Use of propranolol in a remote region of rural Guatemala to treat a large facial infantile haemangioma.

We present a female infant with a right-sided facial and neck haemangioma, from a remote, resource-poor community in rural Guatemala. She received first-line treatment, propranolol, with marked reduction in tumour size and erythema. Treatment was stopped after 35 weeks due to recurrent diarrhoea and sustained weight loss. Propranolol can be used to safely treat infants with haemangiomas in remote, rural communities if there is adequate follow-up, education and communication. Periocular haemangiomas should be treated promptly to avoid visual impairment. Infants with large facial haemangiomas sh

Clinical assessment of self-reported acute flaccid paralysis in a population-based setting in Guatemala.

Historically, poliovirus infection has been an important cause of acute flaccid paralysis (AFP) worldwide; however, successful elimination of wild-type poliovirus in much of the world has highlighted the importance of other causes of AFP. Despite the evolving etiology, AFP surveillance in most developing countries still focuses on poliovirus detection and fails to detect many AFP cases, particularly among adults. We assessed 41 subjects self-reporting symptoms suggestive of AFP during a population-based health survey in the Department of Santa Rosa, Guatemala. Thirty-five (85%) of the suspecte

A comparison of the epidemiology and clinical presentation of seasonal influenza A and 2009 pandemic influenza A (H1N1) in Guatemala.

BACKGROUND: A new influenza A (H1N1) virus was first found in April 2009 and proceeded to cause a global pandemic. We compare the epidemiology and clinical presentation of seasonal influenza A (H1N1 and H3N2) and 2009 pandemic influenza A (H1N1) (pH1N1) using a prospective surveillance system for acute respiratory disease in Guatemala. METHODOLOGY/FINDINGS: Patients admitted to two public hospitals in Guatemala in 2008-2009 who met a pneumonia case definition, and ambulatory patients with influenza-like illness (ILI) at 10 ambulatory clinics were invited to participate. Data were collected th

No evidence for prolonged excretion of polioviruses in persons with residual paralytic poliomyelitis in Ethiopia, Pakistan and Guatemala.

Persons who have developed acute flaccid paralysis following infection with wild-type polioviruses or vaccine-associated paralytic poliomyelitis usually excrete polioviruses for only a few weeks. However, some patients with paralytic poliomyelitis have had prolonged excretion of polioviruses for periods of up to 10 years after onset of disease. Most prolonged excretors have been identified in industrialized countries. We studied 348 patients 2-28 years old in Ethiopia, Pakistan and Guatemala with residual paralytic poliomyelitis to determine if they had IgA or IgG deficiency or persistent poli

Effect of supplementary feeding on the prevention of mild-to-moderate wasting in conditions of endemic malnutrition in Guatemala.

OBJECTIVE: To estimate the effect of supplementary feeding on the prevention of wasting in preschool children in a rural area of Guatemala with a high prevalence of malnutrition. METHODS: Children aged 6-48 months, with a weight-for-length exceeding 90% of that of the median NCHS/WHO/CDC reference population, received either Atole, a drink with a high protein and moderate energy content, or Fresco, a drink with no protein and a low energy content. Children consuming amounts of Atole equivalent to more than 10% of the age-adjusted recommended dietary intake for energy were termed "supplemented

[Introduction of Bacillus sphaericus strain-2362 (GRISELESF) for biological control of malaria vectors in Guatemala].

Malaria continues to be an important health problem in a number of countries of Central and South America where it is considered as a highly prevent endemic disease. The objective of this paper is to assess the entomo-epidemiological impact of a pilot program for the biological control of malaria-transmitting vectors, which was implemented in 1998 in Escuintla, Republic of Guatemala. This program was based on the use of 20,000 L of biolarvicide Bacillus sphaericus- strain-2362 (GRISELESF) which was applied in the 46 localities of highest epidemiological risk at a rate of 10 mL/m2 of effective

Insecticide barrier spraying for the control of sand fly vectors of cutaneous leishmaniasis in rural Guatemala.

An initial evaluation of insecticide barrier spraying directed against sand fly vectors of cutaneous leishmaniasis was done in a nonclimax forested area with heavy undergrowth in Peten, Guatemala. A 100 m-wide swath of vegetation was sprayed once with a 1:3 mixture of cyfluthrin insecticide and a palm oil carrier using back-pack sprayers to simulate a central cantonment area in one site while another site remained as an untreated control. Prior to spraying and throughout 87 days post-treatment, sand fly populations were monitored at both sites with light traps set at ground and canopy levels a

Geographical distribution of the prevalence of nodules of Onchocerca volvulus in Guatemala over the last four decades.

In Guatemala a national nodulectomy campaign has been carried out since 1935 in three operational areas, namely the Eastern, Central and Western endemic zones. The prevalence of nodules was calculated in each 1 km grid square in these zones in each decade and plotted on mesh maps. A total of nine foci of high endemicity, three in each operational area, was found in the decade 1940-49. Nodule prevalence was concentric in each endemic focus; high in the centre and gradually falling towards periphery. The foci were found mainly on Tertiary or Quaternary-Tertiary volcanic rocks in the Eastern and

Diarrhea associated with rotavirus in rural Guatemala: a longitudinal study of 24 infants and young children.

