Surgical technique and clinical application of the transverse gracilismyocutaneous free flap.

The unreliability of the distal skin component of the gracilis myocutaneous free flap has been frequently reported. To improve the reliability of the skin we orientated the cutaneous paddle in a transverse direction in the proximal third of the gracilis muscle, as first described by Yousif et al in 1993. Their anatomical studies showed that cutaneous branches of the dominant proximal pedicle have a pronounced tendency to travel in a transverse direction, supplying the skin anteriorly over the adductor longus and sartorius muscles and extending beyond the posterior margin of the gracilis muscle

Computer modeling of the pathomechanics of spastic hip dislocation in children.

Spastic muscles about the hip cause subluxation, dislocation, and lead to acetabular dysplasia. Spastic hip disease occurs when the muscles about the hip exert forces that are too high or in the wrong direction or both. To determine the role of the hip forces in the progression of spastic hip disease and the effect of both muscle-lengthening and bony reconstructive surgeries, a computerized mathematical model of a spastic hip joint was created. The magnitude and direction of the forces of spastic hips undergoing surgery were analyzed preoperatively and postoperatively to determine which proced

Long-term outcomes of dual innervation in functional muscle transfers for facial palsy.

BACKGROUND: This study describes a different approach with a 2-stage facial reanimation in patients with long-standing unilateral facial paralysis using free gracilis muscle transfer, innervated by both cross-facial nerve graft and masseteric nerve. The authors present their rationale, surgical technique, and long-term outcomes. METHODS: Between August 2012 and March 2016, 11 patients (6 female and 5 male patients) underwent a 2-staged dually innervated gracilis muscle transfer. Patients were evaluated with physical examination and needle electromyography. A standardized assessment of preoper

Free gracilis transfer reinnervated by the nerve to the supinator for the reconstruction of finger and thumb extension in longstanding C7-T1 brachial plexus root avulsion.

PURPOSE: To report the clinical results of a free gracilis muscle transfer to finger and thumb extensors reinnervated by supinator muscle motor branches in patients with longstanding C7-T1 root avulsion. METHODS: Between January 2010 and January 2011, 3 young adult patients with traumatic C7-T1 brachial plexus palsies had gracilis transfer to the thumb and finger extensors at a mean of 38 months after injury. The muscle flap was connected to radial vessels and comitant veins and to nerve branches supplying the supinator muscle. RESULTS: All patients had recovery of active thumb and finger ex

Buccal mucosa graft tube urethroplasty for the treatment of bulbopenile urethral trauma accompanied with massive tissue loss due to improvised explosive device injury.

Urinary diversion with suprapubic cystostomy and delayed urethroplasty is recommended for the treatment of penetrating posterior urethral traumas. A devastating urethral trauma caused by a blast injury due to an improvised explosive device is an extremely rare clinical condition and treatment options are limited due to accompanying massive tissue and muscle loss. Staged urethral reconstruction using a pedicled gracilis muscle flap with a skin or buccal mucosa graft is the preferred treatment option for complex urethral traumas. In case of a devastated urethra due to an intensive explosive devi

An emerging modality for Fournier's Gangrene reconstruction: bilateral gracilis flaps with skin graft case reports.

INTRODUCTION: Fournier's gangrene often results in an extensive defect with the exposure of testes. While the initial goal of surgical management is debridement and control of infection, subsequent management involves consideration of wound closure, functional and aesthetic outcomes. We presented the technique of gracilis musculocutaneous flap with overlying split thickness skin grafting for scrotal reconstruction after Fournier's Gangrene. PRESENTATION OF CASE: Two patients with Fournier's gangrene underwent adequate surgical debridement of the scrotum and perineum. Bilateral pedicled gracil

Technique of Distal Tendon Excision for the Treatment of Chronic Pain Caused by Isolated Proximal Gracilis Tears.

Proximal gracilis tear is a rare sports-related injury that can easily lead to misdiagnosis and underdiagnosis. Clinical manifestations are dominated by localized pain, tenderness, and dysfunction. Here, we report a therapy of proximal segmental tear of the gracilis and demonstrate its treatment process and clinical outcome. This Technical Note describes in detail the minimally invasive distal gracilis resection technique for the treatment of chronic proximal gracilis ruptures, focusing on the most important technical aspects and thus attempting to introduce this procedure and make it more wid

Gracilis pull-through flap for the repair of a recalcitrant recto-vaginal fistula.

Recto-vaginal fistulas are difficult to treat due to their high recurrence rate. Currently, no single surgical intervention is universally regarded as the best treatment option for rectovaginal fistulas. We present a case of recurrent recto-vaginal fistula surgically treated with a gracilis pull-through flap. The surgical goals in this patient were complete excision of the recto-vaginal fistula and introduction of fresh, vascularized muscle to seal the fistula. A defunctioning colostomy was performed 1 month prior to the present procedure. The gracilis muscle and tendon were mobilized, pulled

The natural history of donor hamstrings unit after anterior cruciate ligament reconstruction: a prospective MRI scan assessment.

PURPOSE: The aim of this prospective study was to assess the natural history of the donor hamstrings, compared to the contralateral knee and thigh. METHODS: In 47 patients who had ACL reconstruction (hamstrings technique), bilateral knees and thighs were investigated using MRI scans, up to 2 years post-operatively. RESULTS: Tendon regeneration below the knee joint line of the semitendinosus occurred in 30/47 (64 %) patients and of the gracilis tendon in 17/47 (36 %) patients. Insertion of both tendons at the "pes anserinus", with normal anatomic relations, was observed in 5/47 (11 %) patient

Very rare arrangement of the pes anserinus: potential clinical significance.

