Internal fixation of acute scaphoid fractures: a new approach to treatment.

Thirty-five patients with acute, unstable fractures of the scaphoid were treated by primary internal fixation using the Herbert Bone Screw System. The use of plaster was avoided whenever possible, and operation was carried out as soon as practicable after injury. The average time for return to work after operation was 3.7 weeks. Only one fracture failed to unite, and this was due to avascular necrosis of the proximal fragment of the scaphoid. Internal fixation of the acute scaphoid fracture successfully avoids the morbidity associated with standard methods of treatment in plaster.

Stability of acetabular fractures after internal fixation. A cadaveric study.

The rigidity of a transverse acetabular fracture in a laboratory cadaveric model immobilized by internal fixation was studied. On the application of a longitudinal load imposed on the fifth lumbar vertebra, four modes of displacement of the acetabular fracture were monitored by the use of variable impedance transducers. The rigidity of various combinations of lag screws and plates was assessed. The fracture deformations documented were generally quite small at any observation point with any of the fixation methods, and in most cases they were recoverable on load release. Anterior column lag sc

Malleolar fractures treated with rigid internal fixation and immediate mobilization.

Twenty=eight consecutive patients with displaced malleolar fractures were treated with anatomically exact open reduction, rigid internal fixation and immediate postoperative exercises, following the principles of the Swiss ASIF group. There were no operative or postoperative complications. One patient had a minor abscess over a loosened screw, while 1 patient developed pes equinus that required treatment. AT follow-up 2 to 12 months after the operation, 25 patients were walking normally. The movements of the ankle joint were excellent or good in 25 patients. The method is recommended.

Internal fixation of intertrochanteric hip fractures: a clinical and biomechanical study.

One hundred seventy-three cases of intertrochanteric fractures treated by internal fixation are reviewed: 72 treated with the Jewett nail and 101 with the Richards compression hip screw. Both devices maintained adequate reduction in the majority of cases. Treatment failure: loss of fixation, symptomatic joint penetration, aseptic necrosis, malunion and nonunion occurred in 25% of the Jewett nail cases and 6% of the Richards screw cases. In vitro biomechanical studies demonstrate that the compression screw is subjected to less bending stress by acting as a lateral "tension band" in stable reduc

[Osteosynthesis with compression as an alternative method in the internal fixation of malleolar fractures].

The authors discussed a group of patients with malleolar fractures operated on between 1974 and 1977. Compressive osteosynthesis was used as an alternative method of treatment. They compared their group of patients with a much larger group of patients that were surgically treated for malleolar fractures in the traumatology clinic in Ljublana between 1974 and 1977. This second group was divided according to the type of fracture, the type of surgical treatment, the time of treatment, and early and late complications. The authors also discussed the specifics of early postoperative functional trea

Suture Augmentation of Internal Fixation for Highly Comminuted Patella Fractures.

Treatment of comminuted patella fractures is challenging. Augmentation of internal fixation has been proposed to improve stability. The authors describe an effective technique to augment internal fixation consisting of locked sutures within the quadriceps and patellar tendons. This technique functions to off-load internal fixation and restore tension to the extensor mechanism. The locked suture augmentation technique was performed for 13 patients undergoing open treatment of patella fractures by the senior author from 2016 to 2019. Patients were followed for an average of 10.2 months. No patie

Posterior Approach for Open Reduction and Internal Fixation for Scapular Fractures.

BACKGROUND: This technique utilizes a full-thickness flap to provide a posterior approach to the scapula for open reduction and internal fracture fixation. The present video article outlines the Judet approach along with an incision modification tip for the surgeon's consideration. DESCRIPTION: Prior to making the incision, perform preoperative planning, patient and C-arm positioning, and identification of the primary fragments of the fracture that necessitate fixation on imaging. The Judet incision is made, and the full-thickness flap is retracted laterally (also described as a "boomerang-sh

Arthroscopic Internal Fixation of Coracoid Fractures: Surgical Technique Guide.

Fractures of the coracoid process are uncommon injuries and are usually the result of high-energy trauma or avulsion-type injuries. Typically coracoid fractures treated with nonoperative management have yielded good results. Operative treatment of coracoid fractures is reserved for a subset of clinical situations, including fracture nonunion. We detail our technique for arthroscopic debridement of a Type II coracoid fracture nonunion, as well as the use of arthroscopic-assisted percutaneous fixation for a Type II coracoid fracture.

