Type II endoleak (T2EL), the most common complication after endovascular aortic aneurysm repair (EVAR), remains a leading cause of reintervention due to persistent retrograde flow from patent inferior mesenteric artery (IMA) and lumbar arteries (LAs). The influence of specific anatomical features of these vessels on the hemodynamic environment remains poorly understood. This study evaluated two key anatomical characteristics: branch vessel count and luminal diameter. Eight post-EVAR models were constructed. Computational fluid dynamics (CFD) simulations were performed to quantify key hemodynam

BACKGROUND: Abdominal aortic aneurysm (AAA) is a life-threatening aortic dilation with high rupture mortality. Endovascular aneurysm repair (EVAR) is the preferred minimally invasive treatment, but post-EVAR endoleaks remain a major challenge.
AIMS: To investigate the predictive value of suprarenal angle, infrarenal angle, and proximal neck diameter for endoleak following EVAR in AAA patients and their relationships.
METHODS: In this retrospective study, 643 consecutive patients undergoing elective EVAR for non-ruptured AAA were grouped into endoleak (n = 179) and non-endoleak (n = 464) grou

BACKGROUND: Type 2 endoleak (T2EL) remains the most frequent complication after endovascular aneurysm repair (EVAR) for abdominal aortic aneurysm and is associated with sac enlargement and late adverse events. Preemptive embolization of aortic side branches has been introduced to reduce its incidence. This study aimed to evaluate the mid-term outcomes of EVAR with preemptive side branch embolization and to identify factors associated with persistent T2EL.
METHODS: We retrospectively reviewed 217 patients who underwent EVAR with preemptive embolization of the inferior mesenteric artery and lum

OBJECTIVE: This study aims to assess the feasibility and effectiveness of physician-modified fenestrated stent grafts (PMEGs) in treating type 1a endoleak after conventional thoracic endovascular aortic repair (TEVAR) in aortic arch pathologies.
METHODS: Patients who developed a type 1a endoleak after conventional TEVAR were included in the study. They underwent treatment with fenestrated PMEGs, which involved single or double fenestration for the supra-aortic trunks.
RESULTS: Twenty patients were treated with PMEGs between October 2018 and November 2023. Among them, 25% received single fene

BACKGROUND: The occurrence of type II endoleaks after endovascular repair of aortic aneurysm has gradually gained increasing attention. We present a case of a patient with an expanding aneurysm after thoracic endovascular aortic repair (TEVAR) for a type II endoleak, in which successful direct ligations of the intercostal artery were performed using a sac incision without cardiopulmonary bypass (CPB) or graft replacement.
CASE PRESENTATION: A 62-year-old male patient, previously treated with TEVAR for a descending thoracic aortic aneurysm, presented with ongoing chest discomfort. Based on the

INTRODUCTION: Endovascular aneurysm repair for abdominal aortic aneurysm is superior to open surgery regarding perioperative morbidity and mortality. Complications such as endoleaks represent a significant source of secondary intervention. Vascular graft infection is another serious complication found in 0.2 to 1 % of series. We hereby describe a case of a rapidly progressive aortic infection by Cutibacterium acnes following a percutaneous embolization procedure for a type II endoleak.
CASE PRESENTATION: A 75-year-old man presented with a fever along with lower back and buttock pain five days

BACKGROUND: Endovascular abdominal aortic aneurysm repair (EVAR) is the most frequently used treatment for aneurysm in abdominal aorta. The endoleak after EVAR causes the aneurysm sac to remain enlarged and risk for rupture.
AIMS: The purpose of the study was to assess the efficacy of strategies and techniques for endoleak treatment.
METHODS: This study was a single center retrospective study of 30 patients who had kinds of endoleak. The 30 patients were from a cohort of 597 patients who received EVAR from the Secondary Xiangya Hospital, Central South University between Jan 2014 to Dec 2021,

OBJECTIVES: Our goal was to analyse the relationships between aortic surface irregularity and a type 1 endoleak (T1EL) after a thoracic endovascular repair using the Najuta fenestrated stent graft.
METHODS: The patients who were treated using the Najuta stent graft for an intact aortic arch aneurysm at the Saitama Cardiovascular and Respiratory Center between June 2013 and June 2017 were analysed retrospectively. The primary end point was the occurrence of a T1EL. The gap between a virtual aorta and the patient's aortic wall at the stent graft fixation was calculated over the whole circumfere

The type IIIb endoleak a rare complication of endovascular aneurysmal repair caused by endoprosthesis problem leading to deteriorating rupture, which is one of the major reasons for lifelong imaging surveillance. We report a case of an early-onset type IIIb endoleak from a tear in main body bifurcation (flow divider) area like crotch point tearing of pants following additional intervention to reduce immediate type Ia endoleak. Successful treatment of the endoleak was achieved by surgical conversion.

