A case report of cervical spine epidural abscess caused by brucellosis.

BACKGROUND: Brucellosis is an endemic zoonotic infection in Saudi Arabia, with thousands of cases reported annually. Although musculoskeletal involvement is common, spinal complications - particularly cervical spondylodiscitis with epidural abscess - are rare and often difficult to diagnose due to nonspecific symptoms. Delayed recognition may lead to neurological compromise requiring urgent intervention. CASE DESCRIPTION: A 55-year-old farmer with regular exposure to cattle and a history of brucellosis presented with progressive right-sided weakness and neck pain. Magnetic resonance imaging r

Spinal Epidural Abscess Complicated by Saddle Pulmonary Embolism: A Case Report.

CASE: A 36-year-old woman with intravenous drug use presented with back pain, paraplegia, and urinary incontinence. MRI revealed a ventral cervical spinal epidural abscess from C5 to T1 with cord signal changes. CT angiography showed a large saddle pulmonary embolism with right heart strain. Multidisciplinary management included initiation of venoarterial extracorporeal membrane oxygenation (VA-ECMO) without anticoagulation, followed by urgent C6 corpectomy and C5 to T1 anterior decompression and fusion. Anticoagulation and thrombectomy were performed postoperatively. CONCLUSION: This rare ca

Beyond Risk Factors: Spinal Epidural Abscess in a Young, Healthy Patient.

Epidural abscess is a rare complication of neuraxial techniques, which, when left unnoticed, can lead to significant neurological deficits and poor outcomes. Identification of patients at high risk and the conduct of a strict aseptic technique are some of the measures that play an important role in epidural abscess prevention. Prompt recognition and treatment of epidural abscesses are essential. Close monitoring in the perioperative period for new-onset clinical signs and symptoms is of major importance. When an epidural abscess is suspected, early laboratory assessment and magnetic resonance

A rare case of Streptobacillus moniliformis epidural abscess requiring neurosurgical decompression.

BACKGROUND: Streptobacillus moniliformis is the primary causative agent of rat bite fever, an infectious disease transmitted through contact with rats through bites, scratches, or exposure to excrement. Before this report, only two instances of spinal epidural abscess (SEA) due to S. moniliformis infection have been documented. We present the case of a 76-year-old male who developed a cervical SEA secondary to S. moniliformis infection, requiring neurosurgical decompression of the spinal cord. CASE DESCRIPTION: A 76-year-old male presented to the emergency department with bilateral shoulder a

Spinal Epidural Abscess: A Review of Presentation, Management, and Medicolegal Implications.

Spinal epidural abscess (SEA) is a rare condition associated with significant morbidity and mortality. Despite advances in diagnostic medicine, early recognition of SEAs remains elusive. The vague presentation of the disease, coupled with its numerous risk factors, the diagnostic requirement for obtaining advanced imaging, and the necessity of specialized care constitute extraordinary challenges to both diagnosis and treatment of SEA. Once diagnosed, SEAs require urgent or emergent medical and/or surgical management. As SEAs are a relatively rare pathology, high-quality data are limited and th

Transpedicular Approach for Ventral Epidural Abscess Evacuation in the Cervical Spine.

OBJECTIVE: Spinal epidural abscess may require prompt surgical intervention. Ventral cervical abscesses pose a particular challenge regarding the approach for surgical evacuation. The aim of this article was to describe the technical nuances of a posterior transpedicular cervical approach for evacuation of a ventral epidural abscess. METHODS: After a standard laminectomy, a foraminotomy was performed to identify the exiting nerve root. Then the medial aspect of the pedicle below the nerve was drilled. This allowed the insertion of a dissector to reach the ventral epidural space and drain the

[A case report of epidural abscess caused by acute frontal sinusitis].

Epidural epidural abscess is usually secondary to acute frontal sinusitis and osteomyelitis of the frontal bone. Nose-derived intracranial connplications are less common than otogenic ones, and infections caused by nasal and sinus trauma, foreign bodies and surgery are more common than those caused by inflammation alone. The frontal lobe abscess caused by frontal sinusitis is more common in brain abscess with nasal intracranial complications.

Gas-Containing Cervical Epidural Abscess Accompanying Bacterial Meningitis in an Adult.

