Expression of purinergic receptor P2Y, G-protein coupled 2, is regulated by estrogen during the estrous cycle in mouse uterus.

Elucidating the molecular intricacies governing uterine function is crucial for reproductive success. The purinergic receptor P2Y, G-protein coupled 2 (P2ry2), regulates critical cellular processes, and we investigated the mechanisms by which estrogen regulates P2ry2 expression in the mouse uterus using in vivo models. Uterine P2ry2 expression was analyzed across the estrous cycle in female mice using gene and protein expression analyses. To confirm hormonal regulation, ovariectomized mice were treated with β-estradiol and/or an estrogen receptor antagonist ICI 182,780. Our findings revealed a

Palbociclib in combination with endocrine therapy versus capecitabine in hormonal receptor-positive, human epidermal growth factor 2-negative, aromatase inhibitor-resistant metastatic breast cancer: a phase III randomised controlled trial-PEARL.

BACKGROUND: Palbociclib plus endocrine therapy (ET) is the standard treatment of hormone receptor-positive and human epidermal growth factor receptor 2-negative, metastatic breast cancer (MBC). However, its efficacy has not been compared with that of chemotherapy in a phase III trial. PATIENTS AND METHODS: PEARL is a multicentre, phase III randomised study in which patients with aromatase inhibitor (AI)-resistant MBC were included in two consecutive cohorts. In cohort 1, patients were randomised 1 : 1 to palbociclib plus exemestane or capecitabine. On discovering new evidence about estrogen r

Prevalence of post-partum thyroiditis in Liguria (Italy): an observational study.

BACKGROUND: Cyclin-dependent kinase (CDK) 4/6 inhibitors (ribociclib, abemaciclib, and palbociclib) are standard first-line therapy for hormone receptor (HR)-positive, human epidermal growth factor receptor 2 (HER2)-negative metastatic breast cancer. While diarrhoea is a known adverse effect, severe gastrointestinal toxicity, particularly colitis, is rare and its histopathologic features are not well-characterized. CASE REPORT: We report the case of a 65-year-old woman with HR-positive, HER2-negative metastatic breast cancer who developed severe diarrhoea and significant weight loss during pa

Novel Estrogen Receptor - Targeted Therapies in Hormone-Receptor Positive Breast Cancer.

Endocrine therapy is the backbone of treatment for HR + /HER2- MBC. The introduction of novel endocrine-based therapies has changed the landscape of metastatic breast cancer care, with even more promising agents on the horizon. Given the consistent success in prolonging PFS and OS, CDK4/6 inhibitors should be used as first-line treatment. Once secondary resistance eventually develops after use of a CDK4/6 inhibitor, use of monotherapy with either AI or fulvestrant has shown poor outcome. For example, in the control group of the EMERALD trial, in which all the patients were required to have pre

The effects of the estrogen receptor blocker, Faslodex (ICI 182,780), on estrogen-accelerated bone maturation in mice.

Sex steroids accelerate bone maturation, but it is believed that estrogen action is needed for terminal epiphyseal fusion. In this study, we investigated the effects of a new estrogen-blocking agent, Faslodex (ICI 182,780), on estrogen-accelerated skeletal maturation in immature mice. On day-of-life 2 through 8, mice pups received either estradiol (5 microg/100 g body weight), Faslodex (100 microg/100 g body weight), a combination of Faslodex + estradiol, or vehicle alone. Skeletal maturation was assessed with a scoring system based on the size and appearance of epiphyseal plates in the forepa

Gedatolisib: First Approval.

Gedatolisib (REVTORPYK®) is a kinase inhibitor of class I phosphatidylinositol 3-kinase (PI3K) isoforms and mechanistic target of rapamycin (mTOR) complexes. It is being developed by Celcuity, under a license from Pfizer, for the treatment of various cancers. PIK3CA, which encodes the catalytic subunit of PI3K, is frequently mutated in a range of cancers, including breast cancer. Gedatolisib received its first approval in the USA on 14 July 2026 for use in combination with fulvestrant, with or without palbociclib, for the treatment of adult patients with hormone receptor positive (HR+), human

Circulating tumour DNA analysis to direct therapy in advanced breast cancer (plasmaMATCH): a multicentre, multicohort, phase 2a, platform trial.

BACKGROUND: Circulating tumour DNA (ctDNA) testing might provide a current assessment of the genomic profile of advanced cancer, without the need to repeat tumour biopsy. We aimed to assess the accuracy of ctDNA testing in advanced breast cancer and the ability of ctDNA testing to select patients for mutation-directed therapy. METHODS: We did an open-label, multicohort, phase 2a, platform trial of ctDNA testing in 18 UK hospitals. Participants were women (aged ≥18 years) with histologically confirmed advanced breast cancer and an Eastern Cooperative Oncology Group performance status 0-2. Pati

Preclinical Rationale, Clinical Efficacy, and Safety of the Selective AKT Kinase Inhibitor Capivasertib in Metastatic Hormone Receptor-Positive, Human Epidermal Growth Factor Receptor 2-Negative Breast Carcinoma: A Practical Narrative Review.

Most patients with advanced/metastatic hormone receptor-positive, HER2-negative breast cancer receive first-line therapy with cycline-dependent kinase 4/6 inhibitors plus endocrine therapy. Almost universally, these patients eventually progress due to the emergence of resistant cancer clones. Targeting the PIK3CA/AKT1/PTEN pathway is a way of overcoming resistance. Recently, the oral, selective AKT kinase inhibitor capivasertib has been approved for the treatment of estrogen receptor-positive/human HER2-growth factor receptor-2 advanced BC with alterations in PIK3CA/AKT1/PTEN, in combination w

Selective Estrogen Receptor Degraders Induce Bradycardia by Modulating Nuclear Estrogen Signaling.

