[Hip arthroplasty for failed internal fixation of femoral neck fractures].

OBJECTIVE: To investigate the result of hip arthroplasty for failed internal fixation of femoral neck fractures. METHODS: From June 2007 to January 2014, 29 cases who underwent hip arthroplasty for failed of internal fixation of femoral neck fractures were reviewed. There were 12 males and 17 females. The mean age was 60.3 years (ranged 43 to 83 years) at the time of the fracture. Left hip was in 16 cases, right hip was in 13 cases. The average interval from fracture to arthroplasty was 23.3 months (ranged, 3 to 48 months). RESULTS: All of 29 cases were performed total hip arthroplasty. Ther

Decreasing incidence of femoral neck fractures in the Medicare population.

This study was designed to evaluate trends in incidence and treatment patterns for intracapsular hip fractures and to evaluate risk factors for complications and mortality. Patients with an intracapsular hip fracture who underwent internal fixation, hemiarthroplasty, or total hip arthroplasty (THA) were identified from a 5% nationwide sample of Medicare data (1998-2007). The authors identified 41,053 patients with intracapsular hip fractures between 1998 and 2007. The number of intracapsular hip fractures treated with internal fixation, hemiarthroplasty, or THA decreased by 21.8%, from 4602 in

Ipsilateral femoral shaft and neck fractures: are cephalomedullary nails appropriate?

A retrospective review was conducted to examine rates of malreduction and nonunion in ipsilateral femoral neck and shaft fractures using different fixation strategies. Twenty-two consecutive patients with 23 fractures were identified. Participants were treated with various fixation strategies for ipsilateral femoral neck and shaft fractures. Cephalomedullary devices were used in 13 cases, while cannulated screws and a retrograde femoral nail were used in nine cases. One patient was treated with cannulated screws and external fixation of the femoral shaft. Radiographic assessment of the quality

Cemented Thompson versus cemented bipolar prostheses for femoral neck fractures.

PURPOSE: To compare early functional outcomes, complications, and mortality in elderly patients treated with the less costly, cemented Thompson prosthesis or the cemented bipolar prosthesis in order to identify factors affecting outcomes. METHODS: Records of 303 patients with femoral neck fractures treated with the cemented Thompson monoblock prosthesis (n=206) or the cemented bipolar prosthesis (n=97) were reviewed. The choice of prosthesis was solely determined by surgeon's preference. Data relating to patient demographics, clinical and residential status, mobility, mental function, mortali

Concomitant ipsilateral femoral neck and shaft fractures--analysis of cases.

Concomitant ipsilateral shaft and femoral neck or pertrochanteric fractures are uncommon. They result from high-energy traumas and the mechanism of injury is usually complex. The proximal fracture is often not displaced and may be hardly visible on x-rays. Overlooking the proximal fracture may result in therapeutic problems. The best method of treatment is by closed stabilization of both fractures with a reconstruction nail. In our opinion, concomitant neck and femoral shaft fractures demand particular alertness in x-ray analysis, precision in fracture stabilization and usually longer patient

Delayed fixation of displaced femoral neck fractures in younger adults.

We report the results of 52 patients aged 20-55 years with displaced femoral neck fractures, in whom delayed closed reduction and internal fixation was performed. Seven patients developed avascular necrosis (AVN) and non-union was seen in five patients. Whereas most patients with non-union were subjected to repeat procedures, none of the patients with AVN required surgery. The follow-up averaged 40 months (range 22-64 months). Three patients were lost to follow-up. Functional outcome was defined by Judet's system. Good to excellent functional outcome was achieved in 45 cases. The study demonst

Surgical treatment of undisplaced femoral neck fractures in the elderly.

The study was a retrospective evaluation and comparison. Eighty-four elderly patients (> 60 years) with undisplaced intracapsular femoral neck fractures were treated with osteosynthesis with either dynamic hip screws (DHS) or multiple cannulated screws (MCS). The Singh index was used to evaluate bone quality. All patients were followed up retrospectively for at least 12 months. The clinical results were compared between the DHS and MCS groups. Both groups were similar in respect of injury mechanisms, mean Singh index, injury-surgery interval, gender and age (all p values > or = 0.29). The MCS

Ipsilateral femoral neck and shaft fractures: complications and their treatment.

