Self-reported depression and suicide attempts among U.S. women physicians.

OBJECTIVE: Studies examining suicide rates for U.S. women physicians and other U.S. women have found odds ratios as high as 4 to 1. Although such reports are controversial and are based on small groups (N = 17 to 49 suicides), they are often cited as evidence of a high prevalence of psychopathology among women physicians. METHOD: The authors used the results of the Women Physicians' Health Study (N = 4,501), a large, nationally distributed questionnaire, to assess the lifetime prevalence of self-identified depression and suicide attempts among U.S. women physicians. RESULTS: An estimated 1.5

Implementing a Stop the Bleed Course and Active Shooter Principles With Prelicensure Nursing Students: A Pilot Study.

INTRODUCTION: Deaths by firearms have surpassed motor vehicle accidents as the leading cause of death within the United States. Mass shootings have increased annually, leading to civilian trauma systems implementing military trauma training and rapid tourniquet use to treat extremity injuries. BACKGROUND: The American College of Surgeons' Stop the Bleed course has become a widely adopted public education initiative focused on the immediate treatment of life-threatening bleeding caused by firearm injuries. METHODS: Thirty prelicensure nursing students engaged in a structured simulation that i

Management of Maxillofacial Trauma in Pregnant Women.

INTRODUCTION: Trauma during pregnancy deserves special attention as management must be directed towards both the mother and the fetus. Management of maxillofacial fractures in pregnancy can adversely affect the well-being of the fetus by impinging on normal functions such as respiration, mastication, and nutrition. Pregnancy complicates the management of facial injury due to the maintenance of the patent airway, anesthesia considerations, and imaging restraints. The purpose of this study is to use three illustrative from our own institution to further elucidate education on the management of m

Outcome and rational management of civilian gunshot injuries to the brain-retrospective analysis of patients treated at the Helsinki University Hospital from 2000 to 2012.

BACKGROUND: Treatment of gunshot wounds of the brain (GSWB) remains controversial and there is high variation in reported survival rates (from < 10 to > 90%) depending on the etiology and country. We retrospectively analyzed the outcome of a series of consecutive GSWB patients admitted alive to a level 1 trauma center in a safe high-income welfare country with a low rate of homicidal gun violence. METHODS: Patients admitted due to a GSWB to the HUS Helsinki University Hospital during 2000-2012 were identified from hospital discharge registry and log books of the emergency room and ICU. CT sca

Impact of the COVID-19 pandemic on gunshot injuries at a level-1 trauma centre: a retrospective study on a 5-year period.

BACKGROUND: Gunshot injuries are a major cause of morbidity and mortality, and evidence shows that violent crimes increased during the COVID-19 pandemic. The aim of this study was to investigate the impact of the pandemic on the prevalence of gunshot injuries and to analyze the demographic characteristics of patients with gunshot injury at a level-1 trauma centre. METHODS: We conducted a retrospective analysis from April 2018 to February 2023. We collected demographic information, injury type, weapon involved, and mechanism of injury. We examined the annual incidence of gunshot injuries to as

Management of pediatric intracranial gunshot wounds: predictors of favorable clinical outcome and a new proposed treatment paradigm.

OBJECT: There has been an increase in civilian gun violence since the late 1980s, with a disproportionately high increase occurring within the pediatric population. To date, no definite treatment paradigm exists for the management of these patients, nor is there a full understanding of the predictors of favorable clinical outcome in this population. METHODS: The authors completed a retrospective review of all victims of intracranial gunshot injury from birth to age 18 years at a major metropolitan Level 1 trauma center (n = 48) from 2002 to 2011. The predictive values of widely accepted adult

Prevalence and correlates of handgun access among adolescents seeking care in an urban emergency department.

OBJECTIVE: To determine prevalence and correlates of handgun access among adolescents seeking care in an urban Emergency Department (ED) in order to inform future injury prevention strategies. METHODS: In this observational cross-sectional study performed in the ED of a large urban hospital, 14- to 18-year-old adolescents completed a computerized survey of risk behaviors. Adolescents seeking ED care (for injury or medical complaint) were approached seven days a week over a 22-month period. Validated measures included measures of demographics, sexual activity, substance use, injury, violent be

Gunshot-Related Metacarpal Fractures: A Retrospective Study and Single Urban Center Experience.

PURPOSE: As gun violence continues to grow, hand surgeons are increasingly called upon to manage complex ballistic hand fractures. However, there is a relative scarcity of literature on civilian gunshot-related metacarpal fractures. In this study, we examine the management strategies and outcomes for ballistic metacarpal fractures at a high-volume, level-1 urban trauma center. METHODS: Through the use of an institutional database of all patients with firearm-related fractures who presented to our level 1 trauma center, we identified all patients with ballistic hand fractures over a 4-year per

Who is killing South African men? A retrospective descriptive study of forensic and police investigations into male homicide.

