Establishing and maintaining treatment effects with less intrusive consequences via a pairing procedure.

The generality and long-term maintenance of a pairing procedure designed to improve the efficacy of less intrusive procedures were evaluated for the treatment of problem behavior maintained by automatic reinforcement exhibited by 2 individuals with developmental disabilities. Results suggested that a less intrusive procedure could be established as a conditioned punisher by pairing it with an effective punisher contingent on problem behavior. Generalization across multiple therapists was demonstrated for both participants. However, generalization to another setting was not achieved for 1 parti

A function-based treatment for school refusal behavior using shaping and fading.

A function-based behavioral treatment package was developed for an 18-year-old male with mild mental retardation admitted to a specialized inpatient unit for treatment of severe destructive behaviors associated with school refusal behavior. Teacher and school attendance reports, parent interview, and unstructured observations led to the hypothesis that the subject's school refusal behavior was maintained by positive reinforcement. The treatment goals included increasing the subject's compliance with a morning hygiene routine and attending school (shaping), increasing the involvement of the sub

Vulnerability and defensive reactions in relation to the human menstrual cycle.

The effects of alterations of the organism's internal state, attributable to the menstrual cycle, upon heart-rate responsivity and conditioning are reported. Two groups of 24 female phobic patients (one with natural cycles and the other taking the oral contraceptive) were tested, either premenstrually or intermenstrually, using a conditioning procedure in which a small blue light was the CS and white noise the UCS. Subjects tested premenstrually were significantly differentiated from those tested intermenstrually respecting HR responsivity to the first UCS. Significant differences were also fo

Optimizing in vivo exposure using occasional reinforced extinction with aversive imagery in spider fearful individuals.

Although exposure-based therapy is widely recognized as effective for treating various anxiety disorders, a significant proportion of patients fail to benefit or experience a return of fear following successful treatment. One promising strategy involves occasional presentation of fear-evoking stimuli during extinction (occasional reinforced extinction, ORE). This study investigates a novel approach to translate ORE into clinical practice by incorporating occasional vivid imagery of individuals' worst-case fear scenarios during in-vivo exposure. Forty-seven spider-fearful individuals were rando

Persistence of safety behaviors or the acquisition of new ones? The effect of cross-US reinstatement in safety behaviors.

Exposure to a stressful trauma after successful treatment can lead to relapse, known as fear reinstatement. In the laboratory, fear reinstatement involves re-exposing individuals to an aversive unconditioned stimulus (US) after successful extinction learning, leading to the reinstatement of fear to a previously threatening conditioned stimulus (CS). While fear reinstatement is typically thought to evoke the return of old fear, preliminary evidence suggests that using a qualitatively different US during reinstatement can elicit new fear (i.e., this new US is expected to follow the CS instead of

A multi-modal assessment of fear conditioning in adolescent anorexia nervosa.

OBJECTIVE: Anorexia nervosa (AN) is a pernicious psychiatric disorder which is principally characterized by a fear of weight gain. Notwithstanding the centrality of fear in the psychopathology of AN, controlled assessments of negative valence systems are lacking. Herein we assess fear conditioning in adolescent females with AN. METHOD: Adolescent girls (Mage = 14.6 years, ±1.57) with DSM-5 diagnoses of AN (N = 25) and age-matched control girls (Mage = 14.8 years, ±1.46) with no DSM-5 diagnoses (N = 25) completed structured clinical interviews and participated in a classical three-phase Pavlov

Impaired learning, memory, and extinction in posttraumatic stress disorder: translational meta-analysis of clinical and preclinical studies.

