Fever of Unknown Origin From the Primary Care Perspective: A Case Report.

A 60-year-old immunocompetent man presented to his primary care physician with 3 weeks of fever without an obvious source or risk factors. After multiple encounters, he was found to have a pyogenic liver abscess on computed tomography (CT). He improved with interventional radiology (IR) drainage and antibiotics. Primary care and outpatient providers should be prepared to initiate an appropriate workup for fever of unknown origin (FUO). Family medicine providers working in acute care settings such as urgent care and the emergency department similarly need a framework for FUO. Once common causes

Subacute Thyroiditis Masking as Fever of Unknown Origin: An Intriguing Case Report.

This study presents a unique case of subacute thyroiditis, which presents as prolonged high-grade fever without any other symptoms except for mild throat pain. A 46-year-old, otherwise healthy male presented with high-grade fever for more than two to three weeks and was found to have hyperthyroidism, with elevated free thyroxine levels (free T4), low thyroid-stimulating hormone (TSH) levels, elevated c-reactive protein (CRP) an inflammatory marker, and heterogeneous bilateral thyroid nodules on imaging studies. His workup was negative for infectious etiology of fever, thus favoring the diagnos

Thyroid tuberculosis diagnosed as papillary thyroid carcinoma with fever of unknown origin.

Thyroid tuberculosis is a rare disease, very few cases have been reported. It is difficult to diagnose because of no typical characteristics. We report on a patient who underwent surgery for suspected thyroid carcinoma, but who was then diagnosed with thyroid tuberculosis. The patient was a woman in her 70s. She had been diagnosed with chronic renal failure and had been on peritoneal dialysis. She complained of fever and a painful left anterior neck swelling. Computed tomography showed thyroid tumor with cervical lymph node swelling, ultrasound-guided fine needle aspiration cytology was suspec

Fever of unknown origin in a patient with red ears: relapsing polychondritis.

Relapsing polychondritis (RP) is a rare autoimmune disease that affects cartilage and multiple organ system. We describe a case of RP presenting with fever, conjunctivitis and inflammatory signs of the right knee. Infectious and neoplastic diseases were excluded by imaging and laboratory examinations. After 17 days of admission the patient reported coughing and odynophagia, and, a physical examination showed red ears. Otorhinolaryngological examination demonstrated arytenoid chondritis. RP was diagnosed based on four McAdam's criteria. The patient was medicated with oral prednisone, and a posi

Fever of unknown origin in a Swiss-born child: don't miss tuberculosis!

Tuberculosis incidence is low in Switzer land. We report here on a Swiss-born toddler. Tuberculosis manifested with a fever of unknown origin, mimicking an inflammatory or autoimmune disorder triggering a high dose of corticosteroid treatment. The disease went unrecognized for several weeks until development of a miliary tuberculosis with advanced central nervous system involvement. This case highlights the difficulties encountered in diagnosing tuberculosis and in identifying the origin of this case. It reminds us that this disease must never be forgotten when facing a child with persistent f

Fever of unknown origin (FUO) due to cyclic neutropenia with relative bradycardia.

Fever of unknown origin (FUO) is the diagnosis associated with a variety of infectious, neoplastic, or rheumatic/inflammatory disorders. FUOs are associated with fevers of > or = 101 degrees F for > or = 3 weeks that remain undiagnosed after intensive in-hospital or ambulatory investigation. Many FUOs caused by rheumatic/inflammatory disorders are particularly difficult to diagnose because of the lack of specific diagnostic tests for many of these disorders. Several causes of obscure rheumatic/inflammatory fevers with a genetic ethnic or geographic disposition can be eliminated by history alon

Spontaneous intra-hepatic haemorrhage in a patient with fever of unknown origin.

Polyarteritis nodosa is a systemic necrotising vasculitis that affects the small- and medium-sized arteries. Multifocal aneurysmal formation in the renal, hepatic, and mesenteric vasculature is a hallmark of this condition, and spontaneous aneurysmal rupture may occur, resulting in life-threatening haemorrhage. We describe a 42-year-old man who initially presented with fever of unknown origin. A diagnosis could not be reached at that time despite extensive investigations. The fever subsided spontaneously after 8 weeks, and the patient remained well for 6 years until he was admitted again for e

Fever of unknown origin in 185 paediatric patients: a single-centre experience.

