NINJ1 as a novel biomarker of intestinal injury in necrotizing enterocolitis.

BACKGROUND: Necrotizing enterocolitis (NEC) remains a lethal gastrointestinal emergency in neonates with an elusive etiology. Ninjurin-1 (NINJ1) mediates plasma membrane rupture during lytic cell death, yet its role in NEC remains undefined. This study investigated the clinical relevance of NINJ1 in NEC progression and evaluated its utility as a biomarker for disease severity. METHODS: We conducted a prospective observational study quantifying serum NINJ1 levels via ELISA in 68 neonates with NEC and 42 gestational age-matched controls without infection. The NEC cohort was stratified by treatme

Necrotizing Enterocolitis in a Term Newborn With Hypoxic-Ischemic Encephalopathy.

Necrotizing enterocolitis (NEC) predominantly affects preterm infants and remains uncommon in term neonates. When it occurs in full-term newborns, it usually reflects underlying vulnerability or cumulative perinatal risk factors. We report the case of a term male newborn (40 + 1 weeks' gestation) who developed NEC in the context of multiple risk factors, including perinatal asphyxia with hypoxic-ischemic encephalopathy managed by therapeutic hypothermia, suspected early-onset neonatal infection requiring empirical antibiotics, and formula feeding. On day 4 of life, the infant presented with si

Lipopolysaccharide-binding protein as a biomarker in the diagnosis of necrotizing enterocolitis.

OBJECTIVES: Necrotizing enterocolitis (NEC) is a life-threatening gastrointestinal disease of neonates. This study aimed to identify Lipopolysaccharide-binding protein (LBP) as a potential biomarker for the diagnosis of NEC through multi-omics analyses and validation. METHODS: We analyzed LBP expression differences in NEC using transcriptomic data from our cohort (intestinal tissue samples from NEC cases and controls) integrated with a public gene expression dataset (GSE46619). Correlations between LBP and common clinical biomarkers (C-reactive protein (CRP), platelet count (PLT), white blood

A predictive model for prognosis in infants with surgical necrotizing enterocolitis.

BACKGROUND: Surgical intervention could improve the prognosis of infants with necrotizing enterocolitis (NEC), but it's also an independent risk factor for severe complications and death. This study aimed to establish a nomogram to predict the prognosis of infants with surgical NEC. METHODS: A total of 189 infants with surgical NEC were enrolled from January 2013 to May 2024. Univariate and multivariate logistic regression analyses were conducted to identify independent risk factors. Then, a nomogram was constructed based on the risk factors. RESULTS: This study demonstrated that gestational

Exclusive Human Milk Diets and the Reduction of Necrotizing Enterocolitis.

BACKGROUND: Necrotizing enterocolitis (NEC) is common in preterm infants, especially infants less than 32 weeks gestation. Mortality from NEC is 7% and occurs in 1 out of 1000 preterm infants. Studies have shown the efficacy of an exclusive milk from mother diet in decreasing rates of NEC and associated mortality. PURPOSE: To evaluate the effectiveness of an existing exclusive human milk diet (EHMD) protocol on the incidence of NEC in extremely premature infants. EHMD, for the purposes of this project is defined as breast milk of mother, with or without human milk-based fortifier. METHODS: A

Echogenic Bowel as an Indicator of Necrotizing Enterocolitis in a Term Newborn.

A 3.5-kilogram infant was born at 40 weeks gestation with an uncomplicated delivery. Prenatal ultrasounds showed echogenic bowel and a ventricular septal defect (VSD), of no clinical significance. Abdominal radiographs showed pneumatosis at 21, 36, and 48 hours of life (HOL). She was treated for necrotizing enterocolitis (NEC) with intravenous antibiotics and parenteral nutrition for 7 days, before working up on feeds and discharging home with breast milk. The only prenatal finding in this case was hyperechogenic bowel, which is a soft marker and often disregarded in the absence of other signs

The Role of Dietary Fats in the Development and Prevention of Necrotizing Enterocolitis.

Necrotizing enterocolitis (NEC) is a significant cause of mortality and morbidity in preterm infants. The pathogenesis of NEC is not completely understood; however, intestinal immaturity and excessive immunoreactivity of intestinal mucosa to intraluminal microbes and nutrients appear to have critical roles. Dietary fats are not only the main source of energy for preterm infants, but also exert potent effects on intestinal development, intestinal microbial colonization, immune function, and inflammatory response. Preterm infants have a relatively low capacity to digest and absorb triglyceride f

Necrotizing Enterocolitis in Neonates With Hyperinsulinemic Hypoglycemia Treated With Diazoxide.