Current evidence-based treatments for post-traumatic stress disorder (PTSD) are efficacious in only part of PTSD patients. Therefore, novel neurobiologically informed approaches are urgently needed. Clinical and translational neuroscience point to altered learning and memory processes as key in (models of) PTSD psychopathology. We extended this notion by clarifying at a meta-level (i) the role of information valence, i.e. neutral versus emotional/fearful, and (ii) comparability, as far as applicable, between clinical and preclinical phenotypes. We hypothesized that cross-species, neutral versu

Restoring the Function of Thalamocortical Circuit Through Correcting Thalamic Kv3.2 Channelopathy Normalizes Fear Extinction Impairments in a PTSD Mouse Model.

Impaired extinction of fear memory is one of the most common symptoms in post-traumatic stress disorder (PTSD), with limited therapeutic strategies due to the poor understanding of its underlying neural substrates. In this study, functional screening is performed and identified hyperactivity in the mediodorsal thalamic nucleus (MD) during fear extinction. Furthermore, the encoding patterns of the hyperactivated MD is investigated during persistent fear responses using multiple machine learning algorithms. The anterior cingulate cortex (ACC) is also identified as a functional downstream region

Contemporary Approaches Toward Neuromodulation of Fear Extinction and Its Underlying Neural Circuits.

Neuroscience and neuroimaging research have now identified brain nodes that are involved in the acquisition, storage, and expression of conditioned fear and its extinction. These brain regions include the ventromedial prefrontal cortex (vmPFC), dorsal anterior cingulate cortex (dACC), amygdala, insular cortex, and hippocampus. Psychiatric neuroimaging research shows that functional dysregulation of these brain regions might contribute to the etiology and symptomatology of various psychopathologies, including anxiety disorders and post traumatic stress disorder (PTSD) (Barad et al. Inhibition of Estradiol Signaling in the Basolateral Amygdala Impairs Extinction Memory Recall for Heroin-Conditioned Cues in a Sex-Specific Manner.

INTRODUCTION: Relapse is a major treatment barrier for opioid use disorder. Environmental cues become associated with the rewarding effects of opioids and can precipitate relapse, even after numerous unreinforced cue presentations, due to deficits in extinction memory recall (EMR). Estradiol (E2) modulates EMR of fear-related cues, but it is unknown whether E2 impacts EMR of reward cues and what brain region(s) are responsible for E2s effects. Here, we hypothesize that inhibition of E2 signaling in the basolateral amygdala (BLA) will impair EMR of a heroin-associated cue in both male and femal

The relationship between Intolerance of Uncertainty and conditioned fear acquisition: Evidence from a large sample.

Despite being considered a valid model for the etiology of anxiety disorders, the fear conditioning paradigm does not always show clear correlations with anxious personality traits that constitute risk factors for the development of anxiety disorders. This may in part due to error variance and the fact that fear conditioning studies are typically underpowered to investigate inter-individual differences. In the current study, we focus on the relationship between conditioned fear acquisition and Intolerance of Uncertainty (IU). In a re-analysis of a large previous study (N = 120), which was cond

Combining the trauma film and fear conditioning paradigms: A theoretical review and meta-analysis with relevance to PTSD.

A growing literature has sought to combine fear conditioning paradigms with the trauma film paradigm to study the associative learning properties of intrusive re-experiencing in PTSD. We review this innovative approach and the recent findings by highlighting their relevance to cognitive and conditioning theories of PTSD. We also conduct a meta-analysis of the available studies to demonstrate that, for most outcome measures, fear learning using a traumatic film clip unconditional stimulus yields results similar to those seen with an electro-tactile unconditional stimulus, which implies that the

Prelimbic and infralimbic medial prefrontal cortex neuron activity signals cocaine seeking variables across multiple timescales.

RATIONALE AND OBJECTIVES: The prefrontal cortex is critical for execution and inhibition of reward seeking. Neural manipulation of rodent medial prefrontal cortex (mPFC) subregions differentially impacts execution and inhibition of cocaine seeking. Dorsal, or prelimbic (PL), and ventral, or infralimbic (IL) mPFC are implicated in cocaine seeking or extinction of cocaine seeking, respectively. This differentiation is not seen across all studies, indicating that further research is needed to understand specific mPFC contributions to drug seeking. METHODS: We recorded neuronal activity in mPFC s

Acute food deprivation-induced relapse to heroin seeking after short and long punishment-imposed abstinence in male rats.

