Endovascular treatment of aortobronchial fistula secondary to coronary artery bypass graft (CABG).

Aortobronchial fistula (ABF) is a rare and late complication of cardiac surgery. If untreated, mortality rate is approximately 100% secondary to exsanguinations haemoptysis. Early diagnosis and treatment are essential for successful management. Open surgical repair is associated with high morbidity and mortality rate, ranging from 25% to 41%. Endovascular treatments of ABF is a less invasive treatment modality and have become an important alternative to open surgical intervention in aortic pathologies. We present a case of ABF that successfully is managed by endovascular approach.

Multifactorial mortality in bongos and other wild ungulates in the north of the Congo Republic.

Wildlife mortality involving bongos, Tragelaphus eurycerus, and other ungulates was investigated in the north of the Congo Republic in 1997. Four bongos, one forest buffalo, Syncerus caffer nanus, and one domestic sheep were examined and sampled. Although an outbreak of rinderpest had been suspected, it was found that the animals, which had been weakened by an Elaeophora sagitta infection and possibly also by adverse climatic conditions, had been exsanguinated and driven to exhaustion by an unusual plague of Stomoxys omega.

Pseudoaneurysm of the superficial femoral artery following accidental trauma: result of treatment by percutaneous stent-graft placement.

Accidental trauma frequently involves the extremities, and can extend to involve their blood supply, causing exsanguinating hemorrhage and pseudoaneurysm in the involved blood vessel. This is traditionally managed by surgical repair. We report a case in which control of life-threatening hemorrhage and exclusion of a large, post-traumatic pseudoaneurysm in the superficial femoral artery was performed by a commercially available stent-graft, without complication. This treatment method may be a safe and effective alternative to surgery in selected patients.

Benckiser's hemorrhage: about an uncommon case report.

BACKGROUND: Benckiser's hemorrhage is a serious obstetric emergency caused by rupture of one or more umbilical vessels of velamentous insertion, putting the fetus in distress and leading to rapid fetal death through exsanguination in utero. It is an uncommon condition associated with a neonatal mortality rate of 75-100%. This hemorrhage of fetal origin happens most often when the membranes rupture, whether spontaneously or artificially. This is why prenatal diagnosis via ultrasound can only be beneficial and make it possible to schedule a prophylactic caesarean section before the onset of feta

Hepatic trauma in Auckland.

A retrospective review of 102 cases of hepatic trauma in the Auckland area between 1979 and 1985 is presented. Particular attention has been focused on those cases where there was massive bleeding, and the prognostic factors that govern outcome have been determined. Mechanism and multiplicity of injury, and the presence of severe hypotension (systolic blood pressure less than .80 mmHg) either at presentation or following induction of anaesthesia were the four most important determinants of prognosis. Blunt trauma caused 82% of these cases, with the mortality in this group being 27%, compared w

A Potentially Lethal Course of a Nasogastric Tube: Case Report.

Nasogastric tube (NGT) insertion is a common procedure, often performed blindly. We report a rare, near-fatal complication in a 44-year-old woman where an NGT traversed the internal jugular vein, superior vena cava, and perforated the right atrium into the pericardial cavity. Prompt imaging and surgical retrieval via sternotomy prevented tamponade or exsanguination. No esophageal injury was found; the likely entry point was the nasopharynx. A sealed duodenal perforation was managed conservatively. This case emphasizes the importance of verifying NGT placement before use and the role of confirm

Counseling and Educating About Vasa Previa Using Intraoperative Photographs of Vessel Rupture: A Case Report.

Vasa previa describes vessels that are unprotected by Wharton's jelly and traverse over or near the endocervix. Rupture of membranes or labor increases the risk of vessel rupture, followed by rapid fetal exsanguination. Learners are often taught to manage this condition by case scenarios and simulations. We present a case of type I vasa previa where intraoperative photographs demonstrate the potentially catastrophic rupture of membranes into the unprotected fetal vessels during a scheduled cesarean delivery. We recommend using intraoperative photographs to educate learners and counsel patients

An Elegant Solution to a Ruptured Right Aberrant Subclavian Artery after Oesophageal Stent Removal.

Arterioenteric or arteriotracheal fistula is a known complication of an aberrant right subclavian artery (ARSA) and is often associated with prolonged nasogastric or endotracheal intubation or oesophageal stenting. Fistula formation from the ARSA can present unexpectedly with rapid exsanguination with massive haemoptysis or haematemesis, and unless promptly recognised and treated is rapidly fatal. We present a novel endovascular method for treating a fistula between the oesophagus, trachea and an ARSA in an unstable patient following oesophageal stent removal, utilising a covered iliac limb st

Hemothorax due to rupture of pulmonary arteriovenous malformation: an interventional emergency.

Spontaneous hemothorax as a result of a ruptured pulmonary arteriovenous malformation (PAVM) is a life-threatening event and requires immediate interventional therapy. We present two patients who survived following emergent embolization. Definitive thoracentesis was delayed until embolization was performed. The tamponade provided by the hemothorax may have prevented exsanguination, suggesting to us that drainage of blood from the pleural space should be delayed until the PAVM has been treated. Hemorrhage from a PAVM may be the first manifestation of hereditary hemorrhagic telangiectasia. Genet

Ruptured aortic aneurysm in a coyote (Canis latrans) from South Carolina.

