[Conservative treatment of neurogenic fecal incontinence. Our experience with children].

OBJECTIVE: The objective of this study was to present the use of retrograde colon enema in myelodysplasic patients with fecal incontinence. PATIENTS AND METHODS: Thirty-three patients, 16 males and 19 females, 5 to 22 years old (average age 11.9 years) with spina bifida and fecal incontinence which obliged them to use a diaper. Retrograde colon enema procedures were performed in all of them. RESULTS: With the use of the retrograde enema the best rectal emptying was achieved and kept 32 children (97%) diaper free after a follow-up period of more than two years. Only one girl (3%) needed pads

Home-Based Biofeedback for Fecal Incontinence: A Randomized Clinical Trial.

IMPORTANCE: Access to biofeedback, a first-line treatment for fecal incontinence (FI), is limited. OBJECTIVE: The objective of this study was to evaluate whether an 8-week duration of a home-based digital therapeutic pelvic health system to treat FI in women is equivalent to a 16-week duration. STUDY DESIGN: We conducted a single-blind, randomized, equivalence trial in women with FI. Participants were randomized to 8 or 16 weeks of twice-daily exercises using the system and could opt to participate in remote health coaching. The primary outcome was the change in FI severity as measured by th

Anoreceptive Sexual Intercourse and Fecal Incontinence: A Cross-Sectional Survey.

INTRODUCTION AND HYPOTHESIS: Anoreceptive sexual intercourse (ASI) is a common sexual practice, yet its relationship with bowel continence remains insufficiently characterized. Evidence evaluating patient-reported continence outcomes and behavioral context is limited. The objective was to evaluate the association between ASI and self-reported fecal incontinence (FI), and to assess its relationship with continence-related quality of life and specific ASI-related practices. METHODS: We conducted a cross-sectional analytical study using an anonymous online survey distributed to a self-selected a

Baiona's Consensus Statement for Fecal Incontinence. Spanish Association of Coloproctology.

Faecal incontinence (FI) is a major health problem, both for individuals and for health systems. It is obvious that, for all these reasons, there is widespread concern for healing it or, at least, reducing as far as possible its numerous undesirable effects, in addition to the high costs it entails. There are different criteria for the diagnostic tests to be carried out and the same applies to the most appropriate treatment, among the numerous options that have proliferated in recent years, not always based on rigorous scientific evidence. For this reason, the Spanish Association of Coloprocto

Transumbilical laparoscopy-assisted Malone procedure for fecal incontinence in children.

BACKGROUND: Transumbilical laparoscopy-assisted Malone procedure (TULAM) is a single-incision laparoscopic procedure in which the appendicostomy is made at umbilicus. The aim of this study is to evaluate the outcomes of TULAM. METHODS: With IRB approval, the medical records of the patients who underwent TULAM were retrospectively reviewed between July 2013 and December 2018. The data collected included ages, gender, underlying diseases, operative techniques, complications, stoma continence, parental satisfaction and follow-up duration. RESULTS: Fifteen patients underwent TULAM at the median

Posterior tibial nerve stimulation for fecal incontinence: where are we?

Neurostimulation remains the mainstay of treatment for patients with faecal incontinence who fails to respond to available conservative measures. Sacral nerve stimulation (SNS) is the main form of neurostimulation that is in use today. Posterior tibial nerve stimulation (PTNS)--both the percutaneous and the transcutaneous routes--remains a relatively new entry in neurostimulation. Though in its infancy, PTNS holds promise to be an effective, patient friendly, safe and cheap treatment. However, presently PTNS only appears to have a minor role with SNS having the limelight in treating patients w

A novel magnetic device to prevent fecal incontinence (preliminary study).

BACKGROUND AND AIMS: In this research we propose an original magnetic device to strengthen the hypo-atonic anal sphincter and prevent fecal incontinence. METHODS: The device consists in a couple of small magnetic plaques to be surgically inserted in the wall of the anal canal between the external and internal anal sphincters with the opposite polarities face to face, so that, attracting themselves, close the anal lumen. Three couples of magnets made of materials of different magnetic force (neodymium>ferrite>plastoferrite) were evaluated in each of three swine anatomical preparations by measu

Left-colon antegrade continence enema (LACE) procedure for fecal incontinence.

PURPOSE: Antegrade continence enemas (ACE) are an efficacious therapeutic option for patients with fecal incontinence. The authors review their institution's experience with a variation of the Monti-Malone ACE procedure using the left colon as a source of an intestinal conduit and enema reservoir. METHODS: From 2000 to 2002, 18 patients with fecal incontinence or intractable constipation underwent left-colon ACE (LACE) procedure. Concomitant Mitrofanoff appendicovesicostomy was performed in 15 patients and bladder augmentation in 9. The majority of patients had neural tube defects. A segment

Antegrade continent enema management of chronic fecal incontinence in children.

BACKGROUND/PURPOSE: Fecal incontinence impedes social and psychological development in children. Conventional bowel management with rectal enemas, medication, and biofeedback frequently will provide intervals free of fecal soiling sufficient for children to be socially continent. METHODS: This report details the improvement achieved by antegrade irrigation of the colon when conventional bowel management programs have failed to provide satisfactory intervals free of fecal soiling. Twenty-three children had conduits performed to administer antegrade continent enemas in the colon from December 1

Fecal incontinence is not associated with UTI: A contemporary case-control study.

INTRODUCTION: Urinary tract infections (UTIs) are a leading cause of infection in adults. The most common cause is gastrointestinal bacteria ascending the urethra into the bladder. Studies showing fecal incontinence (FI) is a risk factor for UTI have been limited to nursing home populations. Healthy patients with recurrent UTI, especially women, often receive counseling, suggesting improper personal hygiene contributes to UTIs. This advice can be stigmatizing. Given UTI prevalence, it is important to elucidate risk factors for improved diagnosis, treatment, and patient education. Our objective

[Current status and prospects of non-surgical treatment for fecal incontinence].

Fecal incontinence is a refractory disease in colorectal surgery. The main clinical manifestation is that patients cannot control the discharge of gas, solid or liquid feces in the rectum autonomously. It is easy to bring shame to patients and seriously affect their physical and mental health. Reducing the frequency of fecal incontinence, restoring anal sphincter function, and improving patient quality of life are important goals for treating fecal incontinence. With the development of medical technology and the improvement of treatment plans for fecal incontinence, patients with fecal inconti

Impact of fecal incontinence and its treatment on quality of life in women.

Fecal incontinence (FI) is a physically and psychosocially debilitating disorder which negatively impacts quality of life (QOL). It bears a significant burden not only on patients but also on their families, caretakers as well as society as a whole. Even though it is considered a somewhat common condition, especially as women age, the prevalence is often underestimated due to patients' reluctance to report symptoms or seek care. The evaluation and treatment of FI can be also hindered by lack of understanding of the current management options among healthcare providers and how they impact on QO
