Rating certainty of evidence when the target threshold is a value-based threshold and the point estimate is close to that threshold.

OBJECTIVES: It is crucial, when using Grading of Recommendations Assessment, Development and Evaluation (GRADE), that authors are clear about what it is in which they are rating the certainty of evidence (ie, the target of certainty of evidence rating). This article addresses challenges GRADE users face in choosing a target of certainty rating for any value-based threshold (ie, little or no difference; an important effect; small, moderate or large difference) when the point estimate is close to that threshold. STUDY DESIGN AND SETTING: Using an iterative process, authors generated and refined

New GRADE Evidence-to-Decision Framework for Pairwise and Multiple Comparisons (GRADE Guidance 45).

Evidence-based decision making in health often requires comparison of multiple options for a given condition. The GRADE (Grading of Recommendations Assessment, Development and Evaluation) evidence-to-decision (EtD) framework provides a structured approach for moving from evidence to decisions but was originally designed for pairwise comparisons. Hence, there is a need to accommodate decision making based on multiple comparisons, especially with the increasing use of systematic reviews and network meta-analyses in guideline development. Furthermore, since the original EtD framework was develope

Unilateral Biportal Endoscopic Discectomy via the Contralateral Sublaminar Approach for Lumbar Disc Herniation with Very High-Grade Migration: A Technical Note and Case Series.

BACKGROUND: Lumbar disc herniation (LDH) with very high-grade migration is difficult to manage surgically because of its anatomic complexity and the risk of incomplete decompression. The unilateral biportal endoscopic (UBE) contralateral sublaminar approach is a minimally invasive technique that may be an alternative option for managing these cases. METHODS: In this multicenter retrospective study, we analyzed the clinical data of 15 patients with lumbar disc herniation with very high-grade upward or downward migration of ruptured disc fragments removed via the UBE contralateral sublaminar ap

A Competing-Risks Approach to the Progression, Regression and Persistence of High-Grade Cervical Dysplasia in Patients over 30 Years Old-A Prospective Study.

Background/Objectives: In Romania, where cervical cancer incidence remains among the highest in the European Union, a risk-based management strategy could support more precise allocation of limited resources. The aim of this study was to test the prognostic utility of immediate pre-treatment and post-treatment risk predictions, derived from the American Society of Colposcopy and Cervical Pathology (ASCCP) risk-based management guidelines for the prediction of progression, regression or persistence of high-grade cervical dysplasia. Methods: In this prospective cohort study, we included 223 pati

A novel less invasive endoscopic-assisted procedure for complete reduction of low-and high-grade isthmic spondylolisthesis performed by anterior and posterior combined approach.

PURPOSE: The optimal surgical management of low- and high-grade isthmic spondylolisthesis (LGS and HGS -IS) is debated as well as whether reduction is needed especially for high-grade spondylolisthesis. Both anterior and posterior techniques can be associated with mechanical disadvantages as hardware failure with loss of reduction and L5 injury. We purpose a novel endoscopic-assisted technique (Sled technique, ST) to achieve a complete reduction in two surgical steps: first anteriorly through a retroperitoneal approach to obtain the greatest part of correction and then posteriorly to complete

Local endoscopic therapy for intraepithelial high-grade neoplasia and early adenocarcinoma in Barrett's oesophagus: acute-phase and intermediate results of a new treatment approach.

BACKGROUND: Radical oesophageal resection has until now been regarded as the gold standard for treatment in intraepithelial high-grade neoplasia or early adenocarcinoma of the oesophagus. However, the mortality and morbidity rates are substantial. DESIGN: A new therapeutic approach involving low-risk endoscopic therapy modalities was examined in the framework of a prospective study. PATIENTS: A total of 115 patients with intraepithelial high-grade neoplasia (19) and early adenocarcinoma (96) in Barrett's oesophagus. METHODS: Endoscopic mucosal resection (EMR) was used in 70 patients, and ph

Investigating the prognostics and theranostic cell cycle-related genes of Hinokinin in high-grade glioma: an integrated network pharmacology, bioinformatics, and molecular docking approach.

BACKGROUND: Glioblastoma (GBM) is the most malignant and prevalent primary brain tumor in adults, accounting for up to 60% of all adult brain tumor, classified as WHO grade 4. Uncontrolled cell-cycle progression is one of the hallmarks of GBM. The patient prognosis is still poor after multimodal treatment, underscoring the need for new therapeutic strategies. Derived from Piper cubeba, hinokinin is a bioactive dibenzylbutyrolactone lignan that has demonstrated interesting anticancer effects. However, little is known about its molecular processes in GBM. METHODS: To understand the therapeutic

A Comparative Study of IVIM-MRI Fitting Techniques in Glioma Grading: Conventional, Bayesian, and Voxel-Wise and Spatially-Aware Deep Learning Approaches.

BACKGROUND: Intravoxel incoherent motion (IVIM) analysis of diffusion-weighted MRI (DWI) provides microvascular perfusion and diffusion information. However, parameter estimation is limited by noise sensitivity, variability across fitting methods, and lack of standardization. Deep-learning (DL)-based approaches, particularly spatially-aware transformers, may improve robustness, but their clinical utility remains unexplored. PURPOSE: To evaluate conventional, Bayesian, and DL-based IVIM fitting methods in glioma patients, focusing on tumor grading accuracy. STUDY TYPE: Retrospective. POPULAT

Risk Factors for Postoperative Cerebrospinal Fluid Leak after Graded Multilayer Cranial Base Repair with Suturing via the Endoscopic Endonasal Approach.

This study analyzed risk factors for postoperative cerebrospinal fluid (CSF) leak after graded multilayer cranial base repair method with dural suturing. We performed surgery via the endoscopic endonasal approach (EEA) from 2012.6 to 2018.4, and those consecutive clinical data were prospectively accumulated and retrospectively analyzed. We tailored the repair method according to the intraoperative CSF leak grade. Among 388 surgeries via the EEA, there were 10 (2.6%) cases of postoperative CSF leak after graded repair with suturing. Postoperative CSF leak occurred in two of the 150 cases withou

Tracking the Corticospinal Tract in Patients With High-Grade Glioma: Clinical Evaluation of Multi-Level Fiber Tracking and Comparison to Conventional Deterministic Approaches.

While the diagnosis of high-grade glioma (HGG) is still associated with a considerably poor prognosis, neurosurgical tumor resection provides an opportunity for prolonged survival and improved quality of life for affected patients. However, successful tumor resection is dependent on a proper surgical planning to avoid surgery-induced functional deficits whilst achieving a maximum extent of resection (EOR). With diffusion magnetic resonance imaging (MRI) providing insight into individual white matter neuroanatomy, the challenge remains to disentangle that information as correctly and as complet
