Exhaled nitric oxide measurement in patients affected by nasal polyposis.

OBJECTIVES: Nitric oxide (NO) is produced in the respiratory tract with a major contribution coming from paranasal sinuses and the nose. The pathophysiological role of NO in the airways has been debated. The aims of this study were to measure fraction of exhaled NO (FENO), a validated marker of airway inflammation, in patients affected by nasal polyposis with and without asthma; to assess the importance of FENO measurement in detecting subclinical involvement of lower airways in patients with clinical rhinosinusal symptoms; and to clarify the impact of endoscopic surgical removal of polyps on

Diagnostic value of exhaled nitric oxide in childhood asthma and allergy.

Fractional exhaled nitric oxide (FE(NO) ) has been proposed as a diagnostic test of asthma. We aimed to investigate in a population based birth cohort of children the usefulness of FE(NO) as a diagnostic tool. The 10-yr follow up of the Environment and Childhood Asthma Study in Oslo included 616 children representative of the prospective birth cohort. Both FE(NO) (single breath technique) and skin prick test (SPT) were measured in 331, limited at the time by equipment availability. Structural parental interview, spirometry, methacholine challenge and exercise test were performed. FE(NO) was si

Exhaled breath condensate pH assays are not influenced by oral ammonia.

BACKGROUND: Measurement of pH in exhaled breath condensate (EBC) is robust and simple. Acidic source fluid (airway lining fluid) traps bases while volatilising acids, leading to EBC acidification in many lung diseases. Lower airway ammonia is one determinant of airway lining fluid pH, raising the concern that addition of the base ammonia by contamination from the mouth might confound EBC pH assays. METHODS: Three discrete methods were used to limit oral ammonia contamination of EBC collections: endotracheal intubation, oral rinsing, and -40 degrees C condenser temperatures. Separately, ammoni

Is exhaled nitric oxide measurement a useful clinical test in asthma?

PURPOSE OF REVIEW: Exhaled nitric oxide measurements (FENO) are easy to perform and are repeatable. Given the strong correlations between FENO and bronchial biopsy and induced sputum eosinophilia, as well as airway hyper-responsiveness, FENO may now be advocated as a surrogate for these tests in certain circumstances. They provide the opportunity to assess pathological rather than physiological changes in asthma. This review highlights recent advances in applying this technology to the diagnosis and ongoing assessment of asthma in the clinical and epidemiological settings. RECENT FINDINGS: Ep

The effects of hyperpnea on exhaled nitric oxide synthesis in normal subjects.

STUDY OBJECTIVES: To determine if the concentration of nitric oxide (NO) in the lungs increases with hyperpnea by contrasting calculated production (ie, the product of the fractional expired NO concentration [FeNO] and minute ventilation [Ve]) [Vno] with the amount of NO in equilibrium with the conducting airways (eNOair) and the amount of NO diffusing from the alveoli (eNOalv). DESIGN: Observational study. SETTING: University teaching hospital. PARTICIPANTS: Normal subjects. INTERVENTIONS: Measurements were made in 16 healthy people during and after 4 min of tidal breathing (10 L/min) and

Exhaled nitric oxide levels in infants with chronic lung disease.

UNLABELLED: Chronic lung disease (CLD) is an inflammatory disorder; in patients with other inflammatory disorders exhaled nitric oxide (NO) levels are elevated. The aim of this study was to test the hypothesis that prematurely born infants with CLD would have elevated exhaled NO levels compared to those without CLD and healthy term-born infants. Ten infants with CLD (median gestational age 26 weeks; CLD group), ten infants without CLD (median gestational age 32 weeks; non-CLD group) and ten term-born infants (term group) were examined at post-conceptional ages between 36 and 45 weeks. NO level

Exhaled and sputum nitric oxide in bronchiectasis: correlation with clinical parameters.

STUDY OBJECTIVES: Although there has been tremendous attention on endogenous nitric oxide (NO) production in many respiratory and systemic diseases, little is known on NO production in bronchiectasis. DESIGN AND SETTING: We determined exhaled and sputum NO levels in 109 patients with stable bronchiectasis (71 women; mean +/- SD age, 58.2 +/- 14.1 years) and 78 control subjects (39 women; mean age, 56.7 +/- 12.1 years) by using an automatic chemiluminescence analyzer. MEASUREMENTS AND RESULTS: There was no significant difference in exhaled NO between patients with bronchiectasis and control s

Increased nitrosothiols in exhaled breath condensate in inflammatory airway diseases.

