Rifampicin-induced fever during treatment against staphylococcal biofilm in a patient with periprosthetic joint infection.

Periprosthetic joint infection (PJI) after total knee arthroplasty remains a challenging complication. The treatment options for PJI include different procedures; however, regardless of the strategy, antibiotics are required. The combination of different antibiotics increased the rates of PJI eradication. For almost 3 decades, rifampicin has been used as part of antibiotic therapy for PJI. Drug fever, a febrile response that coincides with the onset of drug administration and disappears after drug discontinuation in the absence of other underlying conditions that could cause fever, is frequent

Chloramphenicol resistant enteric fever.

In recent times there have been several reports of chloramphenicol resistant enteric fever necessitating the use of other antimicrobial agents. Clinical profile of 15 chloramphenicol resistant patients of enteric fever was studied. Three such patients (20%) responded to chloramphenicol despite in vitro resistance to the drug. Hence chloramphenicol still remains the drug of first choice in enteric fever as a majority (68.4%--26 out of 38) of our bacteriologically proven enteric fever patients were cured by the drug. The remaining 12 cases responded satisfactorily to ciprofloxacin (46.7%), genta

Fever of unknown origin in renal transplant patients with tacrolimus.

The immunosuppressive agent tacrolimus is now widely used for the prevention of acute and chronic rejection in renal allograft recipients. We here report on three patients, who developed drug-induced fever due to tacrolimus one to five months after renal transplantation. Extensive search for a focus, autoantibodies and virus infection remained inconclusive. Therefore, drug-induced fever was suggested. After discontinuing tacrolimus and switching to cyclosporine A fever completely resolved within 24 h. This report demonstrates that tacrolimus-induced drug fever should be included in the differe

Fever in a patient with mild acute pancreatitis caused by ulinastatin injection: A case report.

RATIONALE: The case describes a patient with mild acute pancreatitis (AP) who developed a persistent high fever during ulinastatin treatment. This highlights the possibility of drug-induced fever, which is a rare but important adverse reaction of ulinastatin and is often overlooked in clinical practice. PATIENT CONCERNS: A 58-year-old male patient with mild AP had a persistent high fever during the treatment with ulinastatin, but there was no evidence of infection. DIAGNOSES: Mild AP, ulinastatin-related drug fever. INTERVENTIONS: The initial conservative treatment for AP included fasting,

Suspected Rifampicin-Induced Fever During Periprosthetic Joint Infection Treatment: A Report of 2 Cases.

CASE: Drug-induced fever is a rare adverse reaction to antibiotics. This case report describes 2 cases of suspected rifampicin-induced fever in patients undergoing periprosthetic joint infection treatment. Both patients experienced fever with the absence of inflammatory symptoms and a discrepancy between clinical improvement and persistent fever. Discontinuation of rifampicin led to rapid symptom resolution. CONCLUSION: Drug-induced fever should be considered when fever persists without infectious symptoms and after excluding other causes. Prompt recognition and discontinuation can lead to ra

[Fever in rheumatic diseases--the spectrum of causes and diagnostic guidelines].

The review presents a systematic approach to the diagnostic problem of fever in combination with inflammatory rheumatic symptoms, specifically, in regards to: actual concepts of the pathogenesis of fever in various rheumatic diseases, fever as expression of disease activity, fever as a warning sign of complications in the course of disease and its treatment (infections, drug-induced fever, fever following drug withdrawal, secondary neoplasias), fever and inflammatory rheumatic symptoms as manifestation of non-rheumatic diseases. Based on the literature and information provided by pharmaceutica

Azathioprine-induced fever in sarcoidosis.

OBJECTIVE: To report a case of drug-induced fever associated with azathioprine treatment in a patient with sarcoidosis. CASE SUMMARY: A 52-year-old man with pulmonary sarcoidosis presented to the emergency department with a 1-day history of fever (temperature 39.9 °C), chills, nausea, and vomiting. One week earlier, azathioprine 50 mg/day had been started for worsening dyspnea. The patient was admitted and evaluated for acute infectious processes. All of his home medications (hydroxychloroquine, prednisone, fluticasone/salmeterol, lovastatin, pantoprazole, zolpidem, ibandronate, albuterol), e

Heparin-Induced Fever: A Case Report and Literature Review.

Burn patients are often plagued by fever due to the inflammatory nature of their injuries as well as the normal postoperative systemic inflammatory response syndrome. One etiology for fever, often not initially considered, is drug-induced fever. A rare cause of drug-induced fever is heparin with only one documented case reported in the literature. We present a case of heparin-induced fever in a patient who experienced a 32% total BSA friction burn after a motorcycle crash.

Dexmedetomidine-Induced Fever in a 66-Year-Old Male With Pneumonia and Pleural Effusion.

