[Drug treatment of gastro-esophageal reflux disease in pregnant women: consensus guidelines of gastroenterologists and gynaecologists].

Lifestyle changes are recommended as the first step in the treatment of pregnant women with heartburn. If symptoms persist, antacids or the mucoprotective sucralfate can be prescribed. If symptoms are persistent and severe, acid secretion inhibitors may be prescribed; the proton-pump inhibitor omeprazole is the drug of choice. It is unlikely that this drug could harm the fetus but the possibility cannot be entirely excluded. Prescription should be delayed until after the first trimester, whenever possible. Patients who have become pregnant while using these drugs can be reassured.

British gastroenterologists' care profile for patients with inflammatory bowel disease: the need for a patients' charter.

The follow-up of patients with inflammatory bowel disease (IBD) was investigated using a postal questionnaire sent to 359 members of the British Society of Gastroenterology (BSG), of whom 235 replied. Of patients with IBD, 96% were weighed on each out-patient clinic review and over 60% had their full blood count checked. Although few centres (20%) have computerized recall of their patients for cancer surveillance, 96% did perform such surveillance on patients with ulcerative colitis. The mean duration of disease before surveillance was initiated was 9.6 years. Most clinicians (80%) only survey

Adherence to clinical guidelines for the evaluation and management of eosinophilic esophagitis among gastroenterologists in the Arab countries.

BACKGROUND: The practice patterns of eosinophilic esophagitis (EoE) remain poorly characterized. Few studies investigated the variability of clinical patterns among gastroenterologists, mainly in the United States. OBJECTIVES: We assessed the practice patterns of gastroenterologists in the Arab countries regarding the diagnosis and management of EoE, and their concordance with the European 2017 guidelines and the Proceedings of the AGREE Conference published in 2018. METHODS: We conducted a cross-sectional, self-administered, online survey of practicing gastroenterologists in the Arab countr

Red flags for appropriate referral to the gastroenterologist and the rheumatologist of patients with inflammatory bowel disease and spondyloarthritis.

Collaboration between gastroenterologists and rheumatologists is recommended for the correct management of patients with associated spondyloarthritis (SpA) and inflammatory bowel disease (IBD). We aimed to establish the appropriateness of several red flags for a prompt specialist referral. A systematic review of the literature was performed using the GRADE method to describe the prevalence of co-existing IBD-SpA and the diagnostic accuracy of red flags proposed by a steering committee. Then, a consensus among expert gastroenterologists and rheumatologists (10 in the steering committee and 13 i

Surgical treatment for rectal cancer: an international perspective on what the medical gastroenterologist needs to know.

Rectal cancer accounts for one third of all colorectal cancers. The age adjusted death rates from colorectal cancer have declined over recent decades due to a combination of colorectal cancer screening, improved diagnostic tests, improved standardized surgical technique, improved medical support, neoadjuvant chemotherapies and radiation treatment or combinations of these. Because of complex treatment algorithms, use of multidisciplinary teams in the management of rectal cancer patients has also been popularized. Medical gastroenterologists performing colonoscopies are frequently the first heal

[Comparative characteristics of treatment quality and quality of life in gastroenterological patients supervised by gastroenterologist and therapeutist].

There was an analysis of the results of a 5-year observation over 226 patients with the most prevailing digestive apparatus diseases: stomach ulcer and duodenal ulcer (SU and DU), chronic pancreatitis (CP), irritable bowel syndrome (IBS) in an outpatient clinic. Patients were supervised by gastroenterologists (168 patients) and therapeutists (58 patients). It was noted that supervision of the patients by gastroenterologists authentically reduces the frequency of hospitalizations and duration of their stay on the sick-list as compared with the patients being observed by therapeutists and result

Dietl's crisis: a syndrome of episodic abdominal pain of urologic origin that may present to a gastroenterologist.

