What causes stress incontinence: Fallacies, fascias and facts.

The traditional view of the pathophysiology of stress urinary incontinence (SUI) was that a loss of urethral support was the primary abnormality present. Recent research has challenged this hypothesis, demonstrating that impaired urethral function plays the key causative role. Improving our understanding of the underlying pathologic mechanisms is important to identifying the cause of treatment failures and for developing novel therapies to treat SUI.

Direct insertion of the medial rectus capsulopalpebral fascia to the tarsus.

PURPOSE: To clarify the insertion of the medial rectus capsulopalpebral fascia to the tarsus in Asians. METHODS: Specimens from 19 (11 right, 8 left) postmortem medial eyelids and orbits of 11 Asians (aged 45-96 years at death) were used. Samples had been fixed in 10% buffered formalin before their removal and microscopic examination. The tarsi were incised at 2 different heights in the upper and lower eyelids, as it was not disclosed which parts had the insertion of the medial rectus capsulopalpebral fascia. The first and second sections, parallel to the eyelid margin, were obtained, respect

Dimensional stability of the free fascia grafts: an animal experiment.

OBJECTIVES/HYPOTHESIS: It appears that autologous free fascia grafts (fascia lata and temporal fascia) change their dimensions during the vital first 5 days of healing. Poor dimensional stability of these grafts can be an important reason for failure of complete closure of tympanic membrane perforations in tympanoplasty operations. There has been no study regarding this dimensional instability. STUDY DESIGN: Prospectively dimensional instability of the free fascia grafts studied in 14 mongrel dogs. METHODS: Fourteen healthy Mongrel dogs were operated on twice. During the first surgery, fasci

A microdissected thin tensor fasciae latae perforator flap.

A new method, named "microdissection," has been introduced to create a thin flap by elevating the tensor fasciae latae perforator flap to serve as microdissected thin tensor fasciae latae perforator flap. In microdissection, perforators that run in the posterolateral direction in the adipose tissue after penetrating the deep fascia are dissected meticulously using an operative microscope, and a thin flap is elevated in a single process. The caliber of the perforator artery and vein in the tensor fasciae latae muscle measures approximately 0.7 mm and 0.9 mm, respectively. When transplanting the

A case of chronic expanding hematoma in the tensor fascia lata.

We describe a patient in whom a chronic expanding hematoma was pathologically confirmed by examination of the resected specimen. It developed gradually in the left lateral thigh, which had been bruised 8 months before, but had produced no mass-related symptoms initially. Computed tomography (CT) showed the mass to be homogeneous with capsule formation, whereas ultrasonography showed it to resemble a multilocular cyst. The lesion was treated by complete excision of a perifascial mass. Grossly, a well-circumscribed, fusiform mass with a soft cystic center and a fibrous pseudocapsule was observed

Use of a parotid fascia flap to prevent postoperative fistula.

OBJECTIVE: The purpose of this study was to investigate the usefulness of a fascia flap technique designed to improve the post-operative results of regional excision in cases of benign tumor in the superficial lobe of the parotid gland and to reduce formation of postoperative fistula. STUDY DESIGN: During surgery in each of 32 patients with benign tumor in the superficial lobe of the parotid gland, a fascia flap was raised from beneath the ear lobe, placed in its original position, and firmly sutured after regional resection of the tumor. The results were compared with those in a control grou

Pharyngoesophageal reconstruction with a tensor fasciae latae free flap.

This report describes our experience with the use of a tensor fasciae latae flap in total reconstructions of the hypopharynx and cervical esophagus. This flap is durable, reliable, and easy to elevate. Moreover, it is possible to obtain a tight two-layer closure of the circumferential suture lines by means of the vascularized fascia. We think this flap will be one of the methods of choice in reconstructions of the hypopharynx and cervical esophagus.

Free temporoparietal fascia flap in reconstruction of the lower extremity.

Twenty-one patients with open wounds of the non-weight-bearing foot or the ankle underwent coverage with a free temporoparietal fascia flap and split-thickness skin graft. Our indications for this thin, well-vascularized flap included osteomyelitis and exposed tendon, bone, or fixation hardware. The pliability of the flap allowed superior restoration of natural anatomical contours. Only one flap was completely lost (95.2% overall success). There were four cases (20%) of partial flap loss, none of which required flap revision or a new flap. Four patients (19%) experienced transient donor-site a

The importance of the endopelvic fascia repair during vaginal hysterectomy.