Drug-induced fever is a significant adverse effect as many commonly used medications can cause this. The incidence of drug fever is even higher in critical care settings because multiple medications are being administered simultaneously. This poses a serious problem in critical care settings as any new fever in these settings also implies any new infection or worsening of preexisting conditions. This may lead to a detailed investigation for the cause of fever, which can be time-consuming, invasive, costly, and may also increase the duration of stay along with an associated increase in morbidit

Fever caused by hydroxyurea: a report of three cases and review of the literature.

Hydroxyurea (HU) is generally regarded as an effective and well-tolerated drug for the treatment of the chronic myeloproliferative syndromes. It has rarely been implicated as a cause of drug fever. We report two patients with primary thrombocythaemia and one patient with polycythaemia vera who developed fever and shaking chills during treatment with HU. Infection was highly suspected and all patients were examined extensively. The fever subsided after discontinuation of therapy with this drug. However, the fever recurred within 1 day after rechallenge. The mechanism of HU-induced fever remaine

Suspected phenobarbital-induced fever in a cat.

CASE SUMMARY: A 3-year-old spayed female domestic shorthair cat developed a fever 1 week after starting the anticonvulsant phenobarbital. A diagnostic work-up for seizures and subsequent onset of fever of unknown origin, consisting of MRI of the brain, cerebrospinal fluid analysis and infectious disease testing, was unremarkable. The cat was switched from phenobarbital onto pregabalin with complete resolution of the fever within 24 h, consistent with a drug-induced fever following phenobarbital administration. Multidrug-resistant typhoid fever in Singapore.

A study was conducted to determine the clinical and epidemiological characteristics of multidrug-resistant (MDR) typhoid fever in Singapore. Twenty-one of 121 patients with typhoid fever had MDR typhoid fever after recent travel to the Indian subcontinent. Fifty patients with drug susceptible typhoid fever were also analysed for comparison. Nineteen of the MDR S. typhi isolates had resistance to ampicillin, chloramphenicol and trimethoprim-sulphamethoxazole (TMP-SMX) while the remainder had resistance to ampicillin and TMP-SMX. The predominant presenting symptoms were fever and diarrhoea. Elev

Typhoid fever infection - Antibiotic resistance and vaccination strategies: A narrative review.

Typhoid fever is a bacterial infection caused by the Gram-negative bacterium Salmonella enterica subspecies enterica serovar Typhi (S. Typhi), prevalent in many low- and middle-income countries. In high-income territories, typhoid fever is predominantly travel-related, consequent to travel in typhoid-endemic regions; however, data show that the level of typhoid vaccination in travellers is low. Successful management of typhoid fever using antibiotics is becoming increasingly difficult due to drug resistance; emerging resistance has spread geographically due to factors such as increasing travel

[Clinical thinking and decision making in practice. A patient with persistent fever].

A 20-year old man was admitted to hospital with fever and malaise after travel to India. He was soon found to have typhoid fever, caused by a multi-drug resistant Salmonella typhi. According to in-vitro resistance testing he was treated with cefotaxim, but fever and shivering were still present after 7 days of cefotaxim, with liver enzyme disturbances. The physicians caring for him started an extensive search for possible complications of typhoid fever, and they thought of the possibility of an alternative disease to explain the fever. When cefotaxim was stopped all symptoms and signs disappea

Afebrile vancomycin-induced delayed hypersensitivity suggestive of incomplete drug rash with eosinophilia and systemic symptoms syndrome: a case report.

BACKGROUND: Vancomycin-induced delayed hypersensitivity reactions are rare and typically accompanied by systemic symptoms such as fever, eosinophilia, and organ dysfunction, known as drug reaction with eosinophilia and systemic symptoms syndrome. However, nonsteroidal anti-inflammatory drugs can mask typical systemic signs, complicating diagnosis. CASE PRESENTATION: A 61-year-old Asian Taiwanese male patient developed widespread erythematous macules and papules, significant skin desquamation, pruritus, and eosinophilia after 25 days of vancomycin therapy initiated for suspected methicillin-re

Tolerability of teicoplanin in 117 hospitalized adults with previous vancomycin-induced fever, rash, or neutropenia: a retrospective chart review.

BACKGROUND: Vancomycin has reliable antibacterial activity against many gram-positive pathogens but is associated with many adverse events. Teicoplanin, another glycopeptide, is associated with fewer adverse events, but its use in patients with previous vancomycin-induced adverse reactions remains controversial. OBJECTIVES: The aims of this work were to evaluate the clinical characteristics of hospitalized patients with vancomycin-induced fever (ie, drug fever), rash, or neutropenia and to examine the tolerability of teicoplanin in these patients. METHODS: This was a retrospective review of
