Best practices and challenges in executing large scale pilots for eHealth deployment and managing innovation: the GATEKEEPER experience.

UNLABELLED: The large-scale deployment of digital health solutions requires robust operational frameworks capable of coordinating heterogeneous settings, diverse stakeholders, and complex technical infrastructures. However, actionable guidance for executing federated, multinational eHealth pilots remains limited in the implementation literature. METHODS: Using a mixed-methods approach, including iterative focus groups, co-creation sessions, and a Delphi study, we developed and refined a practice-derived management framework over four years within the GATEKEEPER project (EU Horizon 2020, Grant

Computed Tomography Coronary Angiography as a Gatekeeper for Invasive Coronary Assessment Before Transcatheter Aortic Valve Implantation.

Transcatheter aortic valve implantation (TAVI) has become the predominant treatment strategy for severe aortic stenosis across all surgical risk categories. The coexistence of coronary artery disease (CAD) in 40-75% of TAVI candidates has traditionally mandated pre-procedural invasive coronary angiography (ICA). However, computed tomography coronary angiography (CTCA), which is already integral to TAVI planning for annular sizing and access route evaluation, offers the potential to assess coronary anatomy simultaneously. Accumulating evidence demonstrates that CTCA possesses excellent sensitiv

Coronary CT angiography is a safe gatekeeper in non-ST-elevation acute coronary syndrome - The VECTOR study.

PURPOSE: In patients with non-ST-segment elevation acute coronary syndrome (NSTEACS), coronary pathology ranges from normal vessels to severe obstructive disease. In NSTEACS patients where invasive coronary angiography (ICA) is recommended, it is unknown whether a non-invasive coronary computed tomography angiography (CCTA) may be used for patient triage. The Very Early Computerized Tomography to Organize Revascularization in patients with NSTEACS (VECTOR) study was a pilot study that assessed whether CCTA can be used to safely discharge NSTEACS patients without obstructive coronary artery dis

Gatekeepers in primary care - a qualitative study on manual therapists' role in triaging patients with back and neck pain.

BACKGROUND: In Sweden, almost 60% of patients seeking care at primary healthcare centers do so due to musculoskeletal disorders. These patients constitute a strain on the healthcare system. Manual therapists are experts in diagnosing patients with these conditions but are usually not part of healthcare centers in Sweden. Further, there is a significant societal problem with overuse of medications, injections, medical imaging, and unnecessary surgeries, so-called low-value care, for back and neck pain. We aimed to explore the current management of patients with back and neck pain in primary hea

Training of Attentional Filtering, but Not of Memory Storage, Enhances Working Memory Efficiency by Strengthening the Neuronal Gatekeeper Network.

Memory training (MT) in older adults with memory deficits often leads to frustration and, therefore, is usually not recommended. Here, we pursued an alternative approach and looked for transfer effects of 1-week attentional filter training (FT) on working memory performance and its neuronal correlates in young healthy humans. The FT effects were compared with pure MT, which lacked the necessity to filter out irrelevant information. Before and after training, all participants performed an fMRI experiment that included a combined task in which stimuli had to be both filtered based on color and s

Social network structure and HIV infection among injecting drug users in Lithuania: gatekeepers as bridges of infection.

The aim of the study was to assess-while controlling for individual risk characteristics-how certain social network structural characteristics (degree, eigenvector, and betweenness centrality) are related to HIV infections. Injecting drug users (N = 299) in Vilnius, Lithuania were recruited using incentivized chain referral sampling for a cross-sectional study. Sociometric social links were established between participants, and UCINET was used to calculate network measures. HIV prevalence was 10 %, and all except two knew they were infected. Of the five variables that remained significant in t

PINCH proteins as dual-function nodes in vascular mural cell homeostasis: from cytoplasmic scaffolds to nuclear gatekeepers.

Wang and colleagues recently reported that PINCH proteins maintain vascular mural cell homeostasis through integrated cytoplasmic signaling and nuclear transcriptional mechanisms, identifying PINCH1 as a direct transcriptional coregulator of genes involved in inflammation, differentiation, and neurovascular integrity. While their multi-omics approach provides compelling mechanistic insight, several critical questions remain that warrant further investigation. This commentary discusses four key areas for future exploration: (1) the regulation of PINCH nucleocytoplasmic shuttling by mechanical c

Single-Cell Analysis Guides Organoid CRISPR Screening to Reveal NF1 as a Gatekeeper of Cervical Squamous Malignant Transformation.

Cervical cancer progresses from high-grade squamous intraepithelial lesions driven by persistent HPV infection. Identifying the gatekeepers restraining malignant transformation could reveal strategies to prevent and treat cervical cancer. Here, we integrated single-cell transcriptomics, CRISPR-Cas9 screening, and organoid modeling to dissect this process. Single-cell analysis of patient samples revealed progressive activation of RAS and proliferation programs along epithelial differentiation paths. A focused CRISPR screen in HPV-positive pre-tumoroids identified NF1 as the top suppressor of ma

Household dietary gatekeepers' perceptions of home cooking and its associated factors: A qualitative inquiry from urban India.

The high consumption of calorie-dense, nutrient-poor meals from out of home sources has been identified as a potential risk factor for the growing burden of obesity and diet-related chronic degenerative diseases among urban Indians. Consumption of home cooked meals is linked with positive dietary and health outcomes. However, little is known about the views of Indians regarding home cooking. Therefore, this investigation was designed to qualitatively explore urban Indian nutritional gatekeepers' perceptions of home cooking and the factors influencing it. A Multi-Goal, Multi-Action, Cognitive-Affective Model for Understanding Gatekeepers' Intention to Report Deviant Close-Others.

