The role of high-risk geographies in the perpetuation of the HIV epidemic in rural South Africa: A spatial molecular epidemiology study.

In this study, we hypothesize that HIV geographical clusters (geospatial areas with significantly higher numbers of HIV positive individuals) can behave as the highly connected nodes in the transmission network. Using data come from one of the most comprehensive demographic surveillance systems in Africa, we found that more than 70% of the HIV transmission links identified were directly connected to an HIV geographical cluster located in a peri-urban area. Moreover, we identified a single central large community of highly connected nodes located within the HIV cluster. This module was composed

Geography, deer, and host biodiversity shape the pattern of Lyme disease emergence in the Thousand Islands Archipelago of Ontario, Canada.

In the Thousand Islands region of eastern Ontario, Canada, Lyme disease is emerging as a serious health risk. The factors that influence Lyme disease risk, as measured by the number of blacklegged tick (Ixodes scapularis) vectors infected with Borrelia burgdorferi, are complex and vary across eastern North America. Despite study sites in the Thousand Islands being in close geographic proximity, host communities differed and both the abundance of ticks and the prevalence of B. burgdorferi infection in them varied among sites. Using this archipelago in a natural experiment, we examined the relat

Advancing poliovirus eradication: lessons learned from piloting direct molecular detection of polioviruses in high-risk and priority geographies.

UNLABELLED: In the Global Polio Laboratory Network (GPLN), poliovirus (PV) screening results from acute flaccid paralysis (AFP) surveillance is based on virus isolation (VI) through cell culture, entailing long turnaround times and the amplification of live poliovirus. An alternative Direct Detection strategy (DD-ITD) for screening viral nucleic acid from stools, bypassing the need for virus culture, has been developed and extensively validated by GPLN partners. A multi-laboratory demonstration project was conceived to field-test the DD-ITD method by GPLN laboratories from the WHO African, Wes

A Theoretical Analysis of the Geography of Schistosomiasis in Burkina Faso Highlights the Roles of Human Mobility and Water Resources Development in Disease Transmission.

We study the geography of schistosomiasis across Burkina Faso by means of a spatially explicit model of water-based disease dynamics. The model quantitatively addresses the geographic stratification of disease burden in a novel framework by explicitly accounting for drivers and controls of the disease, including spatial information on the distributions of population and infrastructure, jointly with a general description of human mobility and climatic/ecological drivers. Spatial patterns of disease are analysed by the extraction and the mapping of suitable eigenvectors of the Jacobian matrix su

Empiric antibiotic treatment of erythema migrans-like skin lesions as a function of geography: a clinical and cost effectiveness modeling study.

The skin lesion of early Lyme disease, erythema migrans (EM), is so characteristic that routine practice is to treat all such patients with antibiotics. Because other skin lesions may resemble EM, it is not known whether presumptive treatment of EM is appropriate in regions where Lyme disease is rare. We constructed a decision model to compare the cost and clinical effectiveness of three strategies for the management of EM: Treat All, Observe, and Serology as a function of the probability that an EM-like lesion is Lyme disease. Treat All was found to be the preferred strategy in regions that a

Overcoming barriers to polio eradication in challenging subnational geographies: lessons from South Khyber Pakhtunkhwa, Pakistan, 2022-2024.

The global polio program has relied on supplementary immunization activities (SIA) in countries where routine immunization (RI) coverage is suboptimal. During these SIAs, oral poliovirus vaccine (OPV) is administered to all children under the age of 5 years. However, implementing SIAs in areas with insecurity and conflict, such as the South Khyber Pakhtunkhwa (South KP) region of Pakistan is very challenging because of attacks against vaccination teams and community boycotts among other reasons. These challenges sometimes lead to cancelation, delays, and poor quality of immunization campaigns.

Income, Geography, and Etiology as Factors Associated with Valve Replacement Outcomes: A Nationwide Cohort of 63,426 Patients in a Middle-Income Country.

BACKGROUND: Heart valve disease is a growing cause of mortality and disability worldwide, disproportionately burdening low- and middle-income countries (LMICs), where rheumatic heart disease remains a major etiology and cardiac surgical capacity covers less than 2% of the estimated need. Population-level outcome data after valve surgery in LMICs are virtually absent. We leveraged Brazil's universal health system to quantify how income, geography, and disease etiology are associated with survival after valve replacement surgery. METHODS: Five Brazilian public health databases covering outpatie

Navigating Digital Geographies and Trauma Contexts: Conceptions of Online Communities and Experiences Among LGBTQ+ People During COVID-19.

The coronavirus pandemic shaped challenges for marginalized groups. Specifically, lesbian, gay, bisexual, transgender, and/or queer (LGBTQ+) people experienced community-building constraints, notably in predominantly rural regions. People are also navigating digital geographies, or online social environments, in novel ways to develop virtual communities in the face of prejudice, discrimination, and potential trauma. Through a minority coping approach, the present study explored LGBTQ+ people's experiences navigating the dynamics of digital geographies during the pandemic while residing in soci
