Epithelial cystic neoplasms of the epididymis are rare, the commonest is papillary serous cystadenoma. Recently, few cases of epididymal nonpapillary serous cystadenoma (NPSC) were reported. They have unique histomorphologic and immunohistochemical features that help distinguish them from other cystic lesions of the paratestis. All the reported cases presented as a single unilateral epididymal cyst. We report a unique example and, to our knowledge, the first case of multiple bilateral NPSC of the epididymis. Moreover, it showed a pigmented epithelium.

Cystatin-related epididymal spermatogenic (CRES) is the defining member of a reproductive subgroup within the family 2 cystatins of the cystatin superfamily of cysteine protease inhibitors. CRES is synthesized and secreted by the initial segment of the epididymis and is present in the sperm acrosome, suggesting roles in sperm maturation and fertilization. We have previously demonstrated that CRES is present within the epididymal lumen as monomeric (14 and N-glycosylated 19-kd forms) as well as sodium dodecyl sulfate-sensitive and sodium dodecyl sulfate-resistant high-molecular mass complexes.

Paratesticular leiomyosarcomas are rare. We report two cases of leiomyosarcoma of the epididymis diagnosed and treated in a regional community hospital in Canada in 2009 and 2010. Both patients were males of ages 58 and 75, respectively. They presented with painless masses which, on ultrasonography, appeared to be solid masses demonstrating blood flow. Carboxylesterases (CEs) represent a multigene family of serine-dependent enzymes. Male-dependent CEs are over-expressed in the male reproductive tract of different animal species (bivalve mollusks, fruit-flies, and mammals). Here, a novel rat epididymis-specific gene named Ces7 was cloned and characterized. BACKGROUND: Mammalian spermatozoa become fully motile and fertile during transit through the luminal fluid of the epididymis. At least 200 proteins are present in the epididymal lumen, but the potential roles of these luminal proteins in male fertility are unknown. Investigation of the function of these proteins will elucidate the mechanism of sperm maturation, and also provide new drug targets for male contraception. OBJECTIVE: Adenomatoid tumor of the epididymis is unfrequent, benign, with no malignant outcomes described.
METHODS: We report five cases, with patient's ages varying from 31 to 76 years, and tumor sizes from 6 to 30 mm. All patients underwent surgery with excision of the mass, some of them after several months of growing. In one of the cases the evolution reached six years.
RESULTS: None of the cases presented recurrence or bad outcome after surgery. Pathology confirmed the benign adenomatoid character in all cases.

OBJECTIVE: Tumors of the epididymis are rare. They are unusually benign and adenomatoid tumors are the most frequent. Report of two cases of this kind of tumor of the epididymis.
METHODS/RESULTS: We report two cases of adenomatoid tumor of the epididymis diagnosed at our hospital during last year. Two males, 35 and 54 years old respectively asked for urology consultation about a palpable scrotal mass. They didn't have any other symptoms. Imaging techniques revealed a solid epididymal mass. OBJECTIVE: To report the case of an epididymal and spermatic cord metastasis of a colon adenocarcinoma.
METHODS/RESULTS: We present the case of a 67 years old man, who presents several months after subtotal left colectomy for a left colon adenocarcinoma (pT3 NoMo), with a symptomatic right palpable testicular tumour. Right orchiectomy was done, demonstrating colon adenocarcinoma metastasis.
CONCLUSIONS: Epididymal and spermatic cord metastases are very infrequent and they usually are found incidentally after prostate cancer orchyectomy.

OBJECTIVE: The purpose of this study was to determine the frequency of an enlarged epididymis in pediatric patients with testicular torsion and to determine whether an altered epididymis might be a helpful ancillary sonographic sign of testicular torsion.
MATERIALS AND METHODS: The sonograms of 50 pediatric patients (age range, neonate-17 years) with testicular torsion were retrospectively reviewed for the size, appearance, and blood flow of the epididymis. Medical records were reviewed for surgical and pathologic findings and to determine whether testicular salvage had been possible.

We present a case of a solitary fibrous tumour, located at the epididymis, in a 65-year-old man, presented with a scrotal mass. Ultrasound and MRI of the scrotum revealed a paratesticular mass, with rich vascularity, arising in the left epididymis. Radiological findings were non-specific and the patient underwent surgery.
