BACKGROUND: Children with juvenile idiopathic arthritis (JIA) can experience significant physical impairment of the lower extremity. Prolonged joint disease and symptoms may cause gait alterations such as reduced walking speed and increased plantar pressures in diseased areas of their feet. There is limited robust clinical trials investigating the effect of non-invasive mechanical therapies such as foot orthoses (FOs) on improving gait parameters in children with JIA.
RESEARCH QUESTION: Are customised preformed FOs effective in improving gait parameters in children with JIA?

In children with cerebral palsy (CP), rigid ventral shell ankle-foot orthoses (vAFOs) are often prescribed to reduce excessive knee flexion in stance and lower the energy cost of walking (ECW). However, how vAFOs affect ECW is a complex issue, as vAFOs may have an impact on lower limb biomechanics, upper body movements, and balance. Besides, the vAFO's biomechanical effect have been shown to be dependent on its stiffness around the ankle joint. We examined whether vAFO stiffness influences trunk movements and gait stability in CP, and whether there is a relationship between these factors and E

'Freiberg's infraction or disease' is an osteonecrotic process primarily affecting the 2nd & 3rd metatarsal heads. Early diagnosis is difficult, and the underlying pathogenesis remains unclear. Surgical options for late disease have been widely reported in the literature, yet details on conservative management for the early stages of Freiberg's are largely absent. Pathology should be treated where possible using an evidence-based approach, yet evidence for conservative management in acute stages of Freiberg's is lacking. A case study is presented that outlines two different prescription interv

BACKGROUND: One of the hypothesised mechanisms by which foot orthoses obtain their clinical effect is by influencing muscle activity, however previous studies have reported highly variable findings. The aim of this study was to determine whether orthoses change muscle activity in people with flat-arched feet towards a pattern observed in people with normal-arched feet.
METHODS: Thirty young asymptomatic adults with flat-arched feet were recruited. Foot posture was classified using two clinical measurements and four skeletal alignment measurements from weight-bearing foot x-rays. Electromyogra

BACKGROUND: Posterior tibialis tendon dysfunction (PTTD) is a debilitating condition that leads to biomechanical changes, for which foot orthoses are often prescribed to attenuate. There is a need to improve the ability to predict these biomechanical alterations, determine the biomechanical effectiveness of foot orthoses, and anticipate their effects on individuals with PTTD during gait.
RESEARCH QUESTION: Is the supination resistance test (SRT) reliable, and capable of predicting foot and ankle biomechanics, as well as the biomechanical effects of foot orthoses in individuals with PTTD durin

The first study to compare walking ability, balance and perceived daily functioning of two microprocessor stance-and-swing-phase-controlled knee-ankle-foot orthoses (SSCKAFOs) and a stance-control KAFO (SCKAFO). A 67-year-old man with knee instability due to the late effects of polio, currently using a NEURO TRONIC SCKAFO received two newly fabricated microprocessor-controlled SSCKAFOs: the NEURO HiTRONIC and C-Brace. Measurements taken after 10-weeks of using each SSCKAFO were compared to the SCKAFO baseline measurements. Outcomes included walking speed, walking energy cost, stair and hill de

BACKGROUND: Sensorimotor foot orthoses (SMFOs) regulate muscle tone. Within this growing body of evidence, the impact of two SMFO elements, the retrocapital pad and toe bar, on the plantar flexors has not yet been investigated. It has been observed and postulated that combining these two elements may reduce calf muscle activity.
MATERIAL AND METHODS: This randomized crossover study included 29 healthy participants (age, 25.8 ± 6.5 years). Each participant performed 30-seconds walking and running trials on a treadmill to compare the SMFOs and no-foot orthoses (NFOs) conditions. The activities

BACKGROUND: Flatfoot is a structural and functional abnormality of the foot that may cause lower limb mechanical damage during walking. The aim of this study was to investigate the acute effect of foot orthoses on the frequency domain of ground reaction forces in children with flatfeet during the stance phase of walking.
METHODS: Bilateral gait data were collected from fifteen male children suffering from flatfeet syndrome. Two Kistler force platforms were used to record the ground reaction forces of each limb during level walking. Arc support foot orthoses were used as an intervention.

BACKGROUND: Mechanical orthoses are used for standing and walking after neurological injury. Most orthoses such as the advanced reciprocating gait orthosis typically use solid ankle-foot orthoses.
OBJECTIVES: The goal of this study was to test the effects of ankle dorsiflexion assistance in patients with spinal cord injury when ambulating with an advanced reciprocating gait orthosis compared to walking with fixed ankles.
STUDY DESIGN: Quasi-experimental.
METHODS: Four patients with spinal cord injury were fitted with an advanced reciprocating gait orthosis equipped with solid and dorsiflexi

Recovering the ability to stand and walk independently can have numerous health benefits for people with spinal cord injury (SCI). Wearable exoskeletons are being considered as a promising alternative to conventional knee-ankle-foot orthoses (KAFOs) for gait training and assisting functional mobility. However, comparisons between these two types of devices in terms of gait biomechanics and energetics have been limited. Through a randomized, crossover clinical trial, this study compared the use of a knee-powered lower limb exoskeleton (the ABLE Exoskeleton) against passive orthoses, which are t

Patients with severe hemiplegia along with knee instability require knee-ankle-foot orthoses (KAFOs) for gait training. However, in these patients, it is unclear which type of walking training is more effective to improve gait function. Providing alternate gait training (AGT) improves walking function in patients with spinal cord injury, but it is still unclear whether this is effective in hemiplegic stroke patients. In this study, we defined "unified AGT" as AGT performed with the same therapeutic concept by physiotherapists. We then investigated whether AGT improved gait function quicker tha

BACKGROUND: Patellofemoral pain (PFP) is a common cause of knee pain in adolescents, but there are limited evidence-based treatment options for this population. Foot orthoses can improve pain and function in adults with PFP, and may be effective for adolescents. The primary aim of this study is to determine the feasibility of conducting a full-scale randomised controlled trial (RCT) evaluating the effects of contoured foot orthoses on knee pain severity and patient-perceived global change, compared to flat shoe insoles, in adolescents with PFP. The secondary aim is to provide an estimate of tr

Plantar fasciitis is a cause of chronic pain under the heel and bottom of the foot. One of the treatments to reduce pain consists of using plantar orthoses to address specific imbalances during foot placement or gait. The aim of the present study was to determine if reduction of pain with a treatment based on plantar orthoses is related to changes in reflexes and muscle activity of the muscles of the lower limbs. Ten patients (51.0±3.5 years, 76.0±2.38 kg, 171.9±1.8 cm, 3 women and 7 men) with plantar fasciitis for less than 1 year were followed up during nine weeks. Soleus M, H, and V waves r