During 1985 to 1989, 177 vaginal hysterectomies were performed in the Department of Gynecology, Kaplan Hospital, Rehovot, Israel, using the Porges technique with some modifications. Ninety patients had some degree of loss of the pelvic support--anterior or posterior wall relaxation, enterocele or uterine prolapse in various degrees. The patients were allocated to two groups, in which two different techniques were compared: group 1, with repair of the pubocervical and pararectal fascia and group 2 without the repair. The repair of the pubocervical and pararectal fascia after vaginal hysterectom

[Fascia canals of the greater sciatic foramen and their practical importance].

Fasciae and fat tissue spaces in the gluteal region, topography of the suprapiriform and infrapiriform foramina have been studied by means of a complex anatomical experimental technique. The suprapiriform foramen should be considered as a fascialosseous canal, as it is 4-4.5 cm long and 0.6-1.0 cm wide. It is formed by the upper margin of the greater sciatic notch covered with a thin fascia, fasciae of the gluteal and piriform muscles and the parietal layer of the pelvic fascia. The proper fascial vaginae of the upper gluteal vessels and nerves are adhered to fascial walls of the canal. This p

Whole mount microscopic sections reveal that Denonvilliers' fascia is one entity and adherent to the mesorectal fascia; implications for the anterior plane in total mesorectal excision?

BACKGROUND: Excellent anatomical knowledge of the rectum and surrounding structures is essential for total mesorectal excision (TME). Denonviliers' fascia (DVF) has been frequently studied, though the optimal anterior plane in TME is still disputed. The relationship of the lateral edges of DVF to the autonomic nerves and mesorectal fascia is unclear. We studied whole mout microscopic sections of en-bloc cadaveric pelvic exenteration and describe implications for TME. METHODS: Four donated human adult cadaveric specimens (two males, two females) were obtained from the Leeds GIFT Research Tissu

Histological analysis, viscoelastic characterization, and modeling of human plantar fascia.

The human plantar fascia is a soft connective tissue that extends from the calcaneal tuberosity to the metatarsal region, providing static support to the foot and functioning as a shock absorber. Its roles and pathology are closely linked to its collagen fiber composition. Therefore, in-depth investigation of the mechanics of this tissue, an area with limited prior research, is crucial, especially for developing accurate in silico foot models for clinical and surgical purposes. This study aims to address this gap by combining experimental procedures, microscopic analysis of collagen fibers, an

Modeling thoracolumbar fascia mechanical tensile behavior with microstructure-level descriptors.

Modeling of fasciae remains limited, despite their recognized role in chronic pain. Developing a comprehensive mechanical model of fasciae could significantly enhance our understanding of their pain-related mechanisms and improve their prevention. This paper presents a computational approach capable of simulating the mechanical behavior of fibrous tissues based on their mesostructure. The thoracolumbar fascia was selected as a case study due to the availability of its experimentally derived mechanical properties in the literature. A discrete element model was developed, representing collagen f

Immunosuppression-free Nonvascularized Rectus Fascia Allotransplantation for Complex Hernia Repair.

BACKGROUND: Complex abdominal wall herniations present major reconstructive challenges, particularly in patients with contaminated fields or previous failed repairs. Synthetic meshes carry substantial infection risk, whereas biological meshes are expensive and exhibit inconsistent long-term strength, leaving many patients without dependable options. This study investigates the feasibility, safety, and immunologic profile of nonvascularized rectus fascia allotransplantation (NVRF-alloTx) for abdominal wall reconstruction in immunocompetent individuals. METHODS: We conducted a prospective singl

Validation of speckle tracking analysis for assessing fascia sliding mobility.

Fascia sliding mobility and deformation magnitude are potential biomarkers for musculoskeletal disorders, particularly in the thoracolumbar fascia over the erector spinae muscles, which are associated with low back pain. The use of speckle tracking analysis of ultrasound images through open-source software has been proposed for assessing fascia sliding mobility and deformation of the fascia. However, little is known about the validity and reliability of speckle tracking analysis. Since open-source projects for speckle tracking analysis have made great progress, an assessment of validity and re
