OBJECTIVE: Because of technical obstacles in controlling current spread to adjacent peripheral nerve, eye movements evoked by activation of the otolith organs have not been investigated in detail compared to eye movements evoked by activation of the canal organs. OBJECTIVE: To determine whether automatic slow eye movement (SEM) analysis performs comparably to standard sleep onset criteria at the multiple sleep latency test (MSLT) and at the maintenance of wakefulness test (MWT) in patients with obstructive sleep apnea syndrome (OSAS).

Some attempts have been made to screen sleep apnea syndrome (SAS). In this study, we performed daytime polysomnography (DPSG) as a means of screening SAS. 33 patients (28 males and 5 females, a mean age of 50.7 years) who were seen with the complaints of sleep-wake disorders, snoring and breathing disturbance in their nocturnal sleep were subjects for the screening. An extraordinary eye movement was seen in a vegetative patient. His eyeballs were exotropic in the primary position and showed dissociated nystagmus which appeared alternately in each eye every few seconds. He also had palatal myoclonus quite asynchronous with the nystagmus. To our knowledge, there has been no such nystagmus documented in the literature. We report the new nystagmus with his EOG and brain MRI.

Based on the anatomical and clinical findings in the very rare cases reported, recent experimental data, and the results of oculography, the authors consider that this condition merits definition as an individual entity. It is usually associated with other disorders of oculomotility which may mask clinical signs, and oculography is necessary to establish the diagnosis. Terminologically, it would appear logical to retain the terms; abduction internuclear ophthalmoplegia (and not, posterior), and adduction internuclear ophthalmoplegia (and not, anterior).