RATIONAL: Stress is a major trigger for drug relapse in humans and animal models, even after prolonged abstinence. However, animal models for stress-induced relapse were criticized for the lack of predictive and face validity. OBJECTIVES: Here we investigated the effect of acute food deprivation stress in a novel stress-induced relapse model using voluntary, punishment-imposed abstinence from heroin. We also performed a detailed characterization of the development of punishment-imposed abstinence. METHODS: Male rats were trained to self-administered heroin (0.1 mg/kg/infusion) for 2 weeks, u

Sleep quality and outcome of exposure therapy in adults with social anxiety disorder.

INTRODUCTION: Poor sleep is prevalent among individuals with social anxiety disorder (SAD) and may negatively affect exposure therapy outcomes. Poor sleep may impair memory and learning, and thus compromise fear extinction learning thought to take place in exposure therapy. We examined poor sleep as a predictor of exposure therapy outcomes for SAD and the moderating role of d-cycloserine (DCS) on this relationship. METHODS: Participants were 152 individuals with a primary diagnosis of SAD. As part of a randomized clinical trial evaluating the efficacy of DCS for enhancing the effects of expos

Human-Specific Neuropeptide S Receptor Variants Regulate Fear Extinction in the Basal Amygdala of Male and Female Mice Depending on Threat Salience.

BACKGROUND: A nonsynonymous single nucleotide polymorphism in the neuropeptide S receptor 1 (NPSR1) gene (rs324981) results in isoleucine-to-asparagine substitution at amino acid 107. In humans, the ancestral variant (NPSR1 I107) is associated with increased anxiety sensitivity and risk of panic disorder, while the human-specific variant (NPSR1 N107) is considered protective against excessive anxiety. In rodents, neurobiological constituents of the NPS system have been analyzed in detail and their anxiolytic-like effects have been endorsed. However, their implication for anxiety and related di

Hypericum perforatum L. prevents the acquisition of and promotes resilience against stress-induced reinstatement of the conditioned place preference induced by cocaine.

Cocaine use disorder is a serious problem worldwide, and there are no approved medications for its treatment. A novel approach to the treatment of drug addiction is the use of natural products, and, in this context, preclinical evidence suggests that Hypericum perforatum L. (Hypericum) is effective against alcohol and other substance use disorders. We hypothesised that Hypericum could also be useful as a treatment for cocaine use disorder, and so we set out to test its effectiveness in a mice model of cocaine addiction. In the first experiment we evaluated its effects on the acquisition of coc

Prefrontal Theta Oscillations Are Modulated by Estradiol Status During Fear Recall and Extinction Recall.

BACKGROUND: Emerging human studies demonstrate that theta oscillations in the dorsal anterior cingulate cortex are enhanced during fear recall (enhanced fear expression) and reduced during successful extinction recall (reduced fear expression). Although evidence suggests sex differences in fear recall and extinction recall, there are currently no human studies examining the oscillatory foundations of these memory processes separately in men and women. METHODS: Because previous studies suggest that estradiol partially mediates these sex differences, we examined 20 men (low estradiol and low pr

Examining the cardiovascular response to fear extinction in a trauma-exposed sample.

BACKGROUND: Trauma and symptoms of posttraumatic stress disorder (PTSD) have repeatedly been linked to impaired cardiovascular functioning. Poor fear extinction is a well-established biomarker of PTSD that may provide insight into mechanisms underlying cardiovascular risk. The current study probed the cardiovascular response to extinction in a sample of trauma-exposed individuals. METHODS: Participants were 51 trauma-exposed women who underwent a fear conditioning paradigm. Heart rate (HR) during extinction was examined in response to a conditioned stimulus that was previously paired with an