Gas-containing spinal epidural abscesses are uncommon. Moreover, acute spinal epidural abscesses rarely complicate bacterial meningitis in adults. Here, we report a rare case of a gas-containing cervical epidural abscess accompanying bacterial meningitis. In spite of aggressive fluid and continuous antibiotic therapy after the isolation of Streptococcus anginosus and Streptococcus constellatus in the cerebrospinal fluid cultures, the patient showed remaining motor dysfunction and bladder involvement. Our experience suggests that the effort to prevent neurologic deterioration by emergent surgic

"Dry tap" during spinal anaesthesia turns out to be epidural abscess.

We report a case of "dry tap" during spinal anaesthesia in a patient posted for incision and drainage of lower limb with cellulitis. When the patient was being given sub-arachnoid block (SAB) for regional anaesthesia, it turned out to be a case of pyogenic ilio-psoas abscess extended up to the paravertebral and epidural spaces. The causative organism was Staphylococcus aureus. This is probably the first case reported when epidural abscess is diagnosed during SAB.

Pediatric lumbar epidural abscess combined with cauda equina syndrome: case report.

Pyogenic epidural abscess is a very rare disease. Once it occurs, it promptly progresses and can cause neurologic paralysis. Mean age of onset has been reported to be 57 years. Here we report making a diagnosis of pyogenic lumbar epidural abscess accompanying cauda equina syndrome in a 10-year-old girl. We treated this case successfully with surgical drainage and antibiotics. We report our case with a review of the literature.

Spinal epidural abscess in hemodialysis patients: a case series and review.

Spinal epidural abscess (SEA) is a rare but potentially devastating condition. We noticed an increase in the number of cases of SEA, with the majority in hemodialysis (HD) patients. This prompted a retrospective chart review of all cases of SEA admitted from 2000 to 2005 and a literature search of similar cases. We identified 19 SEA cases treated at Long Island College Hospital during this 6-year period, of which six were on HD: four were dialyzed via catheter, one via arteriovenous fistula, and in one the method of dialysis was not documented. Four patients had bacteremia with Staphylococcus

Cervical epidural abscess secondary to aorto-duodenal fistula: a case report.

Although cervical epidural abscess is rare, it should be strongly suspected in any patient with unexplainable neck pain and fever, especially when the patient has a predisposing factor for this infectious process. The authors report a case of cervical epidural abscess in a 39-yr-old man with an aorto-duodenal fistula, which complicated the interposition of artificial graft for abdominal aortic aneurysm rupture, which had undertaken 40 months before. Timely detection and intervention rendered him a full neurological recovery. This extremely rare case is presented with a literature review.

Epidural abscess: a delayed complication of esophageal stenting for benign stricture.

A case of anterior cervical epidural abscess associated with perforation of an endoscopically placed esophageal stent is presented. Although delayed esophageal perforation is a known complication of endoscopic stenting, no cases presenting with epidural abscess have yet been reported. The increasing application of endoscopic stenting for benign esophageal strictures provides greater opportunity for this type of delayed complication.

[Cervical spinal epidural abscess caused by methicillin-resistant Staphylococcus aureus (MRSA)].

Epidural abscess is an uncommon infectious disease. The cervical spine is the least frequent site of spinal epidural abscess. It has been reported that early diagnosis and surgical treatment prevent neurological deficit, but it is difficult to diagnose this disease clinically. We presented a rare case of cervical epidural abscess caused by MRSA. A 54-year-old man was admitted to our hospital because of acute renal failure and hepatitis. He was treated with hemodialysis via the femoral route. His renal function recovered but high fever continued. MRSA was identified from the AV shunt catheter.

Epidural abscess of the cervical spine: MR findings in five cases.

Cervical epidural abscess is an uncommon infectious process of the spinal epidural space. Although this disorder is often unsuspected clinically, the patient's signs and symptoms may suggest other diagnoses that frequently lead to an MR examination. We retrospectively reviewed the MR examinations of five patients with surgically proved cervical epidural abscess in order to assist in the diagnosis of this clinically elusive disorder. Each epidural abscess was evaluated for MR signal intensity, location, extent, delineation, and enhancement pattern. We assessed the spinal cord for compression an

Changing concepts in spinal epidural abscess: a report of 29 cases.

A review of our recent experience with spinal epidural abscess (SEA) reveals several important changing concepts in diagnosis, etiology, management, and outcome. All cases of SEA seen by our service from August 1975 to July 1989 were reviewed retrospectively, and 29 patients were identified (19 men and 10 women, aged 13-78 years). Abscesses were located in the lumbar (n = 21), thoracic (n = 7), and cervical (n = 1) epidural spaces. Gram-positive organisms were the infectious agent in 72% of the cases, and Staphylococcus aureus was the sole agent in 45%; other agents were Gram-negative aerobes
