Abnormal glycemic homeostasis at the onset of serious mental illnesses: A common pathway.

OBJECTIVE: Patients with serious mental illnesses exhibit a reduced lifespan compared with the general population, a finding that can not solely rely on high suicide risk, low access to medical care and unhealthy lifestyle. The main causes of death are medical related pathologies such as type 2 diabetes mellitus and cardiovascular disease; however pharmacological treatment might play a role. MATERIAL AND METHODS: We compared a two hour glucose load in naïve patients at the onset of a serious mental illness (N=102) (84 patients with a first episode of schizophrenia and related disorders, 6 wit

Synergistic use of 1,5-AG and HbA1c for early prediction of gestational diabetes: capturing BMI-dependent glycemic phenotypes.

PURPOSE: Recognizing metabolic heterogeneity in gestational diabetes mellitus (GDM) and body mass index (BMI)-linked phenotypes, we evaluated whether combining hemoglobin A1c (HbA1c, reflecting fasting glycaemia) and 1,5-anhydroglucitol (1,5-AG, reflecting post-load glucose excursions) improves early prediction and whether performance differs by BMI. METHODS: In this multicenter retrospective study, pregnant women who had 1,5-AG and HbA1c measured before 20 weeks of gestation at two tertiary centers in Japan were included. Spearman's correlation was used to assess associations between glycemi

Linking gestational RBP4 to bone resorption via postprandial glycemic pathways: A cross-sectional mediation analysis.

BACKGROUND: Retinol-binding protein 4 (RBP4), a key adipokine associated with glucose metabolism, is elevated during pregnancy and linked to gestational diabetes mellitus (GDM). However, its relationship with gestational bone resorption remains unclear. This study aimed to investigate the association between RBP4 and bone resorption (measured by β-C-terminal telopeptide of type I collagen [β-CTX]) and explore potential mediation pathways involving glycemic dysregulation in pregnancy. METHODS: In this cross-sectional study, we analyzed serum RBP4, β-CTX, glucose metrics (fasting blood glucose,

Review: Piglets' (Re)Feeding Patterns, Mineral Metabolism, and Their Twisty Tail.

The appearance rate of nutrients into systemic circulation affects hormones like insulin and through that efficiency of growth. This also affects mineral requirements critical for metabolism, notably phosphate (P), magnesium (Mg), and potassium (K). Fasting animals have a downregulated metabolism, upon which P, Mg, and K are exported from their cells into the blood and are subsequently excreted in their urine. Abrupt resumption of feed intake, especially of highly glycemic feeds, creates an acute need for these minerals, which can result in deficiency symptoms, particularly with P deficiency.

Oral glucose load and mixed meal feeding lowers testosterone levels in healthy eugonadal men.

PURPOSE: Precise evaluation of serum testosterone levels is important in making an accurate diagnosis of androgen deficiency. Recent practice guidelines on male androgen deficiency recommend that testosterone be measured in the morning while fasting. Although there is ample evidence regarding morning measurement of testosterone, studies that evaluated the effect of glucose load or meals were limited by inclusion of hypogonadal or diabetic men, and measurement of testosterone was not performed using mass spectrometry. METHODS: Sixty men (23-97 years) without pre-diabetes or diabetes who had no

Glucose metabolism in the Belgrade rat, a model of iron-loading anemia.

The iron-diabetes hypothesis proposes an association between iron overload and glucose metabolism that is supported by a number of epidemiological studies. The prevalence of type 2 diabetes in patients with hereditary hemochromatosis and iron-loading thalassemia supports this hypothesis. The Belgrade rat carries a mutation in the iron transporter divalent metal transporter 1 (DMT1) resulting in iron-loading anemia. In this study, we characterized the glycometabolic status of the Belgrade rat. Belgrade rats displayed normal glycemic control. Insulin signaling and secretion were not impaired, an

Efficacy of oral insulin nanoparticles for the management of hyperglycemia in a rat model of diabetes induced with streptozotocin.

Insulin is the cornerstone of treatment in type 1 diabetes mellitus. However, because of its protein structure, insulin has to be administered via injection, and many attempts have been made to create oral formulations, especially using nanoparticles (NPs). The aim of this study was to compare the hypoglycemic effect of insulin-loaded NPs to that of subcutaneous insulin in an in vivo rat model of diabetes. We used biodegradable D-α-tocopherol polyethylene glycol succinate-emulsified, chitosan-capped poly(lactic-co-glycolic acid) NPs loaded with soluble human insulin in a dose of 20 IU/kg body

Prehabilitation for Breast Reconstruction Surgery: A Practical and Evidence-based Guide.

