BACKGROUND: Paraplegia is a serious complication of descending and thoracoabdominal aortic aneurysms (dTAAs and TAAAs) surgery. Motor evoked potentials (MEPs) enable monitoring the functional integrity of motor pathways during dTAA and TAAA surgery. Although MEPs are sensitive to temperature changes, there are few human data on changes of MEPs during mild and deep hypothermia. Therefore, we investigated changes of MEPs in deep hypothermic circulatory arrest (DHCA) in dTAA and TAAA surgery.
METHODS: Fifteen consecutive patients undergoing dTAA and TAAA surgery using DHCA were enrolled. MEPs we

PURPOSE: Monitoring motor evoked potentials (MEPs) has been recognized as a highly reliable method to detect intraoperative spinal cord ischemia (SCI) in aortic repair. However, the data regarding the sensitivity and specificity of MEPs for predicting postoperative paraplegia are limited. We retrospectively assessed the value of intraoperative MEP amplitudes for predicting postoperative paraplegia.
METHODS: The medical records of 44 patients were reviewed. A train-of-five stimulation was delivered to C3-C4, and MEPs were recorded from the abductor pollicis brevis and the tibialis anterior mus

STUDY DESIGN: Case report.
OBJECTIVE: To report a case of false-negative intraoperative motor-evoked potentials (MEP) that developed paraplegia after surgery.
SUMMARY OF BACKGROUND DATA: Although several false-negative results have been reported with somatosensory-evoked potentials, there is no report noted with MEP. Therefore, several authors have preferred using MEPs as a gold standard in neuromonitoring.
METHODS: We report a case of false-negative MEP during the scoliosis surgery which is the first report showing false-negative MEPs during operation.
RESULTS: A 15-year-old girl with sev

Six infants with obstetric brachial palsy, ranging from 4 to 7 months of age, were investigated. One was suspected of having extensive brachial plexus lesions and five were suspected of having a unilateral lesion of both roots C5 and C6. All were referred to our center to investigate the possibility for reconstructive surgery. In all infants, even at this age, transcranial magnetic stimulation resulted in motor evoked potentials (MEP) in the biceps (in one, in the brachioradial) muscles. Averaging could not be done because of the intraindividual variation in latency. The MEP was easier to reco

We have used magnetic resonance imaging (MRI) and motor evoked potentials (MEPs) for monitoring disease progression within the CNS of the Twitcher mouse, the murine model for globoid cell leukodystrophy (GLD). GLD is a lysosomal storage disorder, resulting from galactocerebrosidase deficiency, causing central and peripheral myelin impairment, leading to death, usually during early infancy. Neuroradiological, electrophysiological, and pathological parameters of myelin maturation were evaluated in Twitcher mice between postnatal days 20 and 45. Healthy controls showed a gradual-appearance MRI T2

1. Growing evidence points toward involvement of the human motor cortex in the control of the ipsilateral hand. We used focal transcranial magnetic stimulation (TMS) to examine the pathways of these ipsilateral motor effects. 2. Ipsilateral motor-evoked potentials (MEPs) were obtained in hand and arm muscles of all 10 healthy adult subjects tested. They occurred in the finger and wrist extensors and the biceps, but no response or inhibitory responses were observed in the opponens pollicis, finger and wrist flexors and the triceps. 3. The production of ipsilateral MEPs required contraction of t

Clinical and electrophysiological findings in six patients with locked-in syndrome are reported. Motor evoked potentials (MEPs) after magnetic stimulation of the motor cortex were absent in four patients, none of whom recovered clinically. In two patients, MEPs could be obtained from the severely paretic limbs and almost full motor recovery followed. Somatosensory evoked potentials were altered in four of the patients, and brainstem auditory evoked potentials were altered in two of four patients examined, showing a clinically unsuspected tegmental involvement. The EEG showed a predominance of

Spinal cord evoked potentials were elicited in cats by transcranial electrical stimulation with electrodes on the vertex and hard palate. Vertex motor evoked potentials (V-MEP) were also recorded. An extracellular microelectrode recording technique was then used to analyze the results by isopotential mapping. The relationship between the distribution of field potentials and the stimulation polarity was studied using the field potential distribution of the V-MEP in the lower thoracic spinal cord that had been represented on the isopotential maps. The first negative wave of the V-MEP showed maxi

1. To clarify the generators of human short-latency somatosensory evoked potentials (SEPs) thought to arise in sensorimotor cortex, we studied the effects on SEPs of surgical excision of somatosensory or motor cortex in humans and monkeys. 2. Normal median nerve SEPs (P20-N30, N20-P30, and P25-N35) were recorded from the cortical surface of a patient (G13) undergoing a cortical excision for relief of focal seizures. All SEPs were abolished both acutely and chronically after excision of the hand area of somatosensory cortex. Similarly, excision of the hand area of somatosensory cortex abolished

Motor evoked potentials (MEP) were examined in 50 patients with space-occupying lesions of the brain stem and spinal cord. MEP findings were correlated with the motor status as established by clinical examination. The results clearly show the high sensitivity of MEP for detection of motor deficits: 17 recordings (77%) from the thenar muscle and 42 (84%) from the anterior tibial muscle correlated correctly with the clinical motor status. False-positive results were found in 5 (23%) thenar recordings and 8 (16%) and anterior tibial recordings. False-negative correlation was not observed. The hig

