Imaging diagnosis: Multiple cartilaginous exostoses and calcinosis circumscripta occurring simultaneously in the cervical spine of a dog.

An 8-month-old female Saint Bernard dog was presented with gait abnormalities consistent with a left-lateralizing cervical myelopathy. Imaging revealed a large, irregular soft tissue and mineral mass at the level of C1 and C2. The lesion was successfully excised, and histopathology was performed, revealing evidence of both multiple cartilaginous exostoses and calcinosis circumscripta. To the authors' knowledge, this is the first report comparing features using magnetic resonance imaging, computed tomography, and radiography. Additionally, multiple cartilaginous exostoses have not previously be

Acute popliteal pseudoaneurysm rupture secondary to distal femoral osteochondroma in a patient with hereditary multiple exostoses.

A 21-year-old man with known hereditary multiple exostoses presented with a 24-hour history of atraumatic bruising and swelling of the posterior thigh. A leaking popliteal pseudoaneurysm was diagnosed on ultrasonography and an emergency saphenous vein bypass graft procedure performed. The patient required a post-operative blood transfusion but otherwise made a full recovery. Vascular complications from osteochondromas are rare and include vessel displacement, stenosis, occlusion, arteriovenous fistulas and pseudoaneurysm formation. Pseudoaneurysms usually present as an enlarging mass behind th

Loss of heterozygosity in chondrosarcomas for markers linked to hereditary multiple exostoses loci on chromosomes 8 and 11.

Hereditary multiple exostoses (EXT; MIM 133700) is an autosomal dominant condition characterized by growth of multiple benign cartilage-capped tumors. EXT greatly increases the relative risk to develop chondrosarcoma, although most chondrosarcomas are sporadic. This observation suggests that, like the genes responsible for retinoblastoma and other dominantly inherited cancer susceptibility disorders, the genes that cause EXT may have tumor-suppressor function and may play a role in the pathogenesis of the related sporadic tumors. To investigate this hypothesis, we evaluated chondrosarcomas for

Spastic disorder in patients with hereditary multiple exostoses, but without spinal cord compression: a new syndrome?

We describe a 37 year old man with a history of a gait disorder which had been worsening over a period of three years. Clinical examination showed the typical signs of a spastic tetraparesis with increased tone of all the extremities. Sensation, autonomic and cerebellar functions were not disturbed. Multiple exostoses had been present since early childhood, but none had been found in the spine or the cranium to cause the tetraspastic disorder. MRI scan was normal. Pedigree analysis of four generations showed that other family members were affected by both disorders. Chromosomal analysis was no

[The uncus exostoses syndrome and its treatment by means of the decompression operation of the vertebral artery (author's transl)].

In degenerative changes of the mobile segments of the cervical vertebral column, deformations of the uncovertebral gaps (joints), resembling exostoses, with a lateral or dorsolateral spread, are frequently seen. This involves irritation of neighbouring structures, such as the arteria and vena vertebralis including the nervus vertebralis or the corresponding nerve roots. This produces a disease pattern with a cervico-encephalic and cervico-brachial mixed pattern of symptoms. Angiography of the a. vertebralis proves the localisation and extent of the disturbance. The decompression of the a. vert

A Unique Case of Asymptomatic Osteochondroma From the Rib in Multiple Hereditary Exostoses: Insights Into Screening and Management.

CASE: A 10-year-old girl with Multiple Hereditary Exostoses (MHE) had a whole spine screening MRI, which revealed a large asymptomatic costovertebral osteochondroma causing spinal cord impingement. She had an en bloc resection, laminectomy of T4, and T3 to T5 posterior instrumented spinal fusion. At the 1-year follow-up, she remained asymptomatic. CONCLUSIONS: Costovertebral osteochondromas are rare and typically discovered once they become symptomatic. This case highlights the value of spinal screening MRIs in children with MHE, emphasizing the need to develop screening protocols and establi

Cervical spinal myelopathy secondary to cervical exostoses: A rare cause of cord compression - A case report.

BACKGROUND: Multiple hereditary exostoses (MHE) is a rare autosomal dominant disorder. It is characterized by the development of cartilage-capped bony outgrowths arising from the epiphyseal or diaphyseal regions of long bones, leading to reduced joint motion and pain secondary to compression. MHE is attributed to the predominant exostin-1 and exostin-2 mutations that cause multiple osteochondromas. The most common locations of these lesions are on the knee joint, humerus, pelvis, scapula, and also the spine. CASE DESCRIPTION: A 26-year-old Caucasian male with a diagnosis of MHE, presenting wi

Maxillary exostoses as contributing etiology to lip hypermobility and associated excessive gingival display: A clinical report.

