Endolymphatic sac tumors (ELSTs) are rare tumors arising from the epithelium of the endolymphatic sac and duct that can be either sporadic or associated with von Hippel-Lindau (VHL) disease. We report a case of a VHL patient with histologically proven residual ELST who underwent Ga DOTATATE PET/CT showing increased activity (SUVmax, 6.29) by the ELST. The presented case of a VHL-associated ELST with increased Ga DOTATATE uptake indicates cell-surface expression of somatostatin receptors by this tumor, suggesting the potential application of somatostatin receptor imaging using Ga DOTA-conjugate

Medical treatment for Meniere's disease is effective in controlling vertigo for approximately 85% of patients. However, when disabling vertigo continues, surgical therapy is indicated. Several surgical approaches are performed to control the symptoms of peripheral vestibular disorders refractory to medical measures, each procedure having many technical variations. Surgery is usually reserved for patients with disabling vertigo. Here, the authors discuss surgical options for vertigo control in Meniere's disease and review the literature on outcomes of these management options. The authors discu

The inner ear is capable of rapidly mounting an immune response that can ultimately lead to cochlear degeneration and permanent hearing loss. The role of the endolymphatic sac in this immune process is not clear. In order to investigate the cytokine expression of cells within the endolymphatic sac, a secondary inner ear immune response to keyhole limpet hemocyanin (KLH) was created in mice. The animals were sacrificed 3-48 h and 7 days following initiation of the immune response. The cochleas and endolymphatic sacs were assayed by immunocytochemistry for IL-1beta, TNFalpha, and IL-6. In recent years the benefit of endolymphatic sac surgery has been controversial. Earlier investigations were restricted to the patient's postoperative symptoms, but detailed analysis of postoperative vestibular function was not reported. Nineteen patients who had undergone endolymphatic sac surgery were followed-up for 6 months on up to 16 occasions. Twelve of the 19 patients showed continuous improvement of canal function beginning no earlier than 2 weeks after the operation. No vestibular compensation was observed after simple mastoidectomy. The postoperative symptoms were not proper indicat

OBJECTIVE: Delayed endolymphatic hydrops (DEH) and Ménière's disease (MD) are both hydropic ear disease. In MD patients, the endotypes of MD are classified based on pathological types of the endolymphatic sac (ES), which can be distinguished by a radiological indicator: the angular trajectory of the vestibular aqueduct (ATVA). Studies have exhibited that MD patients of different pathological types of ES manifest different clinical characteristics. Here, we aimed to investigate the prevalence of each ATVA category in DEH and MD, and to further explore the clinical significance of ATVA in ipsila

Dizziness is a prevalent clinical complaint, and determining the underlying etiology can be challenging. Posterior fossa masses have been observed to cause vertigo, with presentations very similar to Meniere's disease. In this report, we describe a case of a patient who presented with recurrent episodes of Meniere's-like vertigo in the setting of a posterior fossa meningioma, who underwent surgical resection of the tumor with significant improvement postoperatively. We postulate that these symptoms were caused by secondary endolymphatic hydrops and were able to demonstrate improvement in high-

INTRODUCTION AND IMPORTANCE: Endolymphatic sac tumor (ELST) is a rare, histologically benign tumor with potentially aggressive behavior. Its management depends on the stage and extent of the disease.
CASE PRESENTATION: We report the case of a 34-year-old male, with no significant medical history, presenting with progressive right-sided hearing loss, high-pitched tinnitus, and recurrent vertigo. Radiological assessment with CT and MRI revealed a hypervascular lesion centered on the petrous apex, with erosion of the internal auditory canal. A retrolabyrinthine surgical approach was performed. BACKGROUND: Meniere's disease (MD) is characterized by idiopathic endolymphatic hydrops (ELH). Frequent vertigo attacks is the most disabling symptom of MD.
OBJECTIVE: This study evaluated the efficacy of triple semicircular canal occlusion combined with endolymphatic sac decompression in the treatment of frequent vertigo in patients with MD.
METHODS: Eleven patients with complete medical records were included in this study conducted from May 2021 to April 2022. All patients were enrolled to undergo triple semicircular canal occlusion (TSCO) with endolymphatic sac decompression (ESD). Variou

We report the case of a 42-year - old female with familiar form von Hippel-Lindau disease (VHL) and recurrent endolymphatic sac tumour (ELST), which was presented like non-homogenous, solid and cystic expansion of the left petrous temporal bone. Histologically, there was found lamellae of bone with adjacent ligament and with papillary projections with fibrovascular core. The papillae were lined by a single layer of cuboidal epithelium with hyperchromatic and lightly pleomorphic nuclei. Sporadically, small cystic formations with eosinophilic, PAS positive secretion were noted. Imunohistochemica

OBJECTIVE: The standard evaluation of patients with suspected Menière's disease (MD) includes initial imaging to rule out tumors of the temporal bone. Few guidelines, however, advocate sequential imaging. We propose that sequential imaging may reveal other etiologies of auditory and vestibular symptoms as demonstrated in a patient with an endolymphatic sac tumor that was originally diagnosed Menière's after initial imaging.
PATIENTS: One patient with MD and initially unremarkable imaging. Repeat imaging several years after diagnosis after additional symptoms demonstrated interval development

Objective. Endolymphatic sac tumor (ELST) is a rare neoplasm arising from the intrapetrous portion of the endolymphatic sac, either isolated or in association with the von Hippel-Lindau disease. We report a sporadic case of ELST with an overview of the literature and a discussion of clinic-radiological, histopathologic, and surgical findings. Case Report. A young woman presented with a progressive hearing loss in the left ear. Otoscopy showed a reddish, bleeding hypotympanic mass. CT demonstrated an expansile lytic mastoid lesion extending to the middle ear, with bone erosion. MRI confirmed a