BACKGROUND: Prehabilitation, the systematic optimization of physical, nutritional, and psychosocial health before surgery, is well established in other surgical specialties but remains underused and inconsistently defined in breast reconstruction. Patients pursuing reconstruction frequently have modifiable vulnerabilities, including malnutrition, sarcopenia, impaired glycemic control, chronic pain, sleep disturbance, anxiety, and depression, which are associated with higher complication rates, delayed functional recovery, and worse quality of life. METHODS: This review summarizes evidence-sup

Intestinal Macrophage-Mediated Exosomal Repackaging Establishes Bioactive Reservoirs for Long-Acting Oral Peptide Therapy.

Oral peptide delivery systems are conventionally engineered to improve intestinal absorption, but they rarely address the progressive and systemic nature of chronic inflammatory disorders such as type-2 diabetes (T2D), which requires both durable glycemic control and immunometabolic management. Here, we report an oral delivery system based on porous calcium carbonate/sodium alginate (PCC/SA) microparticles loaded with exenatide-chitosan oligosaccharide (EXE-COS). The PCC/SA carrier facilitates the efficient intestinal absorption and transepithelial transport of EXE-COS. Notably, in the lamina

A novel, potent, and long-lasting dipeptidyl peptidase-4 inhibitor, teneligliptin, improves postprandial hyperglycemia and dyslipidemia after single and repeated administrations.

Dipeptidyl peptidase-4 (DPP-4) inhibitors have been demonstrated to improve glycemic control, in particular postprandial hyperglycemic control, in patients with type 2 diabetes. Teneligliptin is a novel chemotype prolylthiazolidine-based DPP-4 inhibitor. The present study aimed to characterize the pharmacological profiles of teneligliptin in vitro and in vivo. Teneligliptin competitively inhibited human plasma, rat plasma, and human recombinant DPP-4 in vitro, with IC(50) values of approximately 1 nmol/l. Oral administration of teneligliptin in Wistar rats resulted in the inhibition of plasma

Load-dependent effects of duodenal glucose on glycemia, gastrointestinal hormones, antropyloroduodenal motility, and energy intake in healthy men.

Gastric emptying is a major determinant of glycemia, gastrointestinal hormone release, and appetite. We determined the effects of different intraduodenal glucose loads on glycemia, insulinemia, glucagon-like peptide-1 (GLP-1), glucose-dependent insulinotropic polypeptide (GIP) and cholecystokinin (CCK), antropyloroduodenal motility, and energy intake in healthy subjects. Blood glucose, plasma hormone, and antropyloroduodenal motor responses to 120-min intraduodenal infusions of glucose at 1) 1 ("G1"), 2) 2 ("G2"), and 3) 4 ("G4") kcal/min or of 4) saline ("control") were measured in 10 healthy

Formulation and Characterization of CCD-based Optimized Geraniol-loaded Antidiabetic Niosomes.

INTRODUCTION: Type 2 Diabetes (T2D) presents a significant global health challenge, characterized by persistent hyperglycemia and insulin resistance, with effective long-term glycemic control remaining a critical unmet need. Geraniol, a naturally occurring monoterpene alcohol, holds promising anti-diabetic potential but is limited by poor water solubility, which restricts its therapeutic application. This study focuses on the formulation and CCD-based optimization of geraniol-loaded niosomes. Additionally, to assess its antidiabetic effect, blood glucose levels were evaluated. METHOD: Geranio

Endothelial HSP72 is not reduced in type 2 diabetes nor is it a key determinant of endothelial insulin sensitivity.

Impaired endothelial insulin signaling and consequent blunting of insulin-induced vasodilation is a feature of type 2 diabetes (T2D) that contributes to vascular disease and glycemic dysregulation. However, the molecular mechanisms underlying endothelial insulin resistance remain poorly known. Herein, we tested the hypothesis that endothelial insulin resistance in T2D is attributed to reduced expression of heat shock protein 72 (HSP72). HSP72 is a cytoprotective chaperone protein that can be upregulated with heating and is reported to promote insulin sensitivity in metabolically active tissues

Evaluating surgical and non-surgical wound approaches in diabetic foot ulcer treatments: A comparative study.

Diabetic foot ulcers (DFUs) remain a serious complication of diabetes mellitus, often leading to infection, prolonged hospitalization and potential limb amputation. Therefore, it is of interest to evaluate the outcomes of surgical versus non-surgical management of DFUs in 100 patients, divided equally into two groups of fifty each. Group A received surgical interventions such as debridement and minor procedures, while Group B underwent non-surgical care including dressings, antibiotics, glycemic control and off-loading. Healing progress, infection control, duration of recovery and complication
