Unexpected Intradural Lumbar Disk Herniation Found During Transforaminal Endoscopic Surgery.

BACKGROUND: Intradural disk herniation (IDH) is defined as the displacement of the intervertebral disk nucleus pulposus into the dural sac. In this lumbar lesion, the affected level differs from that of a traditional extradural herniation: 55% of cases occur at the level of L4-5, 16% at L3-4, and 10% at L5-S1. Upper lumbar IDH is extremely rare. We present a case of an IDH at the level of L2-3 that was diagnosed during endoscopic surgery. CASE DESCRIPTION: A 65-year-old male patient presented with severe radiating pain in the anterior right thigh that was accompanied by a tingling sensation i

Management of Root-Level Double Crush: Case Report with Technical Notes on Contralateral Interlaminar Foraminotomy with Full Endoscopic Uniportal Approach.

BACKGROUND: Double crush of a nerve at the root level is not common. We describe here a double crush of the right L4 nerve with foramina to far lateral disk (ventral) and extraforaminal (dorsal) compression. The double crush was managed by endoscopy with a contralateral uniportal approach from the left interlaminar space. Right lateral recess stenosis at the same level was subsequently managed with the same approach. CASE DESCRIPTION: A 79-year-old lady presented to our clinic with a low backache, right leg pain, and weakness. On her right side, the straight leg raise test was 70 degrees, gre

The value of nerve root infiltration for leg pain when used with a nerve stimulator.

Randomized comparative study of the efficacy of nerve root infiltration (NRI) guided by neurostimulator to find the value of nerve stimulators in nerve root infiltration was undertaken. The response to nerve root infiltration using local anaesthetics and steroid is unpredictable, partly because the exact nerve root giving rise to pain may not be truly infiltrated. The nerve stimulator is advocated to identify the nerve root of concern prior to infiltration. The current study assessed the results of NRI with and without the nerve stimulator. Ninety-six patients with leg pain awaiting selective

Accuracy of machine learning in predicting recurrence risk of lumbar disc herniation after percutaneous endoscopic lumbar discectomy: a systematic review and meta-analysis.

BACKGROUND: Lumbar disc herniation (LDH) is a major cause of low back pain and radicular leg pain. Percutaneous endoscopic lumbar discectomy (PELD) is a widely used minimally invasive procedure for the treatment of LDH; however, recurrent lumbar disc herniation (RLDH) after surgery remains a significant clinical challenge. Recent studies have explored the application of machine learning (ML) models for predicting recurrence risk after PELD, but variations in variable selection and algorithm choice have led to inconsistent predictive performance. This systematic review and meta-analysis evaluat
