Incidence of Delayed Recovery from Femoral Nerve Block in Total Knee Arthroplasty.

OBJECTIVE: Femoral nerve block has been proven as an effective analgesia for total knee arthroplasty (TKA). Delayed recovery from nerve block can result in serious complication during postoperative period. This prospective, single-center, observational study investigated the incidence in delayed recovery from femoral nerve block more than 24 hours postoperatively. MATERIAL AND METHOD: Two hundred and forty patients with femoral nerve block as part of anesthesia plan for elective unilateral TKA were recruited into study. Participants were assessed for sensory or motor impairment lasting longer

Lateral femoral cutaneous nerve block after total hip arthroplasty: a randomised trial.

BACKGROUND: Peripheral regional nerve blocks are commonly used for pain management after lower extremity surgery, but motor blockade can be a significant concern. The lateral femoral cutaneous nerve (LFCN) is a purely sensory nerve from the lumbar plexus. We hypothesised that an LFCN block would reduce movement-related pain after total hip arthroplasty (THA) in patients with moderate-to-severe pain. METHODS: Sixty patients with visual analogue scale (VAS) score > 40 mm during 30-degree active flexion of the hip on either the first or second postoperative day after THA were included in this pr

Femoral nerve palsy after pelvic fracture treated with INFIX: a case series.

OBJECTIVE: The treatment of some pelvic injuries has evolved recently to include the use of a subcutaneous anterior pelvic fixator (INFIX). We present 8 cases of femoral nerve palsy in 6 patients after application of an INFIX to highlight this potentially devastating complication to pelvic surgeons using this technique and discuss how it might be avoided in the future. DESIGN: Retrospective chart review. Case series. SETTING: Five level 1 and 2 trauma centers, tertiary referral hospitals. PATIENTS/PARTICIPANTS: Six patients with anterior pelvic ring injury treated with an INFIX who experien

Isolated sciatic nerve entrapment by ectopic bone after femoral head fracture-dislocation.

Although posttraumatic pelvic heterotopic ossification (HO) after hip fracture dislocation is well established, and nerve encasement by HO may occur, the development of neurologic deficit is rare. A thorough history and adequate clinical suspicion are imperative in the workup of affected patients. Computed tomography and magnetic resonance imaging provide good visualization and assist in surgical planning. If symptoms persist and are recalcitrant to conservative management, surgical intervention with HO excision and nerve neurolysis can be performed with success.

Continuous femoral nerve block versus patient-controlled analgesia following total knee arthroplasty.

PURPOSE: To compare total knee arthroplasty (TKA) patients who received continuous femoral nerve block (FNB) with local anaesthetics through a catheter versus patient-controlled analgesia (PCA) with intravenous morphine. METHODS: 50 women and 10 men aged 51 to 84 years with matched characteristics underwent TKA and received either continuous FNB with local anaesthetics through a catheter (n=30) or PCA with intravenous morphine (n=30). None of the patients had had previous knee surgery. All operations were performed according to the standard protocol. Daily mean pain numerical rating scale at

[Acute groin pain and femoral nerve deficit in a warfarin treated patient].

Warfarin is frequently used as an anticoagulant. The most common and serious complication is bleeding. We present a 43-year-old warfarinized patient who was admitted with unilateral groin pain and a neurological deficit of the lumbar nerve root L1-4. A CT scan showed a haematoma in and behind the ilio-psoas muscle extending to below the inguinal ligament, due to a warfarin induced coagulation deficit (INR 7.0) which was reversed on admission with vitamin K. There is no conclusive evidence in the literature supporting either conservative or surgical treatment modalities. Our patient was success

Femoral nerve palsy associated with an associated posterior wall transverse acetabular fracture.

A case is presented of a patient who sustained bilateral acetabular fractures in a motor vehicle crash. On the right, he sustained a variant associated posterior wall transverse fracture, with a femoral nerve lesion noted preoperatively. Open reduction and internal fixation of both fractures was performed in a staged fashion with an ilio-inguinal approach on the right. At surgery, the femoral nerve was 90% divided and damaged over approximately 5 cm of length, with marked damage also of the ilio-psoas muscle and tendon and the inguinal canal. No attempt was made to repair the nerve, and at 11-

Peroneal nerve palsy after early cast application for femoral fractures in children.

