Anti-MPO-ANCA-positive microscopic polyangiitis following subacute bacterial endocarditis.

Although infectious agents such as Staphylococcus aureus have been implicated in the pathogenesis of Wegener's granulomatosis, the role of bacterial infections in the pathogenesis of other types of small-vessel vasculitides associated with antineutrophil cytoplasmic antibodies (ANCA) is less clear. We describe a patient who developed a non-granulomatous necrotising small vessel vasculitis and perinuclear ANCA (p-ANCA) directed against myeloperoxidase (MPO) after recurrent episodes of bacterial endocarditis due to Staph. aureus. Although cytoplasmic ANCA (c-ANCA) directed against proteinase 3 h

Right ventricular and septal anomalies complicated by subacute bacterial endocarditis.

We report the case of a 31-year-old woman with no history of heart disease. She came to the hospital with fever, dyspnea, palpitation, and edema of the lower extremities. She was found to have aortic, mitral, and pulmonary valve insufficiency, and the initial diagnosis was subacute bacterial endocarditis. At surgery, we replaced the aortic and mitral valves with mechanical prostheses and the pulmonary valve with a bioprosthesis. The prostheses were soaked intraoperatively with fluconazole and the heart chambers were irrigated with povidone-iodine to prevent infection by bacteria and fungi. We

Subacute bacterial endocarditis masquerading as type III essential mixed cryoglobulinemia.

An adult man presented severely ill with vasculitis of his lower extremities and with impaired kidney function. After detailed evaluation at a local hospital, a diagnosis of essential type III cryoglobulinemia was made. High-dose steroid and cyclophosphamide therapy was begun. The patient improved dramatically. However, 6 wk later when his steroid dose was reduced to 30 mg daily, vasculitis recurred. Intensifying his immunosuppressive therapy only worsened his condition. He was than transferred to the Ohio State University Medical Center for consideration for plasmapheresis for the presumed es

Cardiovascular infections: bacterial endocarditis of oral origin. Pathogenesis and prophylaxis.

The diagnosis infective endocarditis describes infection of the endocardial surface of the heart and indicates the presence of micro-organisms in the lesion. In most cases, the heart valves are affected, but the disease can also occur on septal defects or on the mural endocardium. The disease has been classified as acute or subacute based on the progression of the untreated disease. The acute form has a fulminant course with high fever and leukocytosis with death in less than 6 weeks. It is most often associated with infections caused by Staphylococcus aureus, Streptococcus pneumoniae or Strep

Rheumatoid factors in subacute bacterial endocarditis and other infectious diseases.

Rheumatoid factors (RF) occur during the course of various infections such as leprosy, infective endocarditis, tuberculosis, trypanosomiasis, visceral larva migrans, infectious mononucleosis, influenza A, hepatitis A or cytomegalovirus. When first described it seemed logical to assume that host-self-immunization with autologous immune complexes provided the initial stimulus for RF production. Subsequently extensive characterization of bacterial, parasitic and viral Fc receptors has suggested an alternative explanation for rheumatoid factor associated with infections. It seems possible that pat

Echocardiographic diagnosis of ruptured aortic valve leaflet in bacterial endocarditis.

Aortic valve rupture, secondary to aortic valve endocarditis, was diagnosed echocardiographically and closely followed preoperatively. The ruptured left coronary cusp of the aortic valve was seen as dense irregular echoes, located anteriorly during ventricular diastole, and protruding into the left ventricular outflow tract in an otherwise normally appearing aortic valve. These echocardiographic findings, when correlated with changes in the clinical status of the patient, prompted immediate cardiac catheterization and aortic valve replacement. Early echocardiographic detection of abnormal aort

Bacterial endocarditis following aortic valve replacement. Clinical and pathologic correlations.

The clinical and pathologic findings in three patients with bacterial infection at the sites of prosthetic aortic valves are described. Each presented typical clinical features of acute bacterial endocarditis followed by the sudden development of severe and ultimately fatal aortic regurgitation. Pathologically the infectious process was characterized by extensive necrosis of the aortic annulus, disruption of the aortic wall, and the formation of abscesses in the periaortic tissues. In two patients the prosthetic valve was partially detached from the annulus, whereas in the other it was totally

Non-bacterial thrombotic endocarditis in ovarian cancer: A systematic review.

BACKGROUND: Non-Bacterial Thrombotic Endocarditis (NBTE) is characterized by the formation of sterile vegetations on the heart valves, often due to an underlying hypercoagulable state in the setting of malignancy, inflammation, or autoimmune disease. NBTE is a rare condition with a reported incidence rate of between 0.9% and 1.6%. Among gynecologic malignancies, NBTE is predominantly observed in ovarian cancer. A comprehensive review of the literature was conducted to understand the association between NBTE and ovarian cancer. METHODS: A search of PubMed/MEDLINE was conducted from database in

Non-bacterial thrombotic endocarditis: a rare presentation and literature review.

A 66-year-old woman with a remote history of breast cancer and prior tobacco use presented to the hospital with chest pain. She was found to have an elevated troponin consistent with a diagnosis of a non-ST segment elevation myocardial infarction (NSTEMI). A left heart catheterisation revealed non-obstructive coronary disease, and subsequent transthoracic and transoesophageal echocardiograms demonstrated vegetations on both the mitral and aortic valves. Multiple blood cultures showed no growth raising suspicion for non-bacterial thrombotic endocarditis (NBTE). A CT of the chest, abdomen and pe

Cardioembolic stroke secondary to non-bacterial endocarditis in Wegener disease.

Wegener granulomatosis is a systemic vasculitis that mainly affects the upper and lower respiratory tract and the kidneys. The presence of an ischemic stroke in this disease is very rare. A 40-year-old man, smoker with cavitated lesions in both lungs, and inflammation in the nasal mucosa and vocal cords developed an ischemic stroke in the left middle cerebral artery, with the etiological study showing non-bacterial endocarditis. Non-bacterial endocarditis can appear in patients with Wegener granulomatosis, with this being the first case of this cardioembolism reported in this disease.

[Internuclear ophthalmoplegia: initial and isolated clinical findings in bacterial endocarditis].

Unilateral internuclear ophthalmoplegia become manifest by homolateral paralysis of ocular adduction and contralateral nystagmus. Principal causes are multiple sclerosis and vertebro-basilar insufficiency. We report a case in which this signal appeared as the first clinical manifestation of bacterial endocarditis and turned up to be the only neurological finding.

Aortic root abscess complicating bacterial endocarditis. Demonstration by computed tomography.

A 68 year old man with an aortic valve prosthesis was admitted to hospital with Staphylococcus aureus endocarditis. Despite antibiotic treatment he continued to be pyrexial. Computed tomography identified a probable abscess between the root of the aorta and the left atrium. The presence of an abscess in this location was subsequently confirmed at operation. Computed tomography is a useful additional diagnostic method for identifying this potentially lethal complication of bacterial endocarditis.

Surgical management of left ventricular-aortic discontinuity complicating bacterial endocarditis.

Successful hemodynamic repair of left ventricular-aortic discontinuity complicating bacterial endocarditis in 2 patients was achieved using a composite valve-woven Dacron tube graft. The prosthetic valve was sutured without tension into the remaining aortic annulus, ventricular muscle, and base of the aortic leaflet of the mitral valve. Use of the composite graft allows adequate debridement of the abscess, restores ventricular-aortic continuity, excludes the abscess wall from systemic pressure, and does not require saphenous vein coronary bypass. Total exclusion of the aortic root, as describe