A radio-collared adult female coyote (Canis latrans) from South Carolina was found dead with no apparent signs of trauma or struggle. Necropsy revealed a ruptured aortic aneurysm within the thoracic cavity as well as severe heartworm infection, with parasites present in the caudal vena cava. Histologically, inflammatory cell infiltrates were frequent in the aneurysm and consisted of eosinophils, neutrophils, lymphocytes, plasma cells, and macrophages. Bacteria, fungi, and parasites were not found in the aneurysm. Death was due to exsanguination. This represents a first report of an aneurysm in

Medical suicide -- groin stabbing.

This case report concerns a 50-year-old male who died from exsanguination due to self-inflicted stabbing injuries to the groin and cutting injuries to the neck. He was a medical doctor suffering from depression following treatment for cancer. Postmortem examination revealed that stab injuries in the groin were directly over the femoral arteries where the femoral pulse is clinically palpated. The injury pattern of this suicidal stabbing is uncommon and the medical knowledge of this person may have well contributed to the choice of the site.

Role of bronchoscopy in massive hemoptysis.

Massive hemoptysis accounts for a minority of all patients with hemoptysis but poses a major challenge for the acute and long-term treatment. Massive hemoptysis can lead to asphyxiation and airway obstruction, shock, and exsanguination. Bronchoscopy plays an integral part in managing massive hemoptysis in diagnosis and treatment (Table 5). Specifically, bronchoscopy allows lateralization and more specific localization of bleeding that is critically important for effective management. Furthermore, acute control of bleeding can sometimes be achieved with instruments and catheters placed through

Fatal black esophagus and duodenum.

Black esophagus (BE) is a rare finding characterized by a black appearance on the distal esophageal mucosa. In the forensic field, BE is often reported in cases of diabetic ketoacidosis and hypothermia. Coexistence of acute duodenitis (black duodenum) has also been reported. Serious complications, such as esophageal perforation and hemorrhagic shock from bleeding from the esophageal mucosa, can be life-threatening. We report a forensic autopsy case of a relatively young man who died of BE and black duodenum with no underlying conditions such as diabetes or alcohol/drug abuse. The patient was a

Between the hammer and the anvil: An unusual power hammer fatality.

Power hammers are mechanised forging devices that constitute a pivotal part of steel manufacturing. Power hammer-associated injuries are a rare occurrence. We report a noteworthy case of a 52-year-old man who sustained a high-energy penetrating injury while working with a power (counterblow) hammer. The man used a sizable disc-shaped metallic object to dislodge the forging wedged in the machine by applying the force of the striking ram on it. On impact, the object ejected and struck the man in the right lateral portion of the chest. The autopsy disclosed extensive damage to the thoracic and ab

Abdominal vascular injuries.

Injuries to major abdominal arteries and veins frequently are associated with exsanguinating hemorrhage and visceral ischemia. Expeditious management is the key to survival and good outcome. Knowledge of anatomic relationships between viscera and vessels forms the basis for directed dissection, optimal exposure, and lasting repair of vessels. Although penetrating mechanism of injury remains the most common cause of these injuries, trauma surgeons must be familiar with patterns of blunt trauma-mediated injury to avoid the devastating consequences of delayed management.

Aorto-oesophageal fistula. A rare cause of gastrointestinal haemorrhage.

Primary aorto-oesophageal fistula is a rare cause of massive and usually fatal gastrointestinal hemorrhage. We herein present a case of a 62-year-old chronic alcohol abuser who had a small haematemesis after a week of persistent vomiting. He succumbed to an exsanguinating bleed 36 hours after admission. An aorto-oesophageal fistula was found at autopsy. We emphasize the need for a high index of suspicion and recognition of premonitory signs for accurate diagnosis and management of this condition.

Brain Injury and Neurodevelopmental Outcome in Survivors After Spontaneous Single Fetal Demise in Monochorionic Twins: A Systematic Review and Meta-Analysis.

BACKGROUND: Monochorionic (MC) twins are at risk of acute exsanguination after single fetal demise (sFD) due to their shared placental circulation, which may result in sequelae for survivors. OBJECTIVES: To evaluate the prevalence of ante- and postnatal brain injury and long-term neurodevelopmental impairment (NDI) in co-twins after sFD. Secondary outcomes were the prevalence of termination of pregnancy (TOP), neonatal death (NND) and potential risk factors for brain injury. SEARCH STRATEGY: PubMed, Embase, Scopus and Web of Science were searched to identify relevant studies in October 2024.

Incidental aortic coarctation in traumatic subclavian artery injury.

Subclavian artery injuries are complex and challenging due to anatomy and exposure during surgery. The surgical management depends on the mechanism of injury, the patient's haemodynamic stability and other injuries sustained. If control of bleeding is lost during surgery, it results in immediate exsanguination, with high mortality and morbidity rates. New techniques with endovascular surgery have changed the approach and outcome of these injuries. In this case report, an incidental finding of coarctation of the aorta in a 32-year-old man after sustaining a gunshot to the chest, with a subseque
