BACKGROUND: Neurological complications are among the most serious adverse events in patients receiving extracorporeal membrane oxygenation (ECMO). Brain injury during ECMO is associated with increased mortality, long-term neurological deficits and impaired quality of life. Current evidence regarding brain function monitoring and management in ECMO patients is scattered, varies in methodological quality and remains controversial in certain areas. Furthermore, standardised recommendations for monitoring and management across different ECMO configurations and patient populations have yet to be es

TOPIC: Fecal impaction, a review of the literature.
PURPOSE: To review the definition, diagnosis, causes, signs and symptoms, and treatment of fecal impaction.
SOURCES: Published literature.
CONCLUSIONS: While a great deal has been published on fecal impaction, there is limited research. Clarification of when constipation changes to impaction is ambiguous. Causes of fecal impaction are multiple, and people can be affected at all ages. Signs and symptoms of and problems associated with fecal impaction are varied. Research-based treatment of fecal impaction is very limited, and nonresearch ba

Older adults admitted to post-acute care often are frail with multiple comorbidities, making them particularly vulnerable to developing delirium. The purpose of this quality improvement project was to create, implement, and evaluate an NP-led, evidence-based delirium prevention protocol in a skilled nursing facility (SNF). The incidence of new-onset delirium was assessed pre- and postimplementation; following implementation, no patients were diagnosed with new-onset delirium, and nurses and certified nursing assistants reported changes to their practice. Based on project design, the approximat

Numerous international clinical guidelines for management of psoriasis have recently been published. We evaluated the quality of guidelines published between 2006 and December 2009 using the Appraisal of Guidelines Research and Evaluation (AGREE) instrument. Eight guidelines from five separate working groups fulfilled inclusion criteria and were evaluated. Four used the standards established by the AGREE instrument in the process of development of their guidelines. Each of the guidelines uniformly received high domain scores (i.e., > 90%) for scope and purpose (range of 94-100%), and clarity a

BACKGROUND: Sacred cows (SC) are old habits in practice, considered routine and above dispute, regardless of evidence to the contrary.
PURPOSE: This is the first known report that aims to conduct a systematic evaluation of practices that have been described in the literature as SC and strategies for planned implementation of evidence-based practices (EBP).
METHODS: A large, complex, academic medical center department of nursing compared SC to EBP. Nurses systematically reviewed and rated the degree to which current practices adhered to best-evidence versus SC. This initiative, "Sacred Cow: G

BACKGROUND: Preventable adverse events remain a significant challenge in US health care systems despite decades of patient safety initiatives. Hospital-acquired conditions such as falls, infections, and pressure injuries contribute to patient morbidity, mortality, and the cost of care. Clinical Nurse Specialists (CNSs) play a critical role in translating evidence into practice and implementing system-level strategies that reduce preventable harm.
PURPOSE: The purpose of this paper is to highlight how CNSs identify risks for preventable adverse events and lead evidence-based prevention strateg

: Postoperative urinary retention (POUR) is the inability to void when the bladder is full after surgery. It is a common complication in postoperative patients, especially in patients undergoing spinal surgery. At our institution, patients who were discharged from the postanesthesia care unit (PACU) to the inpatient surgical unit typically had bladder distention and a bladder volume of more than 450 mL. In an effort to address this situation, an interprofessional group of advanced practice RNs and physicians formed a team, reviewed the existing literature, examined the PACU nursing practice gu

Community nurses face many challenges when trying to practice evidence-based, person-centred care. Ongoing concerns regarding the impact of the 2013 Francis Report ( Ford and Lintern, 2017 ) suggest that individualised and holistic care is an impossible dream, one made harder when the client appears uncooperative. This paper presents a case study that sets out how some of these challenges were met in a potentially difficult situation experienced by a student nurse and her mentor in practice, in which the student was supported to further examine and explore issues that may have influenced the s

Transfusion-related reactions cause unwanted interruptions in blood-product administration and potential complications for patients. The most common reactions are febrile nonhemolytic transfusion reactions (FNHTRs) and allergic transfusion reactions (ATRs). The presence of leukocytes in blood products has been associated with these reactions, and efficacy of leukoreduction in minimizing FNHTRs and ATRs has recently been investigated. In addition, premedication with acetaminophen and diphenhydramine is the most widely used practice in minimizing FNHTRs and ATRs, yet the benefit of this is not s

Command hallucinations are relatively common in voice hearers and are taken seriously because of the potential threat to self and others. Many variables mediate the relationship between hearing commands and acting on them. This article describes the implementation of the Harm Command Safety Protocol and the Unpleasant Voices Scale to respond to command hallucinations to harm in the context of the dissemination of a multisite, evidence-based behavioral management course for patients with auditory hallucinations.

Radiation dermatitis, or radiodermatitis, is a significant symptom caused by radiation therapy for the treatment of cancerous and noncancerous conditions. Radiodermatitis can negatively affect patients' physical functioning and quality of life. The Oncology Nursing Society coordinated a Putting Evidence Into Practice (PEP) project team to develop a PEP resource summarizing current evidence for the management of patients with radiodermatitis. Oncology nurses play an important role in educating, assessing, and monitoring patients for this symptom. Many common nursing interventions for radioderma

Many children's nurses have significant contact with children who have breathing difficulties and should be using systematic criteria to assess their nursing needs. Children's nurses do not appear to follow systematic criteria but are strongly influenced by the medical model and this may be detrimental to holistic assessment and the development of nursing diagnoses based on nursing needs. As nursing, along with other disciplines, develops evidence-based practice, children's nurses should develop evidence for best practice mindful of children's views, parental needs and the education of future

The current quagmire in the regulation of CNS practice is largely due to confusion about definitional approaches to advanced nursing practice. Establishing clarity about the nursing focused and medical focused definitions of advanced practice can greatly assist in developing and implementing appropriate levels of regulation for the CNS role and scope of practice. It has been my experience that state boards of nursing want to do their very best in helping to protect the public while not creating insurmountable problems that essentially put APRNs such as CNSs out of work and raise restraint of t

OBJECTIVE: The goals of the meta-analysis were to investigate the overall effectiveness of cognitive behavioral group therapy (CBGT) for depression and relapse prevention in depression from 2000 to 2010, and to investigate how the variables (episode, residual symptoms, group size, control group, group manual, therapist experience, therapy frequency, session length, and take-home assignment) of a CBGT study could affect the effect size.
METHOD: This study collected actual study designs sought of CBGT for depression published from 2000 to 2010. These studies were then cross-referenced using Med

This paper discusses current evidence-based guidelines for the management of faecal incontinence in the elderly. The key aspects of maintaining dignity, quality of life and self-esteem are addressed within a presentation of good assessment, prevention, management and treatment principles. Particular focus is given to the added problems that faecal incontinence may bring to the elderly person suffering with this debilitating, and often embarrassing condition. The increased risk to skin integrity is discussed, with reference to available guidance to help nurses manage the risk of skin breakdown
