Screening swabs surpass traditional risk factors as predictors of MRSA bacteremia.

BACKGROUND: Consideration to add empiric MRSA therapy with vancomycin is a common clinical dilemma. However, vancomycin overuse has important adverse events. MRSA colonization screening is commonly performed for infection control. We hypothesized that in cases of S. aureus bacteremia, a score based on patient level factors and MRSA colonization could predict the risk of MRSA infection and inform the need for empiric coverage. METHODS: Using modern machine learning statistical methods (LASSO regression and random forests), we designed a predictive score for MRSA infection based on patient leve

Transcriptomic profiling of blood from autoimmune hepatitis patients reveals potential mechanisms with implications for management.

Autoimmune hepatitis (AIH) is a poorly understood, chronic disease, for which corticosteroids are still the mainstay of therapy and most patients undergo liver biopsy to obtain a diagnosis. We aimed to determine if there was a transcriptomic signature of AIH in the peripheral blood and investigate underlying biologic pathways revealed by gene expression analysis. Whole blood RNA from 75 AIH patients and 25 healthy volunteers was extracted and sequenced. Differential gene expression analysis revealed 249 genes that were significantly differentially expressed in AIH patients compared to controls

Gentian violet for pyoderma gangrenosum: a retrospective chart review.

Pyoderma gangrenosum is a rare autoinflammatory skin disease. Treatment is multifactorial, addressing inflammation, pain, underlying disease, if present, and the wound. Gentian violet has been used for hundreds of years in a variety of dermatologic conditions for its anti-inflammatory properties. This study aims to evaluate gentian violet in wound healing for pyoderma gangrenosum. We conducted a retrospective chart review of patients with pyoderma gangrenosum treated with gentian violet at the Wake Forest School of Medicine Department of Dermatology in the last 10 years. The primary outcome wa

Toward FOXM1 inhibitors based on supervised machine learning classification models and similarity-based screening.

Ovarian cancer remains among the deadliest gynaecological malignancies, with aberrant activation of the Forkhead Box M1 (FOXM1) transcription factor strongly implicated in tumour progression, therapy resistance, and poor prognosis. Given the limited availability of validated FOXM1 inhibitors and the lack of prior dedicated machine learning-based QSAR studies identified in our literature survey, this study employed a supervised machine learning framework to predict the bioactivity of structurally diverse small-molecule inhibitors. A curated dataset of 101 compounds with reported IC50 values was

Machine learning framework for prediction of drug-induced liver injury in patients on antitubercular therapy: Multicentric study.

AIMS: Antitubercular drug-induced liver injury (AT-DILI) is the leading cause of drug-induced liver injury in India, yet integrated risk stratification models remain inadequately defined. This study aimed to identify baseline determinants of AT-DILI and to develop machine learning models for its risk prediction at two Indian tertiary centres. METHODS: In this multicentre prospective cohort study conducted from August 2021 to January 2025, 695 treatment-naïve adults with newly diagnosed tuberculosis were followed during first-line therapy. AT-DILI was diagnosed using modified American Thoracic

A machine learning-based predictive model for 48-week hepatitis B surface antigen seroclearance in chronic hepatitis B patients treated with pegylated interferon α-2b: prediction at week 24.

BACKGROUND: Chronic hepatitis B (CHB) is an infectious disease mainly affecting the liver, caused by the hepatitis B virus (HBV). In the treatment of CHB, pegylated interferon α-2b (PEG-IFNα-2b) is one of the important therapeutic options. However, there are significant individual differences in patients' responses to this treatment and only a few patients can achieve hepatitis B surface antigen (HBsAg) seroclearance. Therefore, an effective method to identify patients with a high likelihood of favorable response at an early stage is urgently needed. METHODS: In this study, we analyzed data f

An artificial intelligence-based approach to identify volume status in patients with severe dengue using wearable PPG data.

Dengue shock syndrome (DSS) is a serious complication of dengue infection which occurs when critical plasma leakage results in haemodynamic shock. Treatment is challenging as fluid therapy must balance the risk of hypoperfusion with volume overload. In this study, we investigate the potential utility of wearable photoplethysmography (PPG) to determine volume status in DSS. In this prospective observational study, we enrolled 250 adults and children with a clinical diagnosis of dengue admitted to the Hospital for Tropical Diseases, Ho Chi Minh City. PPG monitoring using a wearable device was ap

Predicting criminal offence in adolescents who exhibit antisocial behaviour: a machine learning study using data from a large randomised controlled trial of multisystemic therapy.

INTRODUCTION: Accurate prediction of short-term offending in young people exhibiting antisocial behaviour could support targeted interventions. Here we develop a set of machine learning (ML) models that predict offending status with good accuracy; furthermore, we show interpretable ML analyses can complement models to inform clinical decision-making. METHODS: This study included 679 individuals aged 11-17 years who displayed moderate-to-severe antisocial behaviour, from a controlled trial of Multisystemic-therapy in England. The outcome was any criminal offence in the 18 months after study ba

Developing a machine learning model for bleeding prediction in patients with cancer-associated thrombosis receiving anticoagulation therapy.

BACKGROUND: Only 1 conventional score is available for assessing bleeding risk in patients with cancer-associated thrombosis (CAT): the CAT-BLEED score. OBJECTIVES: Our aim was to develop a machine learning-based risk assessment model for predicting bleeding in CAT and to evaluate its predictive performance in comparison to that of the CAT-BLEED score. METHODS: We collected 488 attributes (clinical data, biochemistry, and International Classification of Diseases, 10th Revision, diagnosis) in 1080 unique patients with CAT. We compared CAT-BLEED score, Ridge and Lasso logistic regression, rand

Prediction of visual function from automatically quantified optical coherence tomography biomarkers in patients with geographic atrophy using machine learning.

