An Open-Source Data Driven Hybrid Modeling System for Infectious Disease Surveillance and Early Warning.

INTRODUCTION: The increasing trend of globalization has led to a heightened risk of imported epidemics; however, existing surveillance systems remain fragmented and reliant on laboratory confirmation. We developed an open-source data-driven hybrid modeling system to provide earlier and more reliable alerts, designed to complement China's multipoint trigger early-warning framework. METHODS: This system integrates heterogeneous signals, including official epidemiology, digital traces, mobility, meteorology, and pathogen genomics, using semantic harmonization and a hybrid analytic stack. Seasona

HIRA: Heart Rate Interval based Rapid Alert score to characterize autonomic dysfunction among patients with sepsis-related acute respiratory failure (ARF).

Objective. To examine whether heart rate interval based rapid alert (HIRA) score derived from a combination model of heart rate variability (HRV) and modified early warning score (MEWS) is a surrogate for the detection of acute respiratory failure (ARF) in critically ill sepsis patients.Approach. Retrospective HRV analysis of sepsis patients admitted to Emory healthcare intensive care unit (ICU) was performed between sepsis-related ARF and sepsis controls without ARF. HRV measures such as time domain, frequency domain, and nonlinear measures were analyzed up to 24 h after patient admission, 1

Risk factors and preliminary prediction models for trauma-induced coagulopathy and 28-day mortality in severely injured patients: a retrospective observational cohort study.

BACKGROUND: Trauma-Induced Coagulopathy (TIC) is a critical complication in severe trauma patients associated with high mortality. Early identification is paramount for survival, yet reliable early warning indicators remain underutilized. This study aimed to identify independent predictors for TIC and mortality, and to establish robust prediction models to guide clinical decision-making. METHODS: A retrospective analysis was conducted on 128 severe trauma patients admitted to a single center. Patients were stratified into TIC (n = 44) and Non-TIC (n = 84) groups based on admission coagulation

Streptococcus oralis endocarditis leading to central nervous system infection in pregnancy.

This report describes a challenging obstetric case in which septic emboli from Streptococcus oralis endocarditis subsequently led to central nervous system infection. There were delays in diagnosis as the patient presented with non-specific symptoms of fever, diarrhoea and vomiting, initially suspected to be due to viral gastroenteritis and later SARS-CoV-2 infection. Antibiotics were commenced once gram positive cocci were isolated from a blood culture. The patient made no significant improvement despite antimicrobial therapy and subsequently developed a worsening headache and delirium. This

Physiological observation track and trigger system.

A patient observation chart was developed to combine physiological observation charting with the calculation and recording of an early warning score (EWS); this was named the Physiological Observation Track and Trigger System (POTTS). Reference to a table of instructions is no longer needed and the EWS can be calculated with minimal error. Increased frequency and accuracy of EWS recording assist in identifying patients at risk of deterioration and enhance patient safety.

Necrotising fasciitis in chronic lymphocytic leukaemia: a diagnostic challenge, management and learning points.

Necrotising soft tissue infection (NSTI), or necrotising fasciitis (NF), is a life-threatening, rapidly spreading bacterial infection affecting soft tissues. Mortality rates can be up to 70%, emphasising the need for early diagnosis and intervention. However, timely identification remains challenging, particularly in immunocompromised patients, due to nonspecific early signs. This case involves a patient who developed NF 12 days after R-CHOP (Rituximab, Cyclophosphamide, Hydroxydaunorubicin, Oncovin, Prednisolone) chemotherapy. Despite clinical and radiological evidence of NF and a high Labora

Benchmarking Time-Series Artificial Intelligence Architectures for Wearable Sensor-Based Fall Prediction: A Synthetic Data Simulation Framework.

Falls among older adults are a major cause of injury and loss of independence, yet most existing systems detect falls only after onset or provide very limited warning time. This study presents a synthetic benchmarking framework for early fall-risk prediction using multimodal wearable-inspired time-series data and compares classical and temporal machine learning architectures under a realistic evaluation protocol. A synthetic dataset of 1000 sequences was generated to emulate normal activity, slip events, and pre-fall instability using biomechanical, physiological, and contextual variables. Eig

Advances in Data-Driven Early Warning Systems for Sepsis Recognition and Intervention in Emergency Care: A Systematic Review of Diagnostic Performance and Clinical Outcomes.

Sepsis is a life-threatening condition, and early recognition in the emergency department (ED) is crucial for reducing mortality. However, traditional ED sepsis screening tools (e.g., Systemic Inflammatory Response Syndrome (SIRS) and quick Sequential Organ Failure Assessment (qSOFA)) often lack sensitivity for early detection. Machine learning (ML)-based early warning systems have been proposed to address this gap by analyzing complex clinical data in real time. To address this, we systematically reviewed studies (2015-2025) assessing ML-based sepsis warning systems in adult EDs. Databases se