The pes anserinus superficialis is composed of the semitendinosus, gracilis and sartorius tendons. Normally, they all insert to the medial side of the tibial tuberosity, and the first two are attached superiorly and medially to the tendon of the sartorius muscle. During anatomical dissection, a new pattern of arrangement of tendons creating the pes anserinus was found. The pes anserinus comprised three tendons; the semitendinosus tendon was located superiorly to the gracilis tendon, and they both had distal attachments on the medial side of the tibial tuberosity. This seemed like the normal ty

Reinnervated free gracilis flap with basal curtain barrier for tongue reconstruction: a case series and surgical technique.

OBJECTIVE: Conventional free flaps for tongue reconstruction often result in adhesion and limited mobility. This study presents a case series of two patients evaluating a novel reinnervated free gracilis flap with a basal curtain barrier design. METHODS: Two patients with cT3N0M0 tongue squamous cell carcinoma underwent hemiglossectomy. Case 1 was reconstructed using the novel technique (gracilis muscle with a skin island barrier), while Case 2 was reconstructed with a standard reinnervated gracilis muscle flap without a skin island. RESULTS: In Case 1, the skin island effectively closed the f

The Gracilis Free Flap.

The gracilis free flap is the ideal modality of emotive and spontaneous facial reanimation in patients with a viable contralateral facial nerve. A 2-stage procedure with a cross-face nerve graft followed by gracilis free flap inset is advocated. In this article, the anatomy of the gracilis muscle, alternative neural sources (including the masseteric nerve), and technical aspects of the procedure are discussed. The literature regarding outcomes and complications is reviewed.

Ganglion cells in arterioles of skeletal muscle: role in sympathetic vasodilation.

Ganglion cells were found in arterioles of gracilis muscles of dogs 2 wk after complete extrinsic denervation. We tested the possibility that they function in active sympathetic vasodilation (SVD) induced in isolated gracilis muscles by hypothalamic stimulation. To this end various drugs were injected into the gracilis perfusate. (Drugs did not reach the systemic circulation, and exerted their effects within the gracilis itself). C6 had no effect. Submaximal doses of atropine or l-hyoscyamine delayed and slowed SVD; higher doses blocked completely. d-Hyoscyamine did not change SVD or acetylcho

The Split Hypoglossal Nerve and Cross-Face Nerve Graft for Dual Innervation of the Functional Muscle Transfer in Facial Reanimation.

Facial paralysis is a disabling deformity. The affected individual is seriously affected both esthetically and functionally. Free functional muscle transfer is currently the corner stone in the management of long-standing facial nerve paralysis. Several nerve options are available to supply the free muscle transfer. These nerves can be used alone or in combination. The aim of this work is to study the possibility and results of dually innervating the free functioning muscle transfer. The dual innervation is done using the split hypoglossal nerve and cross-face nerve graft (CFNG) both sutured i

Complex Salvage Lower Lip Reconstruction Using Gracilis Free Flap.

BACKGROUND: The gracilis muscle microneurovascular free tissue transfer is a workhorse in dynamic facial reanimation. For patients with prior free flap reconstruction for large lower lip defects and in cases where local or regional tissue flap is inadequate for reconstruction, the gracilis free flap should be considered as a standard reconstructive option. OBJECTIVES: To define salvage lip reconstruction with gracilis muscle free flap. METHODS: We performed chart review of four patients with history of total lower lip defects with prior free flap reconstruction who underwent salvage lip reco

The gracilis free flap revisited: a review of 25 cases of transfer to traumatic extremity wounds.

Trauma to the extremities often results in a complex bony and soft-tissue injury requiring free flap reconstruction. Muscles from various body sites have been used in extremity reconstruction since the early 1970s. The gracilis muscle is usually not considered the first choice for free flap reconstruction of these defects. It is usually relegated to small defects or used to reanimate the face. Our purpose is to present our experience with the gracilis muscle as a first-choice flap in reconstruction of traumatic extremity defects. A retrospective review of all gracilis muscle free flap transfer

The spreaded gracilis flap revisited: Comparing outcomes in lower limb reconstruction.

BACKGROUND: In lower limb reconstruction, the free gracilis muscle flap is usually chosen for coverage of small- to medium-sized defects. Aim of this study was a re-appraisal regarding the reconstructive indications of the gracilis muscle flap with a focus on postoperative complications in order to evaluate if it can also be routinely used to reconstruct large-size lower limb defects. METHODS: A total of 204 patients underwent free gracilis muscle transfers for lower limb reconstruction at our institution. Cases were divided according to the size of the defect into two groups (<150 cm2 versus

Reliability of functioning free muscle transfer and vascularized ulnar nerve grafting for elbow flexion in complete brachial plexus palsy.

UNLABELLED: We compared outcomes of primary vascularized ulnar nerve grafts from the C5 root neurotizing biceps and brachialis muscles, and gracilis functioning free muscle transfer neurotized by the distal spinal accessory nerve, as a primary or salvage procedure after complete brachial plexus injury. At 45 months, three of eight primary vascularized ulnar nerve graft patients regained grade 4 elbow flexion, while one regained grade 3. All 13 primary gracilis transfer patients regained grade 4 elbow flexion. Four patients with vascularized ulnar nerve grafts failed and subsequently had salvag