Nitrosothiols (RS-NOs) are formed by interaction of nitric oxide (NO) with glutathione and may limit the detrimental effect of NO. Because NO generation is increased in airway inflammation, we have measured RS-NOs in exhaled breath condensate in patients with asthma, cystic fibrosis, or chronic obstructive pulmonary disease (COPD). We also measured exhaled NO and nitrite (NO(2-)) in the same subjects. RS-NOs were detectable in exhaled breath condensate of all subjects. RS-NOs were higher in subjects with severe asthma (0.81 +/- 0.06 microM) when compared with normal control subjects (0.11 +/-

Exhaled nitric oxide measurements in a population sample of young adults.

In epidemiologic studies of asthma there is a group with recent wheeze, but with no airway hyperresponsiveness (AHR), in whom it is unclear whether any significant airway abnormality exists. Exhaled nitric oxide (NO) has been proposed as a measure of airway inflammation. We measured exhaled NO in a population sample of 306 young adults who also underwent bronchial challenge with histamine or a bronchodilator test. Subjects blew into a 3-L Tedlar bag against a 2-mm-diameter resistance to close the soft palate and exclude nasal air. The NO content of expired gas from a single breath was analyzed

The size distribution of droplets in the exhaled breath of healthy human subjects.

Droplets carried in exhaled breath may carry microorganisms capable of transmitting disease over both short and long distances. The size distribution of such droplets will influence the type of organisms that may be carried as well as strategies for controlling airborne infection. The aim of this study was to characterize the size distribution of droplets exhaled by healthy individuals. Exhaled droplets from human subjects performing four respiratory actions (mouth breathing, nose breathing, coughing, talking) were measured by both an optical particle counter (OPC) and an analytical transmissi

Endogenous nitric oxide in single exhalations and the change during exercise.

The concentrations of endogenous nitric oxide (NO) and CO2 were measured in exhaled air from healthy humans. During a single exhalation, NO concentrations peaked when the CO2 level reached its plateau. However, whereas CO2 levels remained at a plateau throughout the exhalation, NO concentrations declined after having peaked. After breathholding (5 to 60 s) NO peaked at a higher level than during normal breathing. Nitric oxide concentrations in exhaled air decreased during physical exercise (50 to 100 W). However, taking the increased ventilatory minute volume into account, excretion of NO mark

Tetrachloroethene in exhaled air of persons living near pollution sources.

Exhaled air was analyzed for tetrachloroethene (PER) in teachers and 4-5-year-old pupils of a kindergarten situated near a factory, and in residents of an old folks ' home situated near a former chemical waste dump. The PER concentrations were higher in the exhaled air of children living near the factory (mean 24 micrograms/m3, n = 6) than in control children (mean 2.8 micrograms/m3, n = 11). In the old folks ' home, the PER concentrations in the exhaled air of people living on the first floor were higher (mean 7.8 micrograms/m3, n = 10) than in the exhaled air of the people living on the seco

Diagnosis of lactose intolerance through the quantification of hydrogen in exhaled air.

Considerations are made on diarrhea secondary to intestinal malabsorption of carbohydrates and on the diagnostic means available in our environment, among which are the reactive strip, the clinitest tablet, tolerance curves and research of intestinal enzymes. The quantification of exhaled hydrogen is mentioned as a diagnostic method, used by several other authors, and a review of the literature on the subject is made. Considerations are made on a hydrogen quantification in the breath of patients with a transitory lactose intolerance. 16 patients were studied, 15 of whom showed an increase in t

A method for measuring the molecular ratio of inhalation to exhalation and effect of inspired oxygen levels on oxygen consumption.

Using a new method for measuring the molecular ratio (R) of inhalation to exhalation, we investigated the effect of high fraction of inspired oxygen (FIO2) on oxygen consumption (VO2), carbon dioxide generation (VCO2), and respiratory quotient (RQ) in mechanically ventilated rats. Twelve rats were equally assigned into two groups by anesthetics: intravenous midazolam/fentanyl vs. inhaled isoflurane. R, VO2, VCO2, and RQ were measured at FIO2 0.3 or 1.0. R error was ± 0.003. R was 1.0099 ± 0.0023 with isoflurane and 1.0074 ± 0.0018 with midazolam/fentanyl. R was 1.0081 ± 0.0017 at an FIO2 of 0.

Box breathing and prolonged exhalation reduces markers of physiological stress reactivity in response to a virtual trier social stress test.

Acute exposure to psychological stress activates the sympathetic "fight or flight" response, increasing heart rate, respiratory rate, and metabolic activity. High-stress occupations such as law enforcement, firefighting, and military service regularly involve exposure to intense stress and may benefit from interventions that mitigate physiological and psychological stress responses. Slow-breathing techniques such as box breathing and prolonged exhalation have been shown to reduce perceived stress and certain physiological markers; however, their relative effectiveness under acute stress condit