Selective estrogen receptor degraders (SERDs) are emerging therapies for estrogen receptor-positive (ER+) breast cancer. However, certain oral SERDs, including giredestrant and camizestrant, have been associated with bradycardia in clinical trials. To elucidate the underlying molecular mechanism, we used chemical biology and genetic approaches in a zebrafish model of SERD-induced bradycardia. Giredestrant and camizestrant induced significant bradycardia in wild-type zebrafish embryos, whereas fulvestrant and amcenestrant (SERDs that do not induce bradycardia in patients) did not alter heart ra

Vepdegestrant (ARV-471, Veppanu) represents an important milestone in targeted protein degradation (TPD) as the first FDA-approved heterobifunctional proteolysis-targeting chimera (PROTAC). Designed to selectively degrade estrogen receptor α (ERα) through recruitment of the cereblon E3 ubiquitin ligase, vepdegestrant provides an event-driven approach to targeting ERα, including clinically relevant estrogen receptor-1 (ESR1)-mutated forms associated with endocrine therapy resistance. Its oral bioavailability and pharmacological properties enabled clinical development in patients with ER-positiv

Cyclin-Dependent Kinase 4 and 6 Inhibitors: A Quantum Leap in the Treatment of Advanced Breast Cancers.

Breast cancer (BC) is defined as an uncontrolled growth of breast cells that affected 2.3 million women in 2020 alone. Until a few years earlier, radiotherapy and chemotherapy were the most commonly used treatments in treating BC; however, many trials and studies were conducted to test the competence of cyclin-dependent kinases 4/6 (CDK4/6) in arresting the cell cycle, and it was found that they were highly influential in halting the disease from progressing. Palbociclib, ribociclib, and abemaciclib are the three drugs that have been approved by the US Food and Drug Administration (FDA) and ar

The Evolving Role of the Estrogen Receptor Mutations in Endocrine Therapy-Resistant Breast Cancer.

Recurrent ligand-binding domain ESR1 mutations have recently been detected in a substantial number of patients with metastatic ER+ breast cancer and evolve under the selective pressure of endocrine treatments. In this review, we evaluate the current understanding of the biological and clinical significance of these mutations. The preclinical studies revealed that these mutations lead to constitutive ligand-independent activity, indicating resistance to aromatase inhibitors and decreased sensitivity to tamoxifen and fulvestrant. Retrospective analyses of ESR1 mutations in baseline plasma circul

Case report: 18F-FES PET/CT predicted treatment responses of second-line and third-line CDK4/6 inhibitors after disease progression on first-line CDK4/6 inhibitor in a HR+/HER2- metastatic breast cancer patient.

BACKGROUND: Cyclin-dependent kinase 4/6 inhibitor (CDK4/6i) has become the commonest first-line treatment of hormonal receptor positive and human epidermal growth factor receptor 2 negative (HR+/HER2-) metastatic breast cancer (MBC). However, therapy is quite individualized after progression of disease (PD) when CDK4/6i fails. Estrogen receptor (ER) status of metastatic lesions of bone, lung or liver might be different from the primary tumor and biopsy of metastatic lesions was invasive and not always available. Prediction of treatment response after PD of CDK4/6i remains unsolved. 18F-fluoroe

Therapeutic strategies using the aromatase inhibitor letrozole and tamoxifen in a breast cancer model.

BACKGROUND: The antiestrogen tamoxifen has potent activity against estrogen receptor-positive breast cancer, but two nonsteroidal aromatase inhibitors, letrozole and anastrozole, show considerable advantages over tamoxifen with respect to patient survival and tolerability. To determine the optimal way to use letrozole and tamoxifen, we studied their effects on a breast tumor xenograft model, MCF-7Ca, that is responsive to both antiestrogens and aromatase inhibitors. METHODS: Female ovariectomized BALB/c athymic nude mice carrying xenograft tumors were treated daily subcutaneously with one of

From Breast to Orbit: A Case Report of Metastatic Breast Cancer With Orbital Involvement.

The approach to manage breast cancer has undergone a significant transformation, leading to longer survival rates. However, there is still a rise in metastasis occurring in less common locations such as the orbit. We report the case of a 40-year-old female diagnosed with luminal A, left-side breast cancer, back in September 2020. She presented with de novo metastatic diseases to the liver, bone, lung, and orbit. She received palliative radiation therapy (RT) to the orbit at the dose of 25 Gray (Gy) in five fractions, and follow-up brain magnetic resonance imaging (MRI) indicated a positive res

Managing capivasertib-induced hyperglycemia: a report of 3 cases.

Phosphoinositide 3-kinase/protein kinase B (AKT) pathway inhibition is an important therapeutic strategy for hormone receptor positive and human epidermal growth factor receptor 2 negative advanced breast cancer. Capivasertib, an oral pan-AKT inhibitor, improves progression-free survival in combination with fulvestrant but is frequently associated with hyperglycemia. Despite this recognized adverse effect, consensus management guidelines are lacking. We describe 3 patients who developed capivasertib-associated hyperglycemia, 2 of whom had severe hyperglycemia with alpelisib therapy, a phosphoi

Treatment of ER-positive breast cancer with FDA-approved aromatase inhibitors.

Breast cancer is a leading cause of death among women globally. Estrogen receptor (ER)-positive breast cancers account for ∼70 % of breast cancer diagnoses worldwide. Endocrine therapy is the primary treatment approach for ER-positive breast cancer. In the United States, the Food and Drug Administration (FDA) has approved several endocrine therapies for ER-positive breast cancers, including selective estrogen receptor modulators (SERMs), selective estrogen receptor downregulators/degraders (SERDs), and aromatase inhibitors (AIs). The current FDA-approved SERMs include tamoxifen, toremifene, an