(1) Successful endovascular repair for abdominal aortic aneurysms is based on the complete exclusion of the aneurysm sac from the systemic circulation. Type Ia endoleak (ELIA) is defined as the persistent perfusion of the aneurysm sac due to incomplete proximal sealing between aorta and endograft, with a consequent risk of rupture and death. Endoleak embolization has been sporadically reported as a viable treatment for ELIA. (2) A systematic literature search in PubMed of all publications in English about ELIA embolization was performed until February 2022. Research methods and reporting were

We report on an octogenarian, who was hospitalized with acute hemoptysis. Computed tomography angiography revealed a monstrously large thrombosed aortic aneurysm in the left thoracic cavity, completely displacing the lung. Eighteen years ago, the patient had suffered traumatic rupture of the descending aorta loco typico. Surgical exploration revealed a large calcified aneurysmal sac, which had perforated into the left lung. Pneumonectomy was performed, and hemoptysis did not reoccur.

BACKGROUND: Aortic sac hygroma and concealed endoleaks (EL) after endovascular aneurysm repair (EVAR) of abdominal aortic aneurysm needs particular attention with aggressive management as they are associated with rapid sac expansion and rupture risk. However, they can be erroneously reported as type IV or V EL with supine computed tomography (CT) scans, leading to delay in management. Therefore, we describe a novel diagnostic technique, 'Prone contrASt enHanced computed tomography Angiography' (PASHA), to document concealed EL METHODS: We present eight case descriptions with continuous sac exp

This report describes an elderly severe aortic stenosis (AS) patient, who had a history of coronary artery bypass grafting and endovascular repair for an abdominal aortic aneurysm (AAA). Type II endoleak with enlargement of AAA was diagnosed and ligation of inferior mesenteric artery (IMA) was recommended. Because aortic valve replacement (AVR) was high risk, we planned transcatheter aortic valve implantation (TAVI). Considering risks of IMA ligation under dual antiplatelet therapy, increased blood pressure after TAVI, and general anesthesia, we performed combined TAVI and IMA ligation. TAVI c

PURPOSES: Endovascular abdominal aortic aneurysm repair (EVAR) is an increasingly used method of repairing abdominal aortic aneurysm (AAA). However, the treatment of persistent type II endoleak is still a controversial issue. Five cases are reported here in which we performed open surgical repair of growing aneurysm due to persistent type II endoleak.
METHOD: Totally 128 EVAR cases were retrospectively reviewed, which were operated in our hospital from April 2008 to October 2013. These cases were followed by periodical contrast-enhanced computed tomography (CT) after EVAR. When persistent typ

A best evidence topic in vascular surgery was written according to a structured protocol. The question addressed was whether, in patients with persistent type 2 endoleak (T2EL) post-endovascular abdominal aortic aneurysm repair (EVAR), intervention is associated with better outcomes than observation. Four hundred and eighty-three papers were found using the reported search, of which 12 represented the best evidence to answer the clinical question. The authors, journal, date and country of publication, patient group studied, study type, relevant outcomes and results of these papers are tabulate

We describe a case of type-2 bronchial artery endoleak after endovascular aortic repair of Kommerell diverticulum (KD) involving right-sided aortic arch and aberrant left subclavian artery (LSA). A 68-year-old woman underwent an endovascular repair of KD with an aberrant LSA in our hospital. Follow-up computed tomography (CT) at 6 months after the procedure showed an endoleak. Digital subtraction angiography revealed a type-2 endoleak from a bronchial artery, but no type-1 or type-2 endoleak from the aberrant left subclavian artery. We performed coil embolization of the KD and the left subclav

Endovascular repair continues to pose a formidable technical challenge in the cases of aneurysm, dissection, and proximal type I endoleak involving the aortic arch. During the process of covering the aortic arch by stent graft to achieve better sealing, maintaining blood flow to the vital supra-aortic branches is difficult. We present a case of successful endovascular treatment of secondary type I endoleak by a double-chimney technique in a 36-year-old woman who had previously undergone a complicated descending aortic dissection repair. This endovascular technology might offer a new option to

INTRODUCTION: proximal perigraft endoleak (PPE) and graft migration are associated with significant morbidity and mortality. Objective data establishing correlation between neck anatomy and these complications are lacking. The aim of this study was to analyse the anatomy of the neck in order to find which variables were significantly associated with PPE and graft migration.
METHODS: one hundred and eighty-four patients underwent endovascular repair (EVR) of infrarenal AAA using an in-house custom-made stent graft (Gianturco stents plus Dacron). Thirty-one patients had PPE and fifteen had graf