AIM: We conducted a prospective study to evaluate the causes and outcome in children with fever of unknown origin (FUO). METHODS: From 1990 to 1999, 185 children with FUO were evaluated. Initial evaluation included routine haematological analysis, Epstein-Barr virus (EBV) serology, urine, stool or blood cultures, chest X-ray and tuberculin probe. RESULTS: In 131 (70%) patients diagnosis was established, and 70 (37.8%) had infectious disease. EBV infection was the most common infection followed by visceral leishmaniasis (VL), urinary tract infection (UTI) and tuberculosis. Autoimmune disorder

[Fever of unknown origin with granulomatous hepatitis, uveitis and acute renal failure].

A 46-year-old man was referred to our hospital because of fever of unknown origin (FUO). Two months before admission, he noted fever, weight loss, and asthenia. Physical examination revealed only mechanical pains of the right shoulder. In the past, patient's father had a tuberculosis. At the age of 29 years, Brucella granulomatous hepatitis and B hepatitis were diagnosed. The past three years, he developed two episodes of uveitis which resolved with local treatment. The hepatic biopsy revealed only a non specific granulomatous hepatitis. Few days after admission, the patient developed acute re

Fever of unknown origin: keys to determining the etiology in older patients.

In light of improvements in imaging modalities and laboratory tests, fewer cases of fever of unknown origin (FUO) are being attributed to infectious causes and more are eventually being diagnosed as secondary to noninfectious causes, particularly tumors and connective tissue diseases. Older patients with FUO usually present with mild, nonspecific, normochromic, and normocytic anemia and an elevated erythrocyte sedimentation rate. The history, physical examination, and imaging studies are key to making a diagnosis. Although the results of laboratory tests are generally nonspecific, such tests a

Iodine-induced subacute thyroiditis with thyrotoxicosis presenting as fever of unknown origin.

A 26-year-old woman with features of bulimia nervosa presented with fever of unknown origin, hepatomegaly, marked leukocytosis, and increased erythrocyte sedimentation rate. Following prolonged observation, slight tenderness over the thyroid gland and signs of thyrotoxicosis occurred. A thyroid scan demonstrated no isotope uptake and the patient admitted abusing an organic iodine preparation in order to control her weight. The diagnosis of iodine-induced subacute thyroiditis with thyrotoxicosis was, therefore, considered. A brief course of low-dose steroids normalized both thyroid function and

Uterine Manifestation of Sarcoidosis Diagnosed in the Setting of Fever of Unknown Origin.

We report a rare case of uterine sarcoidosis. A 28-year-old woman presented with periodic fever over 38°C for the past 5 months. One year prior, splenomegaly was noted. The serum angiotensin-converting enzyme levels were elevated; however, the lungs, lymph nodes, eyes, and skin appeared normal. Fluorine-18-fluorodeoxyglucose positron emission tomography revealed accumulations in the uterine body and spleen. Magnetic resonance imaging showed multiple irregular uterine masses. Intraoperative frozen section assessment of the uterine masses suggested sarcoma; therefore, total hysterectomy was perf

Distribution of the causes of fever of unknown origin in China, 2013-2022.

BACKGROUND AND OBJECTIVES: Fever of unknown origin (FUO) has long been a cause for concern among clinicians, and its spectrum has evolved with progress in medicine. This study aimed to investigate the distribution of causes of FUO in China between 2013 and 2022 to facilitate the clinical understanding of the etiology of FUO. MATERIALS AND METHODS: Case series of FUO in China published between 2013 and 2022 were retrieved from PubMed, Wanfang Data, and CNKI databases and retrospectively analyzed. The rates of different causes of FUO were calculated, and these data were compared with previously

Cytomegalovirus Hepatitis in an Immunocompetent Patient: A Cause of Fever of Unknown Origin.

Acute hepatitis is most often self-resolving and a benign condition that rarely requires any anti-viral drugs. In immunocompromised patients (HIV-infected patients and transplant recipients), the morbidity and mortality associated with cytomegalovirus (CMV) infection have been extensively reported in the medical literature. We are describing a rare case of acute severe cytomegalovirus hepatitis in an immunocompetent host. In immunocompetent individuals, in most cases, it causes a subclinical infection and hence doesn't require an anti-viral agent for treatment. Our patient was unique because i
