Diagnostic testing for vocal fold paralysis: survey of practice and evidence-based medicine review.

OBJECTIVES/HYPOTHESIS: Vocal fold paralysis continues to be a dominant topic in laryngology. Although the majority of cases can be attributed to a known etiology, a significant number of patients present without a clear precipitating event. Over 1,500 studies regarding vocal fold paralysis exist in the medical literature, although only a small percentage report on the use of serum or radiographic testing for the evaluation of idiopathic paralysis. Despite this, patients are routinely subjected to diagnostic evaluation to investigate the underlying cause. To characterize contemporary practice,

Microbiome-Based Therapies in Ulcerative Colitis: Mechanisms, Clinical Evidence, and a Precision-Medicine Framework.

Microbiome-based therapies are reshaping the therapeutic landscape for ulcerative colitis (UC), offering new avenues for disease management beyond conventional immunomodulatory and biologic treatments. UC remains a chronic, relapsing condition with significant unmet clinical needs, as many patients fail to achieve sustained remission or experience adverse effects with current therapies. The gut microbiome has emerged as a central contributor to UC pathogenesis, influencing epithelial barrier integrity, immune homeostasis, and metabolic signaling. Interventions such as fecal microbiota transpla

Artificial intelligence to enhance research integrity in evidence-based medicine: toward scalable trustworthiness assessment.

Fraudulent, falsified, and otherwise unreliable clinical research threatens the foundations of evidence-based medicine because systematic reviews, meta-analyses, and clinical practice guidelines depend on the trustworthiness of the studies they include. Retractions are increasing faster than publication output and often occur only after substantial delay, allowing problematic trials to be cited, pooled, and incorporated into downstream clinical recommendations. Recent evidence shows that retracted randomized trials have contaminated thousands of meta-analyses and hundreds of guideline document

Neuro-reproductive toxicity and carcinogenicity of 1-bromopropane: studies for evidence-based preventive medicine.

Bromopropane was introduced commercially as an alternative to ozone-depleting and global-warming solvents. The identification of 1-bromopropane neurotoxicity in animal experiments was followed by reports of human cases of 1-bromopropane toxicity. In humans, the most common clinical features of 1-bromopropane neurotoxicity are decreased sensation, weakness in extremities, and walking difficulties. Moreover, decreased cognition, abnormal findings on brain magnetic resonance imaging, urinary incontinence, and numbness in the perineal area have also been described in workers exposed to 1-bromoprop

Counseling in fetal medicine: evidence-based answers to clinical questions on morbidly adherent placenta.

Although the incidence of morbidly adherent placenta (MAP) has risen progressively in the last two decades, there remains uncertainty about the diagnosis and management of this condition. The aim of this review is to provide up-to-date and evidence-based answers to common clinical questions regarding the diagnosis and management of MAP. Different risk factors have been associated with MAP; however, previous Cesarean section and placenta previa are the most frequently associated. Ultrasound is the primary method for diagnosing MAP and has a good overall diagnostic accuracy for its detection. Wh

Evidence-based medicine for polypropylene mesh use compared with native tissue vaginal prolapse repair.

A review of the current medical literature for the use of polypropylene (PP) mesh for vaginally performed prolapse repair, including only those studies reporting prospective, randomized, controlled trials compared with native tissue repairs was undertaken. Five full manuscript publications and 4 studies still in abstract form were all consistent with PP mesh producing better anatomical results for cystocele repair, but when functional results in terms of the patient's quality of life are considered, no significant difference is found between PP mesh and native tissue repairs. PP mesh use resul

Evidence-based interventional pain medicine according to clinical diagnoses. 16. Complex regional pain syndrome.

Complex regional pain syndrome (CRPS), formerly known as reflex sympathetic dystrophy is a pain syndrome with an unclear pathophysiology and unpredictable clinical course. The disease is often therapy resistant, the natural course not always favorable. The diagnosis of CRPS is based on signs and symptoms derived from medical history and physical examination. Pharmacological pain management and physical rehabilitation of limb function are the main pillars of therapy and should be started as early as possible. If, however, there is no improvement of limb function and persistent severe pain, inte

EASDnet: Empowering human-centered evidence-based medicine through an evidence and attention-based spatial disparity network for discriminative colorectal cancer histopathological screening and attribution.