A 53-yr-old woman presented with a 2-yr history of recurrent episodes of severe abdominal pain and nausea. Multiple investigations by a general surgeon, a urologist, and a gastroenterologist failed to identify the cause. She was referred to our Biliary Service for ERCP and sphincter of Oddi manometry. However, a detailed history was inconsistent with biliary pain, and the patient, having discussed the risks and benefits, elected not to proceed with ERCP. The patient was asked to come to the hospital during an acute attack of her pain for assessment. When this was done, transabdominal ultrasoun

Evaluating the Practice of Prescribing Beta-blockers in Compensated Cirrhosis by Gastroenterologists in the Asia Pacific Region.

Non-selective beta-blockers (NSBBs) have a role in the management of portal hypertension. They are currently advocated in patients with clinically significant portal hypertension (CSPH) based on Baveno-VII consensus. Current survey aimed to evaluate the practice and perceptions of prescribing NSBBs in portal hypertension by gastroenterologists and hepatologists in Asia-Pacific region in patients with compensated cirrhosis. Out of 1500 gastroenterologists approached in the region, 328 gastroenterologists responded and completed the survey. 75% of the respondents were found not to be following p

Hypermobile Ehlers-Danlos syndrome and disorders of the gastrointestinal tract: What the gastroenterologist needs to know.

BACKGROUND AND AIM: Hypermobile Ehlers-Danlos syndrome (hEDS) and the hypermobility spectrum disorders (HSD) can be challenging to diagnose and manage. Gastrointestinal symptoms and disorders of gut-brain interaction are common in this cohort and multifactorial in origin. The primary aim of this review is to arm the gastroenterologist with a clinically useful understanding of HSD/hEDS, by exploring the association of gastrointestinal disorders with HSD/hEDS, highlighting current pathophysiological understanding and providing a pragmatic approach to managing these patients. METHODS: Literature

Interobserver variation in the radiographic diagnosis of gastric ulcer. Gastroenterologists' guesses as to level of interobserver variation.

A total of 156 double contrast barium examinations were independently interpreted by two observers for the presence of gastric ulcer. The observed agreement between the two readings was 93 per cent, and after adjusting for the expected chance agreement a kappa value of 0.73 was obtained. At the first reading 4 ulcers, and at the second reading 8 ulcers were interpreted as malignant. None of these interpretations agreed. In a questionnaire study it was demonstrated that gastroenterologists had a realistic impression of the level of agreement in the radiographic detection of gastric ulcer.

Assessing inflammation and predicting relapse in ulcerative colitis is more accurate when performed by IBD specialized gastroenterologists.

BACKGROUND: Ulcerative colitis is monitored through clinical, laboratory, endoscopic, and histologic measures. Persistence of histologic inflammation despite clinical and endoscopic remission is associated with disease flare. METHODS: We compared the identification of clinically significant histologic inflammation by general and IBD-specialized gastroenterologists (GI) using a simplified binary scoring system and looked at associations with disease relapse. RESULTS: Both general and IBD-specialized GIs identified significant inflammation associated with relapse, though IBD-specialized GIs sc

Diagnostic-Therapeutic Care Pathway in Chronic Constipation: AIGO (Italian Association of Gastroenterologists and Gastrointestinal Endoscopists) Position Paper.

Chronic constipation (CC) is one of the most common disorders of gut-brain interaction, affecting more than 11% of adults in Western countries, with higher prevalence in women and in the elderly. Despite its significant impact on quality of life, most patients self-manage their symptoms, while only a minority seek medical attention from general practitioners (GPs) or specialists. Proper assessment not only often requires a multidimensional approach but also accurate diagnostic and therapeutic pathways that define the exact role of GPs and specialists. This paper describes a comprehensive Diagn

The role of female endoscopists: are women gastroenterologists better at obtaining biopsies for celiac disease than men?

OBJECTIVES: Celiac disease (CD) is commonly found in women. Given the sex differences in diagnosed patients, we hypothesized sex differences in physicians obtaining biopsies for CD may exist. MATERIALS AND METHODS: We retrospectively reviewed duodenal biopsies for suspected CD excluding pre-existing CD patients. Appropriate biopsy practice was defined as ≥5 specimens per ACG guidelines. RESULTS: We included 125 patients (females, 92). There were 85 properly (68%) biopsied. Presence of a female endoscopist was associated with better adherence to biopsy guidelines (OR, 2.99, 95% CI, 1.19-7.54;