Geographic atrophy (GA) is a vision-threatening manifestation of age-related macular degeneration (AMD), one of the leading causes of blindness globally. Objective, rapid, reliable, and scalable quantification of GA from optical coherence tomography (OCT) retinal scans is necessary for disease monitoring, prognostic research, and clinical endpoints for therapy development. Such automatically quantified biomarkers on OCT are likely to further elucidate structure-function correlation in GA and thus the pathophysiological mechanisms of disease development and progression. In this work, we aimed t

Using Intraoperative Microelectrode Recordings to Predict Improvement in Bradykinesia for Parkinson's Disease Patients.

Predicting the effectiveness of deep brain stimulation (DBS) for Parkinson's disease (PD) remains challenging due to the variability of individual responses. Local field potentials (LFPs) derived from intraoperative microelectrode recordings (MERs) within the subthalamic nucleus (STN) during DBS surgery reflect the neural activity associated with motor function in patients with PD. These signals can be utilized to assess the potential improvement index in response to therapeutic interventions. This study investigates the use of machine learning to predict DBS treatment outcomes prior to therap

Machine learning-based predictive model for high- grade cytokine release syndrome in chimeric antigen receptor T-cell therapy.

INTRODUCTION: The development of robust predictive models for high-grade cytokine release syndrome (CRS) in CAR-T recipients remains limited by sparse clinical trial data. METHODS: We analyzed of 496 COVID-19 patients revealed that CRS plays a pivotal role in disease progression and serves as a valuable data source for understanding CRS progression. Building on this insight, we evaluated and compared the predictive performance of three machine learning models, with the ultimate goal of developing a predictive model for high-grade CRS in patients receiving CAR-T therapy. RESULTS: Among evalua

Machine learning analyses constructed a novel model to predict recurrent thrombosis in adults with essential thrombocythemia.

The current study involving 318 essential thrombocythemia (ET) patients with prior thrombosis was designed to identify risk factors that were predictive of recurrent thrombosis. The whole cohort was randomly split into derivation and validation cohorts. The random forest method, support vector machine with built-in recursive feature elimination model, and logistic multivariable analysis were performed in the derivation cohort, and cardiovascular risk factor (CVF) and RBC distribution width with standard deviation (RDW-SD) were finally selected as independent predictors. Subsequently we devise

Improving pancreatic adenocarcinoma prognosis models through ligand receptor interactions and histopathological integration.

BACKGROUND: Pancreatic adenocarcinoma (PAAD) has an extremely poor prognosis, and existing prognostic markers fail to fully capture the complex heterogeneity of the tumor microenvironment. This study aimed to integrate ligand-receptor (L-R) interactions, multi-omics data, and deep learning-based pathological images to construct an interpretable multimodal prognostic model and to elucidate the mechanisms underlying the cancer-associated fibroblast (CAF) microenvironment. METHODS: Significant L-R interactions were identified using BulkSignalR, followed by sequential Cox, least absolute shrinkag

Distinct gut virome profiles are associated with response to anti-PD-1 therapy in non-small cell lung cancer.

BACKGROUND: The gut microbiota is a key modulator of immune checkpoint inhibitor (ICI) efficacy, yet the contribution of the gut virome remains poorly defined, particularly in advanced non–small cell lung cancer (NSCLC). Here, we characterized the gut virome and explored its potential role in shaping response to PD-1 blockade. METHODS: We performed metagenomic virome profiling of fecal samples from 338 advanced NSCLC patients treated with PD-1 inhibitors and evaluated model generalizability in an independent external cohort (n = 30). Viral diversity, taxonomic composition, and functional poten

Timing matters: a machine learning-driven comparison of community and hospital-onset sepsis.

BACKGROUND: Sepsis is a leading cause of morbidity and mortality in hospitalized patients. While most sepsis research has focused on community-onset sepsis (COS), hospital-onset sepsis (HOS) is increasingly recognized as a distinct clinical entity with worse outcomes. However, a few studies have systematically compared the characteristics and outcomes of COS and HOS. Understand the differences between COS and HOS from a clinical perspective and study hospital outcomes in sepsis patients using a machine learning driven approach. METHODS: We conducted a retrospective analysis of sepsis events at

Confronting the "lethal duo" in the ICU: early identification of Aspergillus-Mucorales co-infection using a clinical-immuno-inflammatory signature.

BACKGROUND: Co-infection with Aspergillus and Mucorales in the intensive care unit (ICU) represents a devastating syndrome with high mortality that is frequently clinically occult. Clinically distinguishing this co-infection from invasive pulmonary aspergillosis (IPA) is challenging but critical for tailoring precise antifungal strategies. METHODS: We conducted a single-center, retrospective observational study involving 93 critically ill patients (75 with Aspergillus infection and 18 with co-infection) admitted between 2017 and 2025. We compared clinical characteristics, inflammatory markers

Development of a heart failure with preserved ejection fraction risk prediction score in primary care settings with a high HIV prevalence.

BACKGROUND: Antiretroviral therapy (ART)-treated people with HIV (PWH) have increased risk of heart failure with preserved ejection fraction (HFpEF). The diagnosis of HFpEF relies on cardiac imaging and expertise and is challenging in primary care settings. Further, existing risk prediction tools do not account for HIV-related risk. We developed a practical HFpEF risk prediction tool using data from the Prevalence and Phenotype of HIV-HFpEF in South Africa, a cross-sectional observational study that enrolled 1508 adults aged ≥40 years (1008 PWH on ART ≥1 year and 500 people without HIV (PWoH))
