Clinical cell-based versus cell-free regenerative endodontics: clarification of concept and term.

There is no consensus on the true meaning of clinical regenerative endodontics, and there is confusion over the concept and the term. Commonly used terms include revitalization and revascularization. The clinical methods for endodontic revitalization procedures and the tissue engineering concept differ depending on whether there is exogenous delivery of cells - called cell therapy, or not. Here, in this review, the difference is clarified by emphasizing the correct terminology: cell-free versus cell-based regenerative endodontic therapy (CF-RET versus CB-RET). The revitalization procedures pra

Epigenetic regulation of dental pulp stem cells and its potential in regenerative endodontics.

Regenerative endodontics (RE) therapy means physiologically replacing damaged pulp tissue and regaining functional dentin-pulp complex. Current clinical RE procedures recruit endogenous stem cells from the apical papilla, periodontal tissue, bone marrow and peripheral blood, with or without application of scaffolds and growth factors in the root canal space, resulting in cementum-like and bone-like tissue formation. Without the involvement of dental pulp stem cells (DPSCs), it is unlikely that functional pulp regeneration can be achieved, even though acceptable repair can be acquired. DPSCs, d

An Insight into Advanced Approaches for Photosensitizer Optimization in Endodontics-A Critical Review.

Apical periodontitis is a biofilm-mediated disease; therefore, an antimicrobial approach is essential to cure or prevent its development. In the quest for efficient strategies to achieve this objective, antimicrobial photodynamic therapy (aPDT) has emerged as an alternative to classical endodontic irrigation solutions and antibiotics. The aim of the present critical review is to summarize the available evidence on photosensitizers (PSs) which has been confirmed in numerous studies from diverse areas combined with several antimicrobial strategies, as well as emerging options in order to optimiz

Root Canal Stripping: Malpractice or Common Procedural Accident-An Ethical Dilemma in Endodontics.

Root canal stripping is defined as an oblong, vertical perforation that appears especially in the middle section of curved root canals during endodontic treatments with nickel-titanium (Ni-Ti) instruments. Its occurrence may drastically affect the outcome of the treatment, transforming a common otherwise efficient endodontic procedure into a complication such as tooth extraction. In order to discuss the ethical and legal consequences, two cases of dental strip perforations are herewith presented. Due to the existence of risk factors for dental strip perforation, experience of the clinician and

Expression, Regulation and Therapeutic Implications of Nod-Like Receptors in Endodontics: A Narrative Review.

BACKGROUND: Nucleotide-binding oligomerization domain (NOD)-like receptors (NLRs) are critical intracellular pattern recognition receptors that orchestrate innate and adaptive immune responses. Their dysregulated activation is mechanistically linked to the pathogenesis of numerous inflammatory diseases. In endodontics, pulpitis and apical periodontitis represent highly prevalent inflammatory conditions, primarily caused by bacterial invasion, which significantly impact patients' quality of life. Current studies demonstrate that NLR-mediated immunity plays a significant role in driving pulpal i

Primum non nocere - The effects of sodium hypochlorite on dentin as used in endodontics.

UNLABELLED: The medical literature is replete with the maxim 'primum non nocere', cautioning health care providers to avoid doing any harm to human subjects in their delivery of medical care. Sodium hypochlorite (NaOCl) is a well-established irrigant for root canal treatment because of its antimicrobial and organic tissue remnant dissolution capability. However, little is known about the deleterious effect of this strong oxidizing agent on the integrity of human mineralized dentin. Iatrogenically-induced loss of dentin integrity may precipitate post-treatment root fracture and has potential me

Clark's rule vis à vis the buccal object rule: its evolution & application in endodontics.

Originally, Clark's rule was used to identify the position of unerupted or supernumerary teeth (a concept known as oral localization) prior to orthodontic tooth movement or surgical removal. However, the versatility of this rule's principles and concepts are also highly applicable to endodontic diagnosis and treatment. While contemporarily known by many different names, such as the "buccal object rule" and the "cone beam-shift technique," the importance of this rule in endodontics was not recognized in the literature until late in the 20th century, despite historical evidence demonstrating its