Internal fixation implants for intracapsular hip fractures in older adults.

BACKGROUND: Hip fractures are a major healthcare problem, presenting a huge challenge and burden to patients, healthcare systems and society. The increased proportion of older adults in the world population means that the absolute number of hip fractures is rising rapidly across the globe. The majority of hip fractures are treated surgically. This review evaluates evidence for types of internal fixation implants used in joint-preserving surgery for intracapsular hip fractures. OBJECTIVES: To determine the relative effects (benefits and harms) of different implants for the internal fixation of

Arthroscopic Assisted Reduction and Internal Fixation of Tibial Plateau Fractures.

BACKGROUND: Tibial plateau fractures present an important entity in orthopaedic fractures. Arthroscopic-assisted reduction and internal fixation is a good alternative to ORIF as it has the advantage of direct visualisation of the articular surface of the plateau, direct assessment of the reduction of the articular surface, and managing any associated intra-articular pathology. AIM: Our study aim is to determine the results of arthroscopic assisted reduction and internal fixation of tibial plateau fractures. METHODS: This study involved 25 patients with tibial plateau fractures presenting to

Complications of open reduction and internal fixation of distal humerus fractures.

Treatment of distal humerus fractures is demanding. Surgery is the optimal treatment and preoperative planning is based on fracture type and degree of comminution.Fixation with two precontoured anatomical locking plates at 90o:90o orthogonal or 180o parallel is the optimal treatment. The main goal of surgical treatment is to obtain stable fixation to allow immediate postoperative elbow mobilization and prevent joint stiffness.Despite evolution of plates and surgical techniques, complications such as mechanical failure, ulnar neuropathy, stiffness, heterotopic ossification, nonunion, malunion, i

Treatment of Periprosthetic Humerus Fractures With Open Reduction and Internal Fixation.

Periprosthetic humerus fractures are relatively uncommon occurrence that can be difficult to manage nonoperatively. Locking plate technology has enhanced the surgical management of these fractures. However, adequate fixation around the stem remains of some concern. We describe an osteosynthesis technique using a locking plate utilizing eccentrically placed screw holes to place "skive screws" in the proximal end of the plate to achieve fixation around the stem of the implant. A clinical series of 5 consecutive patients treated with this technique is presented with an average follow-up of 29 mon

Pitfalls associated with open reduction and internal fixation of fractured ribs.

Rib fracture is exceedingly common and remains a leading cause of death in patients with chest injury. Probability of death increases by 19% with each broken rib, and the probability of death increases further with age. Treatment is centered on pain control and early mobilization to provide adequate pulmonary hygiene. Multimodality interventions, such as incentive spirometry, postural changes, and coughing, are pivotal in minimizing the risk of pneumonia and death. Recently, many studies have found mortality benefit to operation fixation (ORIF) of ribs in select patients. However, this procedu

[4-point internal fixator stabilization of a sacral insufficiency fracture].

Sacral insufficiency fractures develop due to a discrepancy between physiological load and load-bearing capacity. Besides osteoporosis as the main predisposing factor, other diseases lead to a loss of the bony elastic resistivity and therefore are able to cause these characteristic bilateral sacral stress fractures. Most patients complain of low back pain and show difficulties in mobilization. The non-operative therapy is based on analgesics and pain-adapted mobilization. Due to the noticeable functional deficit and persistent discomfort, surgery is necessary in many cases. Various operative me

[Internal fixation of radial shaft fractures: Anatomical and biomechanical principles].

Radius is a critical bone for functioning of the forearm and therefore its reconstruction following fracture of its shaft must be anatomical in all planes and along all axes. The method of choice is plate fixation. However, it is still associated with a number of unnecessary complications that were not resolved even by introduction of locking plates, but rather the opposite. All the more it is surprising that discussions about anatomical and biomechanical principles of plate fixation have been reduced to minimum or even neglected in the current literature. This applies primarily to the choice

[Postoperative course following open reduction and internal fixation of fractures].

The postoperative management after open reduction and internal fixation (ORIF) of fractures is addressed with this article. The topics selected focus on topics that are of relevance for the family practitioner. Because of large differences in the treatment of adult - and geriatric fracture patients these two groups are separately looked at in Part 1 and Part 2 of the article. Fractures in children or fractures of the growing skeleton are not addressed. In the early postoperative phase, the fracture patient might seek for advice in case of complications, such as wound infection or complex regio
