Hemorheologic approach in the treatment of diabetic foot ulcers.

Pentoxifylline, a new trisubstituted methylxanthine derivative, is the most well known of a new group of hemorheologic agents. It has been shown to improve hemorheologic abnormalities associated with diabetes and atherosclerosis. The authors examined the role of pentoxifylline in the treatment of diabetic foot ulcers. Forty diabetic patients with foot ulcerations were included in the study, 20 of whom received conventional therapy and 20 received pentoxifylline (400 mg three times a day) in addition. Healing of ulcers after eight weeks of treatment was significantly higher in those on pentoxif

Metatarsal head resection for diabetic foot ulcers.

Twenty-five diabetic patients underwent 34 metatarsal head resections for chronic neuropathic ulceration. All ulcers were located on the plantar surface beneath the metatarsophalangeal joints. The ulcers had been present for a mean of 9.0 +/- 7.8 months before operation, yet they healed in a mean of 2.4 +/- 1.6 months postoperatively. None recurred during the mean follow-up time of 13.8 +/- 11.0 months. Moderate peripheral vascular disease, impaired renal function, and retinopathy did not affect the time required for ulcer healing. There were two complications: one wound infection and one hema

Fluorescein angiography for predicting healing of foot ulcers.

Fluorescein angiography (FA) was performed on 83 patients (68 diabetics) with foot ulcer or gangrene. Densitometric measurements were made on the FA images, and different FA parameters were defined. These parameters, as well as systolic ankle and toe blood pressures, were evaluated for predicting the future course, i.e., whether healing would occur or whether major amputation below or above the knee had to be performed. The toe slope (i.e., the rate of increase of fluorescence on the big toe during the first 10 seconds after its appearance on the toe) predicted healing correctly in 0.83 and ma

A clinico-pathological study of diabetic foot ulcers.

During a 32-month period 94 foot ulcers in 54 diabetic patients aged 38-90 years (mean 64 years) were managed in a specialist foot clinic. Fifty-six percent were men, and they were significantly younger than women; 46% were taking insulin. Mean duration of diabetes was 13.4 years. Comparison with controls revealed a higher prevalence (p less than 0.01) of retinopathy (60% vs 23%), neuropathy (89% vs 31%), vasculopathy (71% vs 34%), arterial calcification (31% vs 20%) and previous lesions (54% vs 4%). There was no difference in quality of diabetic control, or smoking habit. A simple classificat

Impaired vibratory perception and diabetic foot ulceration.

We have studied risk factors for diabetic foot ulceration by comparing diabetic patients who had active foot ulcers (n = 86) with diabetic patients who had no history of foot ulcers (n = 49). Whereas there was a strong association of diabetic foot ulceration with abnormal vibratory perception (Odds Ratio = 10.77; p less than 0.001, which increased with worsening vibratory perception), there was little association with abnormality of the ankle-pressure index (Odds Ratio = 2.84, p = n.s.). Although foot ulceration and limited joint mobility were associated (Odds Ratio = 3.57, p less than 0.001),

Foot and ankle ulcers associated with hematologic disorders.

Ulcers of the foot and ankle secondary to blood dyscrasias may become more common as a result of modern medicine's ability to keep patients alive long enough to become victims of chronic disease. Also, many pharmaceutic agents have, among their side effects, the ability to produce hematologic complications. The physician should consider that hematologic disorders may be among the etiologic agents responsible for foot and ankle ulcers.

Local treatment for neuropathic foot ulceration and osteoarthropathy.

Forty-one patients with 42 involved feet received local care for neuropathic foot disease. Thirty-one patients had ulcers (nine with associated fractures), and 10 did not (eight with fracture and two with severe ligamentous injury). Patients were examined at least 1 year after initiation of treatment, and follow-up averaged 3 years (range, 1 to 9). Treatment included ulcer cleansing, debridement, local bone excision, molded plaster casts, and antibiotics for patients with significant infection. Most patients used special shoes. Of the 42 involved extremities, 39 (93%) healed with treatment, 21

The role of autonomic neuropathy in diabetic foot ulceration.

In an attempt to define the role of autonomic neuropathy in the pathogenesis of diabetic foot ulcers, we have studied diabetic patients with and without ulcers, and a group of non-diabetic controls. A series of standard autonomic function tests have been compared with measurement of the Galvanic Skin Response (GSR). Patients with foot ulcers showed evidence of more severe somatic and autonomic neuropathy than diabetic subjects with no ulcer. The GSR was found to correlate well with the more conventional tests. In addition it proved to be abnormal more often than any other test for autonomic ne

Ulcer-free survival days and ulcer healing in patients with diabetic foot ulcers: A prospective cohort study.

Healing rates may not give a complete indication of the effectiveness and management of diabetic foot ulcers because of high recurrence rates. The most important outcome for patients is remaining ulcer-free; however, this has hardly been investigated. The aim of our study was to prospectively investigate ulcer-free survival days and ulcer healing in patients with diabetic foot ulcers. This was a prospective cohort study of all referrals to our diabetic foot expertise centre from December 2014 to April 2017. Outcomes were determined after a minimum follow-up period of 12 months. Primary outcome

Breaking the Cycle of Recurrent Diabetic Foot Ulceration: A Novel and Sustainable Offloading Modality to Treat Diabetic Foot Ulceration and Prevent Recurrence.

