Ultrasound assessment of the elbow.

Ultrasonography of the elbow is a very helpful and reliable diagnostic procedure for a broad spectrum of rheumatic and orthopedic conditions, representing a possible substitute to magnetic resonance imaging for evaluation of soft tissues of the elbow. Musculoskeletal ultrasound (US) shows many advantages over other imaging modalities, probably the most important being its capability to perform a dynamic assessment of musculoskeletal elements with patient's partnership and observation during examination. In addition, ultrasonography is cost effective, easy available, and has excellent and multi

Hemophilic arthropathy of the elbow.

Hemophilia is a hereditary disease in which circulating levels of coagulation factors are lacking, resulting in a propensity toward bleeding. Intra-articular hemorrhages are a hallmark of hemophilia and may lead a cascade of cytokine elaboration and inflammatory-mediated changes, which ultimately result in cartilage loss and arthropathy. Diarthrodial joints, such as the knee, elbow, and ankle, are most commonly affected. This article highlights issues surrounding hemophilic arthropathy of the elbow and focuses on preventive measures, management strategies of the hemophilic elbow, and treatment

Osteochondrosis dissecans of the elbow.

This case shows a typical presentation of an osteochondrosis dissecans lesion on the capitellum, with associated intra-articular loose bodie(s), diagnosed on plain film. Osteochondrosis dissecans (OCD) is a rather common entity. Mostly the knee joint is involved. Only 5% of OCD lesions occur in the elbow joint. Possible causes of osteochondrosis dissecans include (repetitive) trauma (e.g. in throwing sports or gymnastics), ischemia, ossification defects, and genetic factors. Conventional radiography mostly has a low sensitivity to detect OCD lesions, and is often normal in early stages. CT is

Coronoid fractures of the elbow.

Coronoid fractures of the ulna are relatively uncommon, yet critical injuries to recognize. They often occur in association with elbow dislocations and play an important role in elbow instability. Historic recommendations are to fix all large coronoid fracture fragments, as well as small fracture fragments associated with instability. There is little data regarding management of small coronoid fracture fragments. The coronoid process acts as a bony buttress to prevent posterior dislocation and has three soft tissue insertions which lend stability as well: the anterior joint capsule of the elbo

[Arthroscopy of the elbow joint].

Clear indications have been recognised for arthroscopic surgery of the elbow since the 1980s. Good indications are loose bodies, mild or moderate restriction of the range of movement and early stages of rheumatoid arthritis. Cartilage diseases such as Panner disease or focal radial chondropathy can be treated by arthroscopic debridement and microfracturing. Impingement syndromes affecting the lateral compartment of the joint respond well to resection of synovial plicae. The technical demands of endoscopic arthrolysis procedures for joints with mild or moderate restriction of range of movement

Dislocations of the elbow in children.

A total of 33 children were treated for acute traumatic dislocation of the elbow between 1994 and 2002; 30 dislocations were posterior and three anterior. Eight children had a pure dislocation and 25 had an associated fracture of the elbow. Two had compound injuries. Two children had injury to the ulnar nerve, one to the radial nerve and one to the median nerve together with injury to the brachial artery. Twenty required open reduction. Complications included pseudarthrosis of the medial epicondyle in one child and loss of flexion and rotation of between 10 degrees and 30 degrees in ten others

Tuberculosis of the elbow in children.

Ten children with tuberculosis of the elbow treated over a 21-year period from 1979 to 1999 were reviewed retrospectively. At presentation, they had pain and swelling of the joint mainly due to synovitis, and the median symptom duration was 10 weeks. All had a decreased range of movement. All were treated with 9 months of antituberculous chemotherapy without synovectomy. The radiologic stage at presentation, rather than duration of symptoms, predicted outcome. Of the 9 patients with stage 1 or 2 disease (synovitis), 8 had an excellent or good outcome. One stage 2 patient had a posterior disloc

Fracture-dislocation of the elbow.

Recognition of the pattern of an elbow fracture-dislocation allows immediate knowledge of the treatment principles, pitfalls, and prognosis of the injury. Specific techniques for each injury component increase the surgeon's ability to restore stability to the elbow. When complications are anticipated and avoided or addressed expediently, it is possible to restore elbow function in spite of the complexity of these injuries.