An ipsilateral femoral neck fracture occurs in approximately 6% to 9% of all femoral shaft fractures. Despite this relatively common presentation, decision-making often is difficult. Furthermore, the risk for complications is greater in the treatment of this combination injury pattern than for single-level fractures. A retrospective review of the authors' large trauma database revealed 13 patients who had healing complications develop after their index surgical procedure. Six of the eight (75%) femoral neck nonunions occurring in these 13 patients developed after the use of a second generation

Noncemented Cathcart elliptical head endoprosthesis for displaced femoral neck fractures.

A total of 93 patients (95 hips) undergoing unipolar noncemented elliptical head endoprosthetic replacement for an acute displaced femoral neck fracture were reviewed clinically and radiographically at an average follow-up of 28 months. The 12-month mortality rate was 22%. The medical complication rate was 15%, and the surgical complication rate was 19%. At most recent follow-up, 66% of patients used an assist device for ambulation or were nonambulatory. Of patients, 64% required full-time nursing care. Radiographically, subsidence of the component was identified in 66% of the hips and acetabu

Bilateral pathological femoral neck fractures secondary to ectopic parathyroid adenoma.

A rare case of bilateral pathological femoral neck fractures secondary to ectopic parathyroid adenoma is reported. Both hip fractures were treated with closed reduction and internal fixation. The right hip fracture healed uneventfully, whereas the left hip fracture developed non-union that required subtrochanteric osteotomy and internal fixation and eventually healed. Neither hip developed avascular necrosis.

Prediction of late complications of femoral neck fractures by scintigraphy.

A prospective study was undertaken to determine the value of scintigraphy in predicting nonunion or necrosis of the femoral head following femoral neck fractures. Scintigraphy was carried out in 49 patients before operation and in 37 one week after operation; in 18 it was done before and after operation. The uptake of isotope was estimated visually as either normal or reduced compared to the opposite side. All the patients were followed up for at least 2 years. The results indicate that pre-operative bone scintigraphy is useful in elderly patients to predict nonunion and necrosis, and therefor

[Traumatology in the elderly. Problems exemplified by femoral neck fractures].

The most frequent complications in response to a fracture of the femoral neck, a typical injury of the elderly, are necrosis of the head of femur and pseudoarthrosis of the femoral neck. Only the around dressed down fracture can be conveyed to conservative treatment. The classification of AO takes these problems into consideration. One possibility to preserve the femur head is by operative insertion of a dynamic hip screw (DHS). Specific problems of the diverse types of fracture and their surgical details are taken into special account.

Serum copper levels in elderly patients with femoral-neck fractures.

The serum copper levels of 46 elderly patients with fractures of the femoral neck were assayed and found to be significantly lower than those of a group of controls matched for age and sex. These findings are consistent with nutritional copper deficiency which may contribute to the development of fractures by reducing bone strength.

[Treatment of femoral neck fractures using a total hip prosthesis].

From October 1. 1977 to December 1. 1982 106 patients suffering from fractures of the femoral neck (n = 75) or necroses of the femoral head and nonunions of the neck fracture respectively (n = 33) underwent total hip replacement by TEP (type Müller-Charnley). The results obtained were much better in comparison to other methods of treatment: less complications, shortening of in-hospital stay (average 21.1 days) and satisfactory till good late results in 82.8%. The mortality rate came up to 13.9% (fractures: 20.0%, necrosis of the head and nonunion: 0.0%). Concerning complications we observed wo

Femoral neck fractures. 165 cases treated by multiple percutaneous pinning.

A prospective series of 165 displaced subcapital fractures of the femoral neck treated with reduction and percutaneous multiple pinning is presented. The method offers the advantages of a short operating time, no loss of blood, immediate weight-bearing and no infection problems. Furthermore the operative procedure can be performed under local anaesthesia. In Garden stage 3 fractures union occurred in 97 per cent and late segmental collapse in 8 per cent. In Garden stage 4 fractures union occurred in 72 per cent and late segmental collapse in 14 per cent. Failures were almost exclusively encoun

Principles or Resources? Best Management of Pediatric Femoral Neck Fractures.

BACKGROUND: Pediatric femoral neck fractures (PFNF) are rare but have a reported complication rate of >30%. To date, there remains a lack of consensus on the definitive risk factors for AVN following PFNF. By comparing the outcomes of PFNF treated at a major US metropolitan area tertiary-level 1 pediatric trauma center (group 1) to an equivalent trauma hospital in a major Latin American capital (group 2), we sought to weigh whether treatment principles or resources are more important for a favorable result. METHODS: This was a retrospective review of all children (<18 years old) treated for P