INTRODUCTION: Delayed healing and recurrence of diabetic foot ulcerations (DFUs) is often related to excessive pressure. Offloading, a mainstay of treatment, can be achieved through a variety of methods. Although pressure mitigation should be continued after wound resolution to prevent recurrence, many offloading modalities are discontinued at that point, with providers instead relying on patient self-directed use of appropriate inserts and shoe gear. Use of a novel offloading modality continued upon wound healing may help break the cycle of recurrent DFUs. CASE REPORT: A patient presented wi

Value of cystatin C in predicting recurrence in patients with severe diabetic foot and diabetic foot ulcer.

Objective: Our objective is to investigate the risk factors and predictive ability of severe diabetic foot (DF) and diabetic foot ulcers (DFUs). Patients & methods: The efficacy of cystatin C in predicting the recurrence of DF and DFU was evaluated using a receiver operating characteristic curve. Results: The findings indicate that, in contrast to non-severe patients, severe cases exhibit elevated levels of cystatin C (p < 0.05). Additionally, a statistically significant increase in cystatin C levels was observed in the subgroup of patients with recurrent DFU (p < 0.01). Conclusion: Cystatin C

Association of Incident Diabetic Foot Ulcer With Lower-Extremity Amputation Risk: The Prospective Seattle Diabetic Foot Study.

AIMS: Although etiologic factors for diabetes-related lower-extremity amputation (LEA) have been extensively examined, the role of incident diabetic foot ulcer (DFU) has not been prospectively evaluated. We investigated the independent effect of incident DFU, among other limb- and person-level factors, on LEA risk in a cohort of veterans with diabetes at one Department of Veterans Affairs general internal medicine clinic. METHODS: We prospectively followed 1458 male U.S. veterans with 2893 lower limbs without DFU between 1990 and 2002 (mean follow-up = 4.9 years). Potential risk factors were

Temperature as a Causative Factor in Diabetic Foot Ulcers: A Call to Revisit Ulceration Pathomechanics.

BACKGROUND: Diabetic foot ulcers (DFUs) are a major burden to patients and to the health-care systems of many countries. To prevent or treat ulcers more effectively, predictive biomarkers are needed. We examined temperature as a biomarker and as a causative factor in ulcer development. METHODS: Thirty-seven individuals with diabetes were enrolled in this observational case-control study: nine with diabetic neuropathy and ulcer history (DFU), 14 with diabetic neuropathy (DN), and 14 nonneuropathic control participants (DC). Resting barefoot plantar temperatures were recorded using an infrared

Pattern and texton-based feature descriptors for Diabetic foot ulcer detection.

Diabetic foot ulcers (DFU) are a dangerous side effect of diabetes mellitus that significantly impairs a patient's health and standard of living. For efficient treatment and to avoid serious outcomes like infections and amputations, DFUs must be detected promptly and precisely. This paper introduces a novel Deep Learning Strategy-based framework for Diabetic Foot Ulcer Detection (DLS-DFUD). The DLS-DFUD framework involves four key steps: preprocessing, segmentation, extraction of features, and classification. The detection process starts with acquiring the input image, which is then preprocess

Predictive modeling of chronic foot ulcer outcomes using longitudinal photoacoustic imaging.

This study reports the first clinical longitudinal photoacoustic imaging (PAI) of chronic foot ulcers, a major complication in patients with peripheral vascular disorders. Compared to traditional methods such as ABI or near-infrared spectroscopy, the photoacoustic imaging approach provides non-invasive, high-resolution, and quantitative monitoring of vascular dynamics over time. Our system provided dorsal-side imaging of vascular structures with an expanded field of view and incorporated a skin artifact suppression algorithm to improve visualization of subdermal vasculature. From the acquired

SIRT6 Overexpression Enhances Diabetic Foot Ulcer Healing via Nrf2 Pathway Activation.

Sirtuin-6 (SIRT6) has a pivotal role in a wide array of cellular biological functions and is linked to the progression of various diseases. Previous findings have identified SIRT6 as a protective modulator against numerous diabetic complications. However, whether SIRT6 exerts a protective role in diabetic foot ulcer (DFU) remains unstudied. This work established a rat model of DFU and evaluated the possible role of SIRT6 in mediating the wound healing in DFU. Marked reductions in SIRT6 levels were observed in wound samples from DFU patients and rats. Increasing SIRT6 expression in wound tissue

Disrupted Alternative Splicing of RAB11FIP3 Contributes to Diabetic Foot Ulcer Dysfunction.

Dysregulation of alternative splicing (AS) has been associated with various complications of diabetes, yet its role in the pathogenesis of diabetic foot ulcers (DFUs) and its involvement in metabolic memory (MM) remain unclear. In this study, we identified specific AS events in RAB11FIP3, notably the full-length (RAB11FIP3-FL) isoform and exon 6 exclusion (RAB11FIP3-Δ6). We found that RAB11FIP3 (FL/Δ6) ratio is significantly elevated in patients with MM and DFUs. Additionally, we demonstrated that knockdown of RAB11FIP3-FL alleviates vascular endothelial damage associated with MM and enhances

Smart Grey Wolf neural network (MGWONET): transforming diabetic foot ulcer analysis.

OBJECTIVE: Diabetes frequently results in diabetic foot ulcers (DFUs), which can lead to lower limb amputation if left untreated. Current DFU management consists of a multidisciplinary team approach, including physicians, podiatrists, wound care specialists, nursing staff and patients. Traditional diagnoses of DFUs can be expensive, lengthy, and generally reliant on local and private clinical evaluation. There is a need for an automated, remote, diagnostic option for patients with suspected DFUs. METHOD: This paper introduces MGWONET, a new model for deep learning (DL) on classification probl