Deformities of the elbow in achondroplasia.

Lack of full extension of the elbow is a common abnormality in patients with achondroplasia. We studied 23 patients (41 elbows) clinically and radiologically. Extension of the elbow was assessed clinically and the angle of posterior bowing of the distal humerus was measured from lateral radiographs. There was limited extension of the elbow in 28 (68.3%) and the mean loss of extension was 13.1 degrees. Posterior bowing of the humerus was seen in all elbows with a mean angle of 17.0 degrees. There was a positive correlation between these two measurements. Posterior bowing greater than 20 degrees

Osteochondritis dissecans of the elbow.

Osteochondritis dissecans of the elbow remains a difficult problem to manage in the young athlete. Though the etiology is unclear, a definite association between overuse with repetitive microtraumatic insult and OCD has been established. Early detection and appropriate treatment can provide the best chance for preventing an unfavorable outcome. In many cases, conservative treatment regimens will provide complete resolution of symptoms, return of function, and full recovery, including return to sports participation. Surgical indications should be recognized, however, and surgical management car

Fractures of the elbow and forearm.

Fractures and dislocations of the elbow usually occur secondary to indirect trauma. In the adult, fractures of the distal humerus almost always involve the condyles. Fractures of the radial head and neck may be subtle, and the appearance of secondary signs, such as the elevated fat pads from an elbow joint effusion, may be diagnostically useful. Dislocations of the elbow can be associated with fractures, such as those involving the ulnar coronoid process. In children, the presence of epiphyseal and apophyseal ossification centers can confuse the inexperienced observer in the setting of elbow t

The complex injury of the elbow.

The complex injury is characterized by a fracture and/or dislocation of the elbow in association with a serial injury of the upper extremity, or a severe soft tissue trauma , or a prolonged ischemia caused by vascular injury or compartment syndrom. They are defined as complex injuries because their treatment differs from that of a simple fracture implying that standardized concepts usually cannot be employed. The results of primary treatement show a high rate of complications. They are accompanied by functional deficits including pseudarthrosis and ankylosis. As a consequence, ulnar neuropathy

[Isolated dislocation of the elbow].

GOALS OF THE STUDY: To analyse the results of non surgical management of isolated dislocation (without any fractures) of the elbow in adult. MATERIAL AND METHODS: We reviewed with an average follow up of 36 months 22 patients (22 elbows) who had been treated conservatively after a first episode of posterior dislocation of the elbow. RESULTS: Nineteen patients (86%) reported an excellent or a good subjective result, although 64% suffered occasionally from pain. None had presented a recurrent dislocation. At physical examination, 27% had a restricted extension of the elbow of 10 degrees and mo

[Standardized examination of the elbow].

The elbow joint often reacts to different injury patterns of the intra- and extra-articular structures with uniform symptoms that can hardly be differentiated. Furthermore, several neural entrapment syndromes around the elbow should be taken into consideration, because they make the differential diagnosis even more difficult. Cervical nerve root and brachial plexus irritations can also be responsible for pain, which is subjectively transferred to the elbow. The purpose of the study was to improve the etiological differentiation by introducing a new standardized physical examination program. Be

Overuse Injuries of the Elbow.

In brief Sports-related overuse injuries brief of the elbow occur most often in people involved in throwing and racket sports. Common injuries include medial and lateral epicondylitis, radial tunnel syndrome, ulnar nerve entrapment, triceps and biceps tendinitis, and conditions related to valgus extension overload: ulnar collateral ligament tears, lateral compartment osteochondral injuries, and posterior olecranon impingement. Accurate diagnosis requires an understanding of elbow anatomy and a careful physical examination. Most of these injuries respond to activity modification, nonsteroidal a

Medial epicondylitis of the elbow.

Ninety-five cases of medial epicondylitis are reported in 83 patients; 90% were related to work and only 10% to sport or leisure activities. Most recovered with conservative treatment. Operation was needed in 12%, which compared with under 4% of patients with lateral epicondylitis over the same period. The results of open release of the common flexor origin were good, with only one exception.
