The use of the temporalis muscle flap in facial and craniofacial reconstructive surgery. A review of 182 cases.

The authors report on their 16-year experience of reconstruction with the temporalis myofascial flap in 182 cases. All aspects of reconstructive cranio-maxillofacial surgery are covered: trauma, deformities, tumours, TMJ ankylosis, facial paralysis. The temporalis myofascial flap was used both as a single and as a composite flap with cranial bone, coronoid process or skin island. Major complications were not observed. On the basis of their experience, the authors confirm the reliability, versatility and reproducibility of the use of this flap. This is due both to its rich blood supply and to i

Repair of the facial nerve using freeze-thawed muscle autografts. A surgical model in the sheep.

In five sheep, repair of the right facial nerve in the preparotid region was undertaken using freeze-thawed muscle autografts aligned coaxially in the nerve gap. The sheep were reviewed at 240 days. Nerve conduction velocities were measured in the distal buccal nerve after stimulation proximal to the repair site and compared with an equivalent site on the normal side. Although mean peak conduction velocities were reduced on the repaired side, electrophysiologic evidence showed regeneration in all five cases. Normal and repaired nerves were retrieved for histologic study. Anatomic indices of re

Acquired bilateral inferior oblique muscle paralysis following facial injections of botulinum toxin.

Purpose: To report a unique case of bilateral isolated inferior oblique muscle paralysis (IOP) causing torsional diplopia following facial botulinum toxin (BTX) injections. Case report: A 37-year-old female presented with acute-onset torsional diplopia, exacerbated in primary and upward gaze, and alleviated on downward gaze. Neurological evaluations, including cranial/orbital MRI and neostigmine testing, were negative. Ophthalmic examination demonstrated bilateral incyclotorsion, consistent with bilateral IOP. History revealed recent periocular BTX injections for infraorbital rhytids, and expe

Targeted chemodenervation of the posterior belly of the digastric muscle for the management of jaw discomfort in facial synkinesis.

BACKGROUND: Botulinum toxin (BT-A) chemodenervation has been proved to significantly improve the physical and psychological well-being of patients suffering from facial synkinesis. Despite this, a cohort of patients has persistent tightness and discomfort around the angle of the jaw, which may be caused by synkinesis within the posterior belly of digastric (PBD) muscle. This study was designed to evaluate the benefits of ultrasound-guided BT-A injections into the PBD. METHODS: Thirty-three patients with recalcitrant tightness and discomfort around the angle of the jaw, despite maximal facial

An Evaluation of Muscle Repair Techniques: Implications in Musculoskeletal Healing and Corollaries in Oral-Facial Clefting.

We performed an animal study to identify the techniques associated with the best muscle healing outcomes in cleft lip/palate surgery. The right triceps of thirty adult male Sprague-Dawley rats were cut and repaired by three different suture techniques: simple (n = 10), overlapping (n = 10), and splitting sutures (n = 10). Muscle tissues were isolated from 5 rats per group 1 and 8 weeks postoperation. The inflammatory response and muscle fiber healing were evaluated by hematoxylin and eosin (H&E) staining, Western blotting, immunohistochemistry for TNF-α and IL-1β, and immunofluorescence for la

The success of free gracilis muscle transfer to restore smile in patients with nonflaccid facial paralysis.

BACKGROUND: Development of synkinesis, hypertonicity, and poor smile excursion after facial nerve insult and recovery contribute to disfigurement, psychological difficulties, and an inability to convey emotion via facial expression. Despite treatment with physical therapy and chemodenervation, some patients who recover from transient flaccid facial paralysis never spontaneously regain the ability to perform a meaningful smile. METHODS: Prospective evaluation was performed on 20 patients with nonflaccid facial paralysis who underwent free gracilis muscle transfer. Patients were evaluated using

Lateral rectus muscle palsy, facial numbness and ataxia as the initial manifestation of multiple sclerosis.

Lateral rectus muscle (LRM) palsy due to a nuclear or fascicular sixth nerve lesion is rare as the presenting sign of multiple sclerosis (MS). It is more common to find this palsy in the company of other nearby cranial nerves deficits. Facial numbness in association with a LRM palsy or paresis may go unappreciated and therefore underreported. We report an unusual patient with a LRM palsy, facial anesthesia, ataxia and as the initial manifestation of MS. There was a demyelinating lesion in the pons seen on magnetic resonance imaging (MRI).

Revisional operations improve results of neurovascular free muscle transfer for treatment of facial paralysis.

BACKGROUND: Neurovascular free muscle transfer is currently the mainstay for smile reconstruction. However, problems such as excessive muscle bulk and dislocation of the transferred muscle attachment have been described. Furthermore, dynamic movements of the transferred muscle are sometimes too strong or too weak, resulting in facial asymmetry. In these cases, secondary revisional operations for the transferred muscle are required after neurovascular free muscle transfer. This report describes revisional operative procedures in detail and examines the extent of improvement of the smile by comp

Free split and segmental latissimus dorsi muscle transfer in one stage for facial reanimation.

The authors report the experience in facial reanimation using free innervated split and segmental latissimus dorsi muscle flap one-stage transfer in 86 patients with longstanding facial palsy. The segmental latissimus dorsi was taken from the distal part of the muscle so that the muscle flap had an ultra-long neurovascular pedicle of 13 to 17.5 cm in length. The muscle flap was made thinner by splitting the segmental muscle. The split segmental muscle flap was transferred to the paralyzed side of the face with its ultra-long neurovascular pedicle passing through a tunnel in the upper lip to th

F-wave recordings from nasal muscle for intraoperative monitoring of facial nerve function.

The generation of the F-wave in muscle after peripheral motor nerve stimulation depends upon the functional integrity of the entire motoaxon between its axon hillock and the motor endplates. Cerebellopontine angle (CPA) surgery bears the risk of damaging the facial nerve in its root exit zone; functional degradation should therefore be reflected by changes of the nasal muscle F-wave. Constant current pulses supraliminal for direct (M) muscular responses were applied subcutaneously to the zygomatic branch of the facial nerve ipsilateral to the operated side, EEG needle electrodes were placed in

Ultrasound Guidance for Botulinum Toxin Injection of Muscles Innervated by the Facial Nerve: A Systematic Review of Anatomical Precision, Safety, and Outcomes.

Botulinum toxin type A (BoNT-A) chemodenervation is commonly used for facial synkinesis and aesthetic indications, but landmark-based techniques are limited by anatomical variability and risk of off-target delivery. High-resolution ultrasound (US) can be used to enhance precision and safety. This systematic review explores the role of US-guided BoNT-A facial chemodenervation in evaluating anatomical accuracy, clinical efficacy, complications, and patient satisfaction. A comprehensive search of 6 databases through April 2025 identified studies assessing US-guided BoNT-A chemodenervation for fac

The influence of the muscles of facial expression on the development of the midface and the nose in cleft lip and palate patients. A reflection of functional anatomy, facial esthetics and physiology of the nose.

The further improvement of well-established techniques in primary and secondary cleft surgery requires a detailed and interdisciplinary knowledge and observation of anatomical, functional and developmental problems. An investigation into the macroscopic and microscopic anatomy of the perinasal and perioral muscles and parts of the human nasal septum, as well as into the pathomorphology of ancient skulls with untreated clefts is presented. On this basis an interpretation of clinical findings in untreated newborns compared with surgically treated CLP-patients has been undertaken. The 3D-CT, supe
