthroplasty (CDA) with ACDF for noncontiguous cervical disk problems. METHODS: Consecutive patients who underwent skip-level anterior cervical diskectomies for symptomatic disk herniations or spondylosis involving noncontiguous pathologies of the subaxial cervical spine were retrospectively analy

Surgical management of cervical radiculopathy.

Cervical radiculopathy presents as pain in a dermatomal distribution. Despite conservative nonoperative therapy, a large subset of patients will require surgical intervention. Indications for surgery include recalcitrant radiculopathy despite nonoperative treatment for more than 6 weeks and progressive motor deficit or disabling motor deficit (deltoid palsy, wrist drop) prior to 6 weeks. Anterior and posterior approaches have both yielded successful results in appropriately selected patients. Anterior cervical diskectomy and fusion is the generally preferred treatment for radiculopathy when th

Adverse events associated with anterior cervical spine surgery.

Anterior cervical procedures for neurologic decompression and fusion, including cervical diskectomy and cervical corpectomy, are commonly performed by orthopaedic surgeons and spinal neurosurgeons. These procedures are highly successful in treating most patients with persistent pain and neurologic symptoms that have not responded to nonsurgical methods. Adverse events occur infrequently, but several have been described, including esophageal injury, vertebral artery injury, dural tear, postoperative airway compromise, spinal cord injury, hematoma, dysphagia, dysphonia, and graft dislodgement. Posterior Percutaneous Endoscopic Cervical Discectomy for Single-Level Soft, Huge Central Disc Herniation.

BACKGROUND: Posterior percutaneous endoscopic cervical discectomy (PPECD) has been proven safe and effective for foraminal cervical disc herniation (CDH). However, central CDH has long been considered as the contraindication of PPECD, because the path is obstructed by the spinal cord and nerve root. OBJECTIVES: To preliminarily assess the feasibility, safety, and effectiveness of PPECD for single-level soft, huge central CDH. STUDY DESIGN: A retrospective cohort study. SETTING: Zhejiang Provincial People's Hospital (Affiliated People's Hospital, Hangzhou Medical College). METHODS: Between