Reporting a close-other to authorities can save lives from extreme deviant behaviors but it also poses a dilemma as gatekeepers struggle between their relationship and societal obligations. We propose and test a multi-goal, multi-action, cognitive-affective model to elucidate this decision-making process. Across various contexts (hate crimes, problem gambling, and violent extremism), gatekeepers' motivational strength to report and not report their close-other and their anticipated regret toward both actions simultaneously contributed to the variance of reporting intentions (30-70%). Notably,

TREM-1 as a potential gatekeeper of neuroinflammatory responses: therapeutic validation and mechanistic insights in experimental traumatic brain injury.

BACKGROUND: Traumatic brain injury (TBI) triggers a cascade of neuroinflammatory responses mediated by microglial activation, which significantly contributes to secondary brain damage. While triggering receptor expressed on myeloid cells-1 (TREM-1) is a key inflammatory amplifier, its mechanistic role and therapeutic potential in TBI remain elusive. Thus, this study endeavored to elucidate the exact role of TREM-1 in experimental TBI. METHODS: A controlled cortical impact (CCI) model was established in male C57BL/6 mice to induce TBI. The temporal expression profile of TREM-1 was assessed usi

Capsaicin-Sensitive Peptidergic Sensory Nerves Are Anti-Inflammatory Gatekeepers in the Hyperacute Phase of a Mouse Rheumatoid Arthritis Model.

Capsaicin-sensitive peptidergic sensory nerves play complex, mainly protective regulatory roles in the inflammatory cascade of the joints via neuropeptide mediators, but the mechanisms of the hyperacute arthritis phase has not been investigated. Therefore, we studied the involvement of these afferents in the early, "black box" period of a rheumatoid arthritis (RA) mouse model. Capsaicin-sensitive fibres were defunctionalized by pretreatment with the ultrapotent capsaicin analog resiniferatoxin and arthritis was induced by K/BxN arthritogenic serum. Disease severity was assessed by clinical sco

GPBAR1 Functions as Gatekeeper for Liver NKT Cells and provides Counterregulatory Signals in Mouse Models of Immune-Mediated Hepatitis.

BACKGROUND & AIMS: GPBAR1, also known as TGR5, is a G protein-coupled receptor activated by bile acids. Hepatic innate immune cells are involved in the immunopathogenesis of human liver diseases and in several murine hepatitis models. Here, by using genetic and pharmacological approaches, we provide evidence that GPBAR1 ligation attenuates the inflammation in rodent models of hepatitis. MATERIAL AND METHODS: Hepatitis was induced by concanavalin A (Con A) or α-galactosyl-ceramide (α-GalCer). 6b-Ethyl-3a,7b-dihydroxy-5b-cholan-24-ol (BAR501), a selective agonist of GPBAR1, was administrated by

A new era of targeting the ancient gatekeepers of the immune system: toll-like agonists in the treatment of allergic rhinitis and asthma.

Toll-like receptors (TLR) belong to a large family of pattern recognition receptors known as the ancient 'gatekeepers' of the immune system. TLRs are located at the first line of defense against invading pathogens as well as aeroallergens, making them interesting targets to modulate the natural history of respiratory allergy. Agonists of TLRs have been widely employed in therapeutic or prophylactic preparations useful for asthma/allergic rhinitis (AR) patients. MPL® (a TLR4 agonist) and the CpG oligodeoxynucleotide of 1018 ISS, a TLR9 agonist, show strong immunogenicity effects that make them

Discovery of 5-(arenethynyl) hetero-monocyclic derivatives as potent inhibitors of BCR-ABL including the T315I gatekeeper mutant.

Ponatinib (AP24534) was previously identified as a pan-BCR-ABL inhibitor that potently inhibits the T315I gatekeeper mutant, and has advanced into clinical development for the treatment of refractory or resistant CML. In this study, we explored a novel series of five and six membered monocycles as alternate hinge-binding templates to replace the 6,5-fused imidazopyridazine core of ponatinib. Like ponatinib, these monocycles are tethered to pendant toluanilides via an ethynyl linker. Several compounds in this series displayed excellent in vitro potency against both native BCR-ABL and the T315I

ABCA1 as a Gatekeeper of Pancreatic β-Cell Cholesterol Homeostasis: Links Between Lipoprotein Stress, Lipotoxicity, and Insulin Secretion.

Pancreatic β cells require tightly controlled cholesterol distribution to maintain glucose sensing, insulin-granule trafficking, and regulated exocytosis. ATP-binding cassette transporter A1 (ABCA1) exports cellular cholesterol and phospholipids to lipid-poor apolipoprotein A-I and is a central component of β-cell cholesterol homeostasis. β-cell-specific Abca1 deletion causes cholesterol accumulation and impaired glucose-stimulated insulin secretion, whereas human studies of rare ABCA1 loss-of-function mutations have reported heterogeneous secretory phenotypes. Gamma-glutamyl transferase as a redox-gatekeeper biomarker for pancreatic cystic neoplasms: A concise roadmap from epidemiology to bedside.

Serum gamma-glutamyl transferase (GGT), a marker of hepatobiliary and oxidative stress, emerged as a predictor of incident pancreatic cystic neoplasms (PCNs) in a 2.65-million nationwide cohort followed for > 10 years. The highest GGT quartile conferred 11% excess PCN risk, with hazard ratios rising across quartiles and persisting after 3- and 5-year lag exclusions. In redox-oncology, pancreatic GGT1 isoform upregulated by oncogenic KRAS cleaves extracellular glutathione, promoting reactive oxygen species-mediated DNA damage, Wnt/β-catenin, and interleukin-6/signal transducer and activator of
