Intracranial schwannoma as atypical facial pain. Case report.

Facial pain is a common complaint that leads those who have it to seek professional help. Often times, the general dentist is the first clinician that a patient consults because of a presumed odontogenic origin of the pain. Occasionally a small number of these patients will be found to have an intracranial tumor. The case reported here is one such patient who was diagnosed and treated for a seventh nerve schwannoma.

[Symptomatic treatment of pain in cervico-facial cancers].

Pain is frequent in patients presenting cervico-facial cancers. It can be acute or persistent, or present at the morbid entity known as chronic pain. The specific anatomical site and the often poor environment explain that the physical disability related to pain is increased by multiple psychosocial problems. Multidisciplinary care management by several actors is required and can be facilitated by a simple decision-making model. The schema presented can be used to prompt discussion and criticism. It needs constant improvement and extensions in order to reach a consensual attitude towards this

Lateralization of chronic facial pain: fact or fiction.

Previous studies have shown that pain is generally reported more on the left side of the body. It has been hypothesized that patient report of left pain dominance may be due to the right hemisphere being less efficient in processing cutaneous sensory input while being dominant for emotional experience as compared with the left hemisphere of the brain. Only in cases of trigeminal neuralgia has self-report of pain been lateralized to the right side. Due to previous research findings, we postulated that other facial pain diagnoses may demonstrate a right-sided dominance due to the lack of neural

Temporomandibular joint dysfunction with facial pain in children.

Temporomandibular joint dysfunction with myofascial pain, a well-known clinical disorder in adults, is reported in children. In a series of 400 patients, 40 children less than 16 years of age were seen. Of these pediatric patients, 35% had a reactive depression as primary psychopathology. The multifactorial nature of the disorder is described. The need to differentiate this syndrome from recurrent otitis media and other pain syndromes is emphasized. A combined medical, oral surgical, and psychiatric management program is outlined.

Atypical facial pain as a defense against psychosis.

The author describes three women who presented psychotic symptoms 24--48 hours before scheduled neurosurgical procedures for atypical facial pain; all had had extensive dental reconstruction and attempted nerve blocks with no relief. Psychiatric hospitalization and administration of major tranquilizers resulted in control of symptoms and relief of pain. Two patients were followed for a year and have had return of psychiatric symptoms or facial pain; both have been maintained on medication and have returned to normal activities. The author suggests that the facial pain may have served as a defe

Facial pain followed by unilateral facial nerve palsy: a case report with literature review.

Peripheral facial nerve palsy is the commonest cranial nerve motor neuropathy. The causes range from cerebrovascular accident to iatrogenic damage, but there are few reports of facial nerve paralysis attributable to odontogenic infections. In majority of the cases, recovery of facial muscle function begins within first three weeks after onset. This article reports a unique case of 32-year-old male patient who developed facial pain followed by unilateral facial nerve paralysis due to odontogenic infection. The treatment included extraction of the associated tooth followed by endodontic treatmen

Curious Case of Facial Pain Mimicking Temporomandibular Disorder.

Facial pain is a common but complex complaint, frequently associated with dental issues or temporomandibular disorder (TMD). However, rare aetiologies can complicate conventional diagnoses and treatment approaches. We present a case of a 36-year-old male with persistent jaw pain and restricted mandibular movement, initially managed as a typical TMD case. Conventional treatments yielded no improvement, prompting advanced imaging, which identified an unusual mass within the lateral pterygoid muscle. A surgical excision of the mass was performed, and histopathological examination revealed a rare

Bilateral Atypical Facial Pain Caused by Eagle's Syndrome.

Recurrent throat pain, "foreign body" sensation, difficulty in swallowing, or vague facial pain is many times caused by the presence of an elongated styloid process. Many times, this condition is misdiagnosed and the patient is treated for facial neuralgia. But once Eagle's syndrome is confirmed by clinical and radiological examination, the treatment is always surgical resection. The approach maybe intraoral or extraoral. In this paper, we present a case of Eagle's syndrome caused by bilateral elongation of the styloid process and where surgical resection of the same gave instant permanent rel

Persistent Idiopathic Facial Pain Originating from Cervical Abnormalities.