Transcranial electrical stimulation motor-evoked potential (TES-MEP) has been widely used to monitor major motor pathways in cranial and spinal surgeries. However, the results of TES-MEP might be strongly influenced by anesthetic agents and muscle relaxants. To compensate for this effect, a technique using compound muscle action potentials of the abductor pollicis brevis (APB-CMAP) evoked by median nerve stimulation has recently been reported. In this article, we adopted the transcranial electrical stimulation motor-evoked potential of facial muscles (TES-FMEP) instead of APB-CMAP as a referen

BACKGROUND/OBJECTIVE: Myogenic motor-evoked potentials (myogenic MEPs) and Direct waves (D-waves) are essential for monitoring motor pathways during intramedullary spinal cord tumor (IMSCT) resections. However, the diagnostic accuracy of alarm criteria and the prognostic value of reversible versus irreversible changes remain unclear. This is the first study addressing this gap.
METHODS: Following PRISMA-DTA guidelines, this review searched PubMed, MEDLINE, and OVID (1985-2024) for studies on intraoperative myogenic MEPs and/or D-waves with postoperative motor outcomes in IMSCT resections.

Background/Objectives: Total intravenous anesthesia (TIVA) typically combines propofol and remifentanil. Remifentanil exerts minimal influence on motor evoked potential (MEP), whereas propofol partially reduces MEP amplitude. Remimazolam, a novel agent, is a component of TIVA. However, evidence of remimazolam on MEP is limited. We aimed to compare the effects of propofol and remimazolam, combined with remifentanil, on relative MEP depression. Methods: Using a crossover design, 18 patients undergoing spine surgery were randomly assigned to receive either propofol or remimazolam as the first age

BACKGROUND: In transcranial magnetic studies (TMS), motor-evoked potentials (MEPs) recorded from the tongue provide important insights into corticobulbar excitability and neuromuscular control. However, their clinical and research utility remains limited due to anatomical and technical challenges, particularly those affecting electrode stability and signal quality.
OBJECTIVE: This study aimed to compare TMS-derived outcomes obtained with traditional surface electrodes and novel suction-based electrodes.
METHODS: Nineteen healthy adults participated in a repeated-measures, cross-over design.

Motor-evoked potential (MEP) monitoring is commonly used in children. MEP monitoring in infants is difficult due to smaller signals requiring higher stimulation voltages. There is limited information on the effect of different anesthetics on MEP monitoring in this age group. This case series describes the effect of different anesthetic regimens on MEP monitoring in infants. Patients <1 year of age who underwent spinal surgery with MEP monitoring between February 2022 and July 2023 at a single tertiary care children hospital were reviewed. The motor-evoked potential amplitudes were classified i

Force modulation relies on accurate proprioception, and force-matching tasks alter corticocerebellar connectivity. Corticocerebellar (N24) and corticomotor pathways are impacted following the acquisition of a motor tracing task (MTT), measured using both somatosensory evoked potentials (SEPs) and transcranial magnetic stimulation. This study compared changes in early SEP peak amplitudes and motor performance following a force-matching tracking task (FMTT) to an MTT. Thirty (18 females) right-handed participants, aged 21.4 ± 2.76, were electrically stimulated over the right-median nerve at 2.47

INTRODUCTION AND OBJECTIVES: Motor evoked potentials (MEPs) have been postulated to be useful in predicting recovery in patients with motor impairment. We aimed to investigate whether MEPs elicited by transcranial magnetic stimulation (TMS), serum brain derived neurotrophic factor (BDNF) and its genotype have prognostic value on stroke recovery in patients with hand paresis due to stroke.
METHODS: This was an observational cohort study. Patients underwent TMS with MEPs from abductor digiti minimi evaluation between 2-14 (D0) and 30 days (D30) after stroke and their impact on motor function of

PURPOSE: The aim of this study was to elucidate normative features of vagal motor-evoked potentials (MEPs) induced by transcranial electrical stimulation (TES) and to determine the influence of functional decline of the recurrent laryngeal nerve (RLN) on vagal MEPs during thyroid surgery.
METHODS: A total of 54 patients undergoing elective thyroid surgery under general anesthesia were enrolled in this study. Vagal MEPs induced by TES were measured from the vocal cord using one of two types of electrodes (wire type or wide and flat type) mounted on an endotracheal tube. We investigated the eff

OBJECTIVE: Our objective is to use the area of the motor evoked potential (MEP) as a diagnostic tool for intraoperative radicular injury.
METHODS: We analyzed the intraoperative neurophysiological monitoring data and clinical outcomes of 203 patients treated for dorsolumbar spine deformity. The decrease in amplitude was compared with the reduction in the MEP area.
RESULTS: In 11 cases, new intraoperative injuries occurred, nine of them were lumbar radiculopathies. Our new criteria, a decrease MEP area of 70%, yielded a sensitivity and specificity of 1, since it detected all the radicular inj