The incidence and contributing factors associated with post-casting peroneal nerve palsy were examined in a series of 110 consecutive pediatric femoral shaft fractures treated with early hip spica cast application. Four patients with peroneal nerve palsy were identified. All four had 90 degrees/90 degrees casts placed and underwent cast wedging for alignment. All palsies resolved with immediate cast removal. Other treatment options for certain femur fractures with significant initial shortening should be considered. We advise pre- and post-cast neurologic examination and avoidance of forceful

A rare lumbosacral trunk variation: communicating branches to the femoral and obturator nerves.

BACKGROUND: This report presents a case of bilateral lumbosacral trunk (LST) variation observed during cadaveric dissection and to analyze its possible developmental mechanism and clinical significance. MATERIALS AND METHODS: During the dissection of a Chinese adult male specimen, bilateral variations of the LST were observed, with communicating branches joining the femoral nerve and/or obturator nerve. RESULTS: The right LST was formed solely by the ventral ramus of the fifth lumbar nerve (L5), without contribution from the fourth lumbar nerve (L4), and gave off one communicating branch (1.

Ultrasound-Guided Regional Anesthesia of the Femoral Nerve in the Pediatric Emergency Department.

OBJECTIVES: Femur fractures are painful, and use of systemic opioids and other sedatives can be dangerous in pediatric patients. The fascia iliaca compartment nerve block and femoral nerve block are regional anesthesia techniques to provide analgesia by anesthetizing the femoral nerve. They are widely used in adult patients and are associated with good effect and reduced opioid use. Ultrasound (US) guidance of nerve blocks can increase their safety and efficacy. We sought to report on the use and safety of US-guided regional anesthesia of the femoral nerve performed by emergency physicians for

Ultrasound of the Lateral Femoral Cutaneous Nerve: A Review of the Literature and Pictorial Essay.

We review the ultrasound (US) findings in patients who present with meralgia paresthetica (MP). The anatomy of the lateral femoral cutaneous nerve at the level where the nerve exits the pelvis and potential entrapment sites that can lead to MP are discussed. A wide range of pathological cases are presented to help in recognizing the US patterns of MP. Finally, our experience with US-guided treatment is discussed.

Lesion of the femoral nerve caused by a hydatid cyst of the right psoas muscle.

INTRODUCTION: Hydatidosis is a human disease caused by the larval form of Echinococcus granulosus. All organs in the human body may be affected by hydatid disease, but excluding liver and lungs, all other organs are considered as uncommon locations. Hydatid disease located in the psoas muscle is uncommon. CASE OUTLINE: The authors present a 36-year-old male living in endemic areas of Serbia, admitted due to pain and weakness of the right thigh and weight loss. Duration of symptoms was one year. CT and MRI revealed a big cystic mass (20 cm long) in the right psoas muscle. Neurological investig

Alcohol neurolysis of the sciatic and femoral nerves to improve pressure ulcer healing.

UNLABELLED: Successful pressure ulcer treatment is challenging and is often plagued with prolonged hospitalizations, multiple surgeries, and high recurrence rates. Pressure ulcer secondary to spinal cord injury is further complicated by spasticity, which contributes to both ulcer continuance and healing. This report illustrates the use of neurolytic regional techniques for spasticity control and pressure ulcer healing. CASE REPORT: We present our experience with a paraplegic man who suffered from chronic right trochanteric and ischial pressure ulcers that failed to heal despite surgical and c

Effect of lithotomy positions on strain of the obturator and lateral femoral cutaneous nerves.

The purpose of this study was to determine if various lithotomy positions increase strain on the obturator and lateral femoral cutaneous nerves in fresh adult cadavers. A static load cell was used to record strain changes of the obturator and lateral femoral cutaneous nerves in the pelvis and anterior thigh when the lower limbs were placed in three sets of positions of varying hip abduction and flexion. The means of the strain measurements, which were measured in grams in all positioning angles were compared to the baseline 0 degrees measurements. Analysis of variance was calculated for the di

[A case of multifocal neurinoma of the femoral nerve associated with chronic myeloblastic dysplasia].

A large multiple lobulated painless mass in the inguinal fossa, was investigated and removed, in a 58 years old patient with undiagnosed chronic myelodysplasia. The patient was initially referred to the urologic clinics for recurrent bleeding from the urinary tract, when the above mentioned and few smaller masses were palpated on the anterior aspect of the thigh. Clinical, laboratory and imaging investigation revealed enlargement of the spleen and the liver accompanied by relatively large and well separated tumors under the inguinal ligament extending to the upper thigh. From clinical history