A population of 24 infants and young children followed prospectively during the first 3 years of life was studied for the occurrence of rotavirus infection by using enzyme-linked immunosorbent assay to detect virus in stools. Infection with rotavirus was associated with 26 (14.2%) of 183 selected diarrheal episodes. Twenty of the 24 infants and young children had diarrhea associated with rotavirus on at least one occasion and six had two such episodes. Rotavirus infection was documented in over 50% of the dehydrating episodes studied, thus further indicating the importance of rotavirus in this

Review of Feasibility and Impact of Point-of-Care Ultrasound Use in Rural Guatemala Outreach Clinics.

BACKGROUND: Despite advances in medical technology, diagnostic gaps persist in low-resource settings. Point-of-care ultrasound (POCUS) is a portable, accessible tool that can expedite diagnosis, guide treatment, and improve clinical outcomes. However, few studies quantify its real-world impact, specifically in low-resource settings. This study seeks to evaluate how POCUS influences clinical diagnosis and treatment decisions in areas with limited radiologic support, as well as highlight cases where its use influenced patient management. METHODS: POCUS use during five days of a primary care out

Genetic and clinical landscape of Duchenne muscular dystrophy in Guatemala: insights from a national study.

Duchenne muscular dystrophy (DMD) is a severe X-linked disorder caused by mutations in the DMD gene, and it has a global prevalence of 3.6 per 100,000 people. Despite its well-documented genetic basis, no previous studies have characterized DMD in Guatemala. We analyzed 33 genetically confirmed cases to estimate the prevalence, describe the mutation spectrum, and assess the clinical features. The prevalence was 0.61 per 100,000 men younger than 30 years. Symptoms began before the age of 5 years in 85% of cases, yet 60% cases were diagnosed after the age of 6 years, highlighting significant dia

First report of Echinococcus canadensis (G7) in backyard pigs from the western highlands of Guatemala.

Echinococcus granulosus sensu lato is the causative agent of cystic echinococcosis (CE), a globally distributed zoonotic infection. In Guatemala, no new data have been reported for the past 80 years on CE. To address this gap, a cross-sectional study at the municipal slaughterhouse of Quetzaltenango was conducted from March to August 2022 to determine the presence of Echinococcus sp. in backyard pigs. Moreover, the species and haplotypes, fertility status of hydatid cysts, association of fertility of the cysts to the sex of the pig and the size of cysts were investigated. For this purpose, 117

HLA-DRB1 and HLA-DQB1 genes in patients diagnosed with systemic lupus erythematosus in Guatemala.

Systemic lupus erythematosus (SLE) is an autoimmune disorder that impacts connective tissue and can affect various organs and systems within the body. One important aspect of this disease is the role of the human leukocyte antigen (HLA) system, a protein complex that plays a role in the immune response. Specifically, the HLA-DRB1 and HLA-DQB1 genes have been implicated in the development of SLE. In order to better understand this relationship in the Guatemalan population, a study was conducted with the objective of characterizing the allelic and haplotype profiles of the HLA-DQB1 and HLA-DRB1

Building collective power in citizen-led initiatives for health accountability in Guatemala: the role of networks.

BACKGROUND: Citizen-led accountability initiatives are a critical strategy for redressing the causes of health inequalities and promoting better health system governance. A growing body of evidence points to the need for putting power relations at the forefront of understanding and operationalizing citizen-led accountability, rather than technical tools and best practices. In this study, we apply a network lens to the question of how initiatives build collective power to redress health system failures affecting marginalized communities in three municipalities in Guatemala. METHODS: Network ma

Overlapping vitamin A interventions in the United States, Guatemala, Zambia, and South Africa: case studies.

Vitamin A (VA) deficiency is a serious public health problem, especially in preschool children who are at risk of increased mortality. In order to address this problem, the World Health Organization recommends periodic high-dose supplementation to children 6-59 months of age in areas of highest risk. Originally, supplementation was meant as a short-term solution until more sustainable interventions could be adopted. Currently, many countries are fortifying commercialized common staple and snack foods with retinyl palmitate. However, in some countries, overlapping programs may lead to excessive

Knowledge, attitudes and practices regarding rabies and exposure to bats in two rural communities in Guatemala.

BACKGROUND: Rabies is a fatal encephalitis caused by rabies virus, of the genus Lyssavirus. The principal reservoir for rabies in Latin America is the common vampire bat (Desmodus rotundus), which feeds routinely on the blood of cattle, and when livestock are scarce, may prey on other mammals, including humans. Although rabies is endemic in common vampire bat populations in Guatemala, there is limited research on the extent of exposure to bats among human populations living near bat refuges. RESULTS: A random sample of 270 of 473 households (57%) in two communities located within 2 Km of a kn

Stunting at birth: recognition of early-life linear growth failure in the western highlands of Guatemala.

OBJECTIVE: Measurements of length at birth, or in the neonatal period, are challenging to obtain and often discounted for lack of validity. Hence, classical 'under-5' stunting rates have been derived from surveys on children from 6 to 59 months of age. Guatemala has a high prevalence of stunting (49.8%), but the age of onset of growth failure is not clearly defined. The objective of the study was to assess length-for-age within the first 1.5 months of life among Guatemalan infants. DESIGN: As part of a cross-sectional observational study, supine length was measured in young infants. Mothers'