The most common cause of persistent hypoglycemia in the neonatal period is hyperinsulinism. Severe, refractory hypoglycemia resulting from hyperinsulinism can lead to significant brain injury and permanent cognitive disability. Diazoxide is the first-line and only US Food and Drug Administration-approved, pharmacologic treatment for refractory hyperinsulinism. In recent years, the use of diazoxide in neonates with persistent hyperinsulinemic hypoglycemia has increased in the United States. Known adverse effects of diazoxide include fluid retention, hypertrichosis, neutropenia, thrombocytopenia

Bowel Ultrasound for the Diagnosis of Necrotizing Enterocolitis: A Meta-analysis.

Radiographic evaluation for necrotizing enterocolitis (NEC) often yields nonspecific findings. Bowel ultrasound (BUS) provides additional information beyond that of abdominal radiographs and may be helpful in the diagnosis of NEC in neonates. We systematically reviewed and aggregated existing literature to get a better estimate of diagnostic accuracy of BUS in the diagnosis of NEC. A literature search was performed using PubMed, Embase, and Cumulative Index to Nursing and Allied Health Literature to identify studies in which infants with clinically suspected NEC were evaluated using BUS. Protective effects of amniotic fluid in the setting of necrotizing enterocolitis.

Necrotizing enterocolitis (NEC) is the most common life threatening condition affecting preterm infants. NEC occurs in 1-5% of all neonatal intensive care admissions and 5-10% of very low birth weight infants. The protective role of human breast milk (BM) has been well established. It has also been shown that amniotic fluid (AF) and BM have many similarities in terms of presence of growth and other immune-modulatory factors. This finding led to the initial hypothesis that AF may exert similar protective effects against the development of NEC, as does BM. Multiple studies have elucidated the pr

Maternal pre-eclampsia as a risk factor for necrotizing enterocolitis.

OBJECTIVE: Similar pro-inflammatory responses are present in pre-eclampsia (PE) and necrotizing enterocolitis (NEC). We hypothesized that maternal PE is an independent risk factor for the development of NEC. METHODS: A retrospective database of all live births (2008-2011) at a tertiary center was constructed. Infant and maternal characteristics were gathered. Babies born to mothers with or without PE were compared. Data were analyzed using Mann-Whitney U, Pearson's χ(2), binary logistic regression and relative risks. RESULTS: Incidence of NEC was 1.5% in non-PE and 4.6% in the PE group (p <

Primary laparotomy is effective and safe in the treatment of necrotizing enterocolitis.

INTRODUCTION: There is ongoing controversy regarding the surgical treatment of necrotizing enterocolitis (NEC) in infants with extremely low birth weight (ELBW). We hypothesize that primary laparotomy is a safe and effective treatment for all infants with surgical NEC. METHODS: We retrospectively compared a group of ELBW infants (<1,000 g, group A, n = 39) with preterm infants (≥1,000 g, group B, n = 18) with surgical NEC during a 4-year period (10/2008-09/2012). Indications for emergency operation were Bell stages IIIa or IIIb. The main outcome measure was survival. Data were analyzed using

[Role of laparoscopy in premature infants with suspicion of necrotizing enterocolitis].

INTRODUCTION: The indication of surgery in necrotizing enterocolitis (NEC) can be difficult in the absence of pneumoperitoneum or peritonitis. We propose laparoscopy for early diagnosis of intestinal perforation or necrosis in order to avoid unneccessary laparotomies. MATERIAL AND METHODS: A new protocol was proposed which included diagnostic and therapeutic laparoscopy in cases of uncertain surgical ECN indication. Subsequently, a prospective study of the 7 patients who underwent laparoscopy was performed, analyzing pathophysiological effects in the premature, technical difficulties of the p

Necrotizing enterocolitis in a term infant with coarctation of the aorta complex.

Necrotizing enterocolitis (NEC) sometimes occurs in term infants with congenital heart disease. This article reports a rare case of a term infant with coarctation of the aorta complex who developed NEC on the 8th day after birth. Spontaneous closure of the ductus arteriosus in the 1st week of life may cause intestinal ischemia and hypoxia with resultant NEC.