PURPOSE: Accurate preoperative staging of colorectal cancer is critical to guide treatment decisions, including eligibility for R0 resection, to reduce recurrence and improve patient survival. However, conventional imaging evaluation depends on human experience, leading to subjectivity and diagnostic uncertainty in clinical interpretation. Therefore, an objective, evidence-based quantitative analysis tool is essential to assist clinicians. This study introduces and validates an evidence-based medicine (EBM) deep learning model that addresses these limitations by providing reliable, automated p

Hierarchies of evidence applied to lifestyle Medicine (HEALM): introduction of a strength-of-evidence approach based on a methodological systematic review.

BACKGROUND: Current methods for assessing strength of evidence prioritize the contributions of randomized controlled trials (RCTs). The objective of this study was to characterize strength of evidence (SOE) tools in recent use, identify their application to lifestyle interventions for improved longevity, vitality, or successful aging, and to assess implications of the findings. METHODS: The search strategy was created in PubMed and modified as needed for four additional databases: Embase, AnthropologyPlus, PsycINFO, and Ageline, supplemented by manual searching. The Second American Society of Regional Anesthesia and Pain Medicine Evidence-Based Medicine Assessment of Ultrasound-Guided Regional Anesthesia: Executive Summary.

OBJECTIVES: In 2009 and again in 2012, the American Society of Regional Anesthesia and Pain Medicine assembled an expert panel to assess the evidence basis for ultrasound guidance as a nerve localization tool for regional anesthesia. METHODS: The 2012 panel reviewed evidence from the first advisory but focused primarily on new information that had emerged since 2009. A new section was added regarding the accuracy and reliability of ultrasound for determining needle-to-nerve proximity. Jadad scores are used to rank study quality. Grades of recommendations consistent with their level of evidenc

[Evidence-based medicine advances in de-escalation strategies for local surgical treatment of breast cancer].

With the continuous optimization of systemic treatment regimens and the advancement of imaging assessment technologies, the overall survival rate of early-stage breast cancer has been steadily improving, providing an evidence-based basis for exploring de-escalation of local surgical treatment. This article systematically reviews five pathways for de-escalation of local surgical treatment in breast cancer and their key clinical evidence: selective exemption from breast surgery after neoadjuvant therapy by verifying pathologic complete response through imaging combined with vacuum-assisted biops

Second Generation Antipsychotics and Cardiovascular Adverse Effects: Developing Evidence-Based Recommendations for Primary Care Medicine.

BACKGROUND: Second-generation antipsychotic medications (SGAs) are often used by primary care physicians (PCPs) to treat multiple psychiatric diagnoses. SGAs have been connected to a number of adverse effects, including cardiovascular disease. Currently, there are no published evidence-based recommendations addressing SGAs and cardiotoxicity that are directed toward PCPs. This project aims to fill this gap. METHODS: Relevant search terms related to SGAs and cardiovascular disease were identified and then used to search databases (PubMed, PubMed Central, and AccessMedicine). Research studies o

Natural product-derived therapies for treating drug-resistant epilepsies: From ethnopharmacology to evidence-based medicine.

ETHNOPHARMACOLOGICAL RELEVANCE: Epilepsy is one of the most prevalent neurological human diseases, affecting 1% of the population in all age groups. Despite the availability of over 25 anti-seizure medications (ASMs), which are approved in most industrialized countries, approximately 30% of epilepsy patients still experience seizures that are resistant to these drugs. Since ASMs target only limited number of neurochemical mechanisms, drug-resistant epilepsy (DRE) is not only an unmet medical need, but also a formidable challenge in drug discovery. AIM: In this review, we examine recently appr

From evidence-based medicine to digital twin technology for predicting ventricular tachycardia in ischaemic cardiomyopathy.

Survivors of myocardial infarction are at risk of life-threatening ventricular tachycardias (VTs) later in their lives. Current guidelines for implantable cardioverter defibrillators (ICDs) implantation to prevent VT-related sudden cardiac death is solely based on symptoms and left ventricular ejection fraction. Catheter ablation of scar-related VTs is performed following ICD therapy, reducing VTs, painful shocks, anxiety, depression and worsening heart failure. We postulate that better prediction of the occurrence and circuit of VT, will improve identification of patients at risk for VT and b