BACKGROUND: Persistent idiopathic facial pain is characterized by persistent facial or oral pain in the absence of a neurologic deficit. This underexplored pain may be conducted by various nerves, including cranial nerves and upper cervical spinal roots, and its etiology is unclear. CASE DESCRIPTION: A patient presented with persistent idiopathic facial pain associated with occipital muscle stiffness after an improper neck massage. The patient achieved almost complete pain relief by coblation of right upper cervical nerves (C1 and C2 spinal roots) followed by continuous cervical epidural anal

Altered cortical excitability in persistent idiopathic facial pain.

INTRODUCTION: Persistent idiopathic facial pain is a refractory and disabling condition of unknown mechanism and etiology. It has been suggested that persistent idiopathic facial pain patients have not only peripheral generators of pain, but also central nervous system changes that would contribute to the persistence of symptoms. We hypothesized that persistent idiopathic facial pain would have changes in brain cortical excitability as measured by transcranial magnetic stimulation compared to healthy controls. METHODS: Twenty-nine persistent idiopathic facial pain patients were compared to ag

Brief pain inventory--facial minimum clinically important difference.

OBJECT: Neurosurgeons are frequently the primary physicians measuring pain relief in patients with trigeminal neuralgia (TN). Unfortunately, the measurement of pain can be complex. The Brief Pain Inventory-Facial (BPI-Facial) is a reliable and validated multidimensional tool that consists of 18 questions. It measures 3 domains of pain: 1) pain intensity (worst and average pain intensity), 2) interference with general activities of daily living (ADL), and 3) face-specific pain interference. The objective of this paper is to determine the patient-reported minimum clinically important difference

Sleep quality and facial pain in fibromyalgia syndrome.

AIMS: This study aims to evaluate the sleep conditions in fibromyalgia syndrome and the influence of the temporomandibular disorders (TMDs) and fibromyalgia association in self-reported sleep quality. METHODOLOGY: Forty female patients with fibromyalgia (FMS) were compared with 40 healthy women [control group (CG)]. Three questionnaires were used (i.e. RDC/TMD to diagnose TMD and to determine pain intensity and disability and Pittsburgh Sleep Quality Index (PSQI) and Epworth Sleepiness Scale (ESS) to assess sleep conditions). Statistical analyses were performed using the Wilcoxon-Mann-Whitney

Acute right-sided facial pain: a case report.

AIM: To report a case of mixed neuralgic and dental pain and to discuss the possible pathophysiological background. SUMMARY: A case of coexistent dental and neuralgic pain in the distribution of the trigeminal nerve is presented, and the endodontic and neurological treatments are described. The possible diagnostic and pathophysiological correlations are discussed. KEY LEARNING POINTS: * Pre-trigeminal and atypical neuralgias are amongst the possible differential diagnoses of acute dental pain. * In a patient with nonodontogenic pain, simultaneous dental pain in the same area could be overloo

[Chronic facial pain due to a brainstem cavernoma].

Idiopathic trigeminal neuralgia is almost always associated with pathological nerve/vessel contact. Symptomatic forms of trigeminal neuralgia include cases of multiple sclerosis, infratentorial tumours and postherpetic neuralgia. Vascular malformations causing neuralgia have rarely been reported. We present the case of a 55-year old woman, who suffered from facial pain and ptosis on her left side. Repeated neurological examinations as well as repeated magnetic resonance imaging did not lead to a definite diagnosis or therapy. The pain suddenly stopped three weeks before admission and only a sl

Atypical facial pain: clinical considerations and differential diagnosis.

Atypical facial pain (ATFP), recently defined as persistent idiopathic facial pain by the revision of the Classification of the International Headache Society (IHS), is a poorly understood condition, which still lacks clear diagnostic criteria and proper treatment. The pain is described as "persistent facial pain that does not have the characteristics of cranial neuralgias and is not attributable to another disorder". In general, however, according to the IHS criteria, a diagnosis of ATFP is possible when the pain in the face is present daily and persists for most or all of the day. The pain i