Several etiologies may contribute to the development of excessive gingival display (EGD). However, little, if any, consideration has been given to the potentially significant role of prominent buccal maxillary exostoses (BMEs) in EGD etiology. Therefore, the aim of this report was to highlight the contribution of BMEs to EGD. Two patients complaining of EGD were evaluated, and BMEs were found associated with a hypermobile upper lip, vertical maxillary excess, and altered passive eruption. Both patients received esthetic crown lengthening (ECL) as the first surgical intervention. The BMEs were

Corrective Surgery for a Forearm Deformity in a Middle-Aged Patient with Multiple Hereditary Exostoses: A Case Report.

CASE: A 48-year-old man underwent corrective surgery for a deformity of the left forearm because of multiple hereditary exostoses (MHE). The patient had no complaint of pain, the appearance of his forearm improved, and acceptable range of motion of the wrist and forearm were maintained at 14-month follow-up. CONCLUSION: The esthetic forearm deformity in the middle-aged patient with MHE was successfully improved without sacrificing function. Although there is little evidence of forearm corrective surgeries for adult patients with MHE, this report could expand surgical indications for them.

Identification of a novel EXT2 frameshift mutation in a family with hereditary multiple exostoses by whole-exome sequencing.

BACKGROUND: Hereditary multiple exostoses (HME), also referred to as multiple osteochondromas, is an autosomal dominant skeletal disease characterized by the development of multiple overgrown benign bony tumors capped by cartilage and is associated with bone deformity, joint limitation, and short stature. Mutations in exostosin glycosyltransferase (EXT)1 and EXT2 genes, which are located on chromosomes 8q24.1 and 11p13, contribute to the pathogenesis of HME. METHODS: In the present study, a genetic analysis of a four-generation Chinese family with HME was conducted using whole-exome sequencin

Advances in the pathogenesis and possible treatments for multiple hereditary exostoses from the 2016 international MHE conference.

Multiple hereditary exostoses (MHE) is an autosomal dominant disorder that affects about 1 in 50,000 children worldwide. MHE, also known as hereditary multiple exostoses (HME) or multiple osteochondromas (MO), is characterized by cartilage-capped outgrowths called osteochondromas that develop adjacent to the growth plates of skeletal elements in young patients. These benign tumors can affect growth plate function, leading to skeletal growth retardation, or deformations, and can encroach on nerves, tendons, muscles, and other surrounding tissues and cause motion impairment, chronic pain, and ea

A 5-year-old boy with a large hereditary multiple exostoses lump grown into thoracic cavity.

Multiple exostosis is associated with the autosomal dominant disorder and hereditary multiple exostoses (HMEs), are rarely growing around the ribs and the formation of a large lump into the thoracic cavity. It's generally agreed that when there are no symptoms present, a HME patient could either avoid any treatment or postpone the operation until adolescence. We present a 5-year-old boy with 4 cm × 3.5 cm HMEs lump invasion into the thoracic cavity, with symptoms of chest distress and right chest pain. This patient received video-assisted thoracoscopic surgery (VATS) and a the lump and 1 cm ri

[Exostoses of the external auditory canal in professional diving. A case for the German Social Accident Insurance Institution].

Exostoses of the external auditory canal are often diagnosed incidentally but may also cause complications, e.g., conductive hearing loss and/or recurrent inflammations of the external ear canal due to stenosis. This paper presents current scientific data on the pathogenesis of ear canal exostoses, which obviously focus on the cold water hypothesis. We present a case of an expert opinion on occupational illness and discuss whether the legal preconditions for recognition "like an occupational disease" according to article 9(2) Social Code Book VII are met. The role of occlusal stress and gingival infection in the formation of exostoses on mandible and maxilla from Neolithic China.

Exostoses on the mandible and maxilla is a frequently observed bone growth of controversial aetiology. The aim of this study is to analyse environmental factors that may stimulate the formation of exostoses on different regions of the maxilla and mandible. Sixty-six well-preserved crania from Neolithic China were studied for the presence of buccal exostoses on the maxilla (BE) and lingual exostoses on the mandible (LME). Other oral health indicators, such as occlusal wear on molars, pathology of temporomandibular joint (TMJ), carious lesions, calculus accretion, periodontal disease, and antemo

Chronic Pain as an Early Diagnostic Clue in Hereditary Multiple Exostoses: Two Pediatric Cases Highlighting Diagnostic Delay.

Persistent musculoskeletal pain in children, particularly when associated with palpable masses or a positive family history, should raise suspicion for hereditary multiple exostoses. Recognizing pain as an early diagnostic clue may reduce delay, enabling timely imaging, structured surveillance, and appropriate long-term management.

Ulnar lengthening for forearm deformities in hereditary multiple exostoses: a systematic review and meta-analysis (2015-2025).

BACKGROUND: Hereditary multiple exostoses (HME) is an autosomal dominant skeletal disorder frequently associated with progressive forearm deformities, including ulnar shortening, radial bowing, and radial head dislocation. Ulnar lengthening using distraction osteogenesis has become the principal surgical strategy; however, outcomes, techniques, and complication profiles vary across studies. This systematic review and meta-analysis synthesize contemporary evidence on radiographic correction, functional outcomes, and complications following ulnar lengthening for HME-related forearm deformities.
