Comparative analysis of adolescent intramedullary nailing and locking plate fixation for femoral shaft fractures.

BACKGROUND: Femoral shaft fractures in adolescents are commonly treated with either antegrade intramedullary nailing (AIN) or lateral plate (LP) fixation. This study compared the clinical outcomes of these two methods. METHODS: Adolescent patients with femoral shaft fractures were randomly assigned to either the AIN group (n = 62) or the LP group (n = 62). Surgical parameters, early recovery metrics, levels of inflammatory cytokines, pain mediators, and bone metabolism markers were assessed. RESULTS: The AIN group had significantly shorter incision lengths (6.8 vs. 8.5 cm, p < 0.001) and low

Articular Surface Damage Following Headless Intramedullary Nail Fixation of Proximal Phalanx Fractures.

PURPOSE: Offering the benefits of rigid fixation while minimizing soft tissue dissection, intramedullary implants have become a popular choice among hand surgeons. Their placement often requires traversing or passing in proximity to joint surfaces. This study aimed to assess the damage to the articular cartilage of the base of the proximal phalanx resulting from antegrade placement of threaded headless intramedullary nails. METHODS: A cadaveric study comparing two techniques for antegrade placement of threaded headless intramedullary nails was conducted in 56 digits. The first entailed a sing

Biomechanical Comparison of Fixation of Metacarpal Shaft Fractures With Intramedullary Threaded Nail and Dorsal Plate.

PURPOSE: The purpose of this study was to investigate the mechanical properties of metacarpal long oblique and transverse shaft fractures stabilized by threaded intramedullary nails and dorsal plates and screws. METHODS: Transverse and oblique midshaft osteotomies were created in 28 paired left and right metacarpal bones from four fresh frozen cadavers. Each matched pair was fixed with one 4.5-mm threaded intramedullary nail and one 2.0-mm dorsal stainless-steel plate and a screw construct. The bones were secured at the proximal end, by a 3-D-printed customized jig, to a tensile testing machi

A comparison of intramedullary cannulated screws versus miniplates for fixation of unstable metacarpal diaphyseal fractures.

We carried out a prospective randomized study to compare the clinical and radiological results of metacarpal diaphyseal fractures treated with retrograde intramedullary headless cannulated screws (IHCS) and plates. Fractures were fixed with IHCS in 34 patients (37 metacarpals) and locked miniplates in 35 patients (40 metacarpals). The mean age was 33 years (range 18-61) in the IHCS group and 32 years (range 17-68) in plate group. All patients were followed up for 1 year. All fractures in the IHCS group united but there was one nonunion in the plate group. At final follow-up, there was no signi

Intramedullary nailing versus external fixation for open tibia fractures in Tanzania: a cost analysis.

OBJECTIVES: Open tibia fractures pose a clinical and economic burden that is disproportionately borne by low-income countries. A randomized trial conducted by our group showed no difference in infection and nonunion comparing 2 treatments: external fixation (EF) and intramedullary nailing (IMN). Secondary outcomes favored IMN. In the absence of clear clinical superiority, we sought to compare costs between EF and IMN. DESIGN: Secondary cost analysis. SETTING: Single institution in Tanzania. PATIENTS/PARTICIPANTS: Adult patients with acute diaphyseal open tibia fractures who participated in

Outcome of intramedullary nail for fixation of osteoporotic femoral shaft fractures in the elderly above 60.

INTRODUCTION: Intramedullary nailing is an acceptable treatment option for femoral shaft fracture in young patients but not extensively studied in the elderly with osteoporotic fractures. Plate fixation for osteoporotic femoral shaft fractures have a high rate of complications and delayed healing time, and the most acceptable treatment is intramedullary nailing. This study evaluated the healing time and incidence of complications in osteoporotic femoral shaft fractures after intramedullary nailing. PATIENTS &METHODS: This was a retrospective study that included 16 patients above 60 years old

Infections after intramedullary fixation of trochanteric fractures are uncommon and implant removal is not usually needed.

BACKGROUND AND PURPOSE: Infections after intramedullary fixation of trochanteric femoral fractures are rare, but potentially life-threatening complications. There are limited data available to support decision making in these cases. PATIENTS AND METHODS: A retrospective study of 995 consecutive operatively treated trochanteric fractures was made to find out different risk factors for infection and to describe the results of treatment. RESULTS: 28 patients developed a surgical site infection (2.8%) after intramedullary fixation of trochanteric fracture. 15 patients (1.5%) had a deep and 13 pa

Rate of Delayed Union With Early Weightbearing Following Intramedullary Screw Fixation of Jones Fractures.

BACKGROUND: Jones fractures of the proximal fifth metatarsal are predisposed to delayed union and nonunion due to a tenuous blood supply. Solid intramedullary (IM) screw fixation is recommended to improve healing, traditionally followed by delayed weightbearing (DWB). However, early weightbearing (EWB) postoperatively may facilitate functional recovery. The purpose of this study was to compare union rates and time to union after solid IM screw fixation of Jones fractures in patients treated with an EWB protocol to those treated with a DWB protocol, as well as to identify any factors that may b

Dual Antegrade Intramedullary Headless Screw Fixation for Treatment of Unstable Proximal Phalanx Fractures.

Background: Although intramedullary headless screw (IMHS) fixation is a promising minimally invasive surgical treatment option for unstable proximal phalanx fractures, a single IMHS may provide inadequate fixation for certain fracture patterns. The purpose of this study was to evaluate the short-term clinical outcomes in a pilot series of patients with proximal phalanx fractures treated with dual antegrade IMHS fixation. Methods: We performed a retrospective chart review of proximal phalanx fractures treated with dual antegrade IMHS fixation with a minimum 1 year of follow-up. Demographic info

Systematic review of outcomes following fixed angle intramedullary fixation of distal radius fractures.

PURPOSE: There remains little consensus regarding the optimal management of distal radius fractures. Fixed angle volar devices have gained recent popularity, but have also been associated with soft tissue complications. Intramedullary (IM) devices offer fixed angle stabilisation with minimally invasive surgical technique and low, IM profile. No formal review of outcomes could be identified. METHODS: We conducted a systematic review of clinical studies regarding the use of fixed angle IM devices in acute extra-articular or simple intra-articular distal radius fractures.