Not much is known about the perpetrators of male homicide in South Africa, which has rates seven times the global average. For the country's first ever male homicide study we describe the epidemiology of perpetrators, their relationship with victims and victim profiles of men killed by male versus female perpetrators. We conducted a retrospective descriptive study of routine data collected through forensic and police investigations, calculating victim and perpetrator homicide rates by age, sex, race, external cause, employment status and setting, stratified by victim-perpetrator relationships.

A Nationwide Analysis of Gunshot Wounds of the Head and Neck: Morbidity, Mortality, and Cost.

BACKGROUND: Gun violence in the United States rose continuously from 2010 to 2022, spiking during the pandemic, and peaking in 2021 at 48,830 deaths (14.8 per 100,000). Previous reports investigated health and financial burden associated with gunshot wounds (GSWs) during 2004 to 2013; however estimates related specifically to head and neck (H&N) injuries have been lacking. This population-based study aims to examine incidence, morbidity, mortality, and health resource utilization of H&N injuries utilizing the Nationwide Inpatient Sample database. METHODS: A population-based study was undertak

Trauma deaths outside the hospital: uncovering the typology in Kenyan capital.

BACKGROUND: Immediate trauma fatality is not amenable to trauma care and primary prevention is the key. The published profiles of deaths due to trauma differ in different regions. Injury mortality rates are higher in developing countries where injury data capture systems are unreliable for prevention purposes. OBJECTIVE: To describe the pattern of pre-hospital injury (immediate) deaths at the Nairobi city mortuary and compare these with hospital (late) trauma deaths. METHODS: Consecutive trauma autopsies performed over one year (November 2009 to December 2010) at the main mortuary of the Nai

Burnout and Exposure to Critical Incidents in a Cohort of Emergency Medical Services Workers from Minnesota.

INTRODUCTION: Very little quantitative data on occupational burnout and exposure to critical incidents are available from contemporary United States emergency medical services (EMS) cohorts. Given that burnout has been associated positively with turnover intentions and absenteeism in EMS workers, studies that uncover correlates of burnout may be integral to combating growing concerns around retention in the profession. METHODS: We administered a 167-item electronic survey that included the Maslach Burnout Inventory (MBI) and a modified version of the Critical Incident History Questionnaire (n

Epidemiology of ballistic fractures in the United States: A 20-year analysis of the Firearm Injury Surveillance Study.

INTRODUCTION: Ballistic (i.e., gunshot-induced) fractures present unique treatment challenges and can be associated with high rates of complications and considerable morbidity. Large-scale epidemiologic data on these types of fractures are scarce. There is concern that gun-related violence may be on the rise, potentially increasing the burden of ballistic orthopaedic trauma, but there are few contemporary studies on the topic. The aim of this study, therefore, was to investigate the incidence and demographics of patients with ballistic fractures in the United States (US) over the last 20 years

Distinctive patterns in child homicide: A retrospective study in Maryland.

INTRODUCTION: Homicide is a leading cause of childhood death, yet few studies distinguish child homicide from adolescent and adult patterns, challenging the development of prevention strategies. We sought to define how child homicide epidemiology differs from that of older age groups in Maryland. METHODS: In this cross-sectional study, we analyzed all homicide decedents in Maryland (2005-2017) using Office of the Chief Medical Examiner data. Age groups were defined per American Academy of Pediatrics: child/early adolescent (0-14), adolescent (15-22), young adult (23-30), and adult (31+). Desc

Problem drinking and physical intimate partner violence against women: evidence from a national survey in Uganda.

BACKGROUND: Problem drinking has been identified as a major risk factor for physical intimate partner violence (PIPV) in many studies. However, few studies have been carried on the subject in developing countries and even fewer have a nationwide perspective. This paper assesses the patterns and levels of PIPV against women and its association with problem drinking of their sexual partners in a nationwide survey in Uganda. METHODS: The data came from the women's dataset in the Uganda Demographic and Health Survey of 2006. Problem drinking among sexual partners was defined by women's reports th

Every minute matters: Improving outcomes for penetrating trauma through prehospital advanced resuscitative care.

BACKGROUND: Prehospital resuscitation with blood products is gaining popularity for patients with traumatic hemorrhage. The MEDEVAC trial demonstrated a survival benefit exclusively among patients who received blood or plasma within 15 minutes of air medical evacuation. In fast-paced urban EMS systems with a high incidence of penetrating trauma, mortality data based on the timing to first blood administration is scarce. We hypothesize a survival benefit in patients with severe hemorrhage when blood is administered within the first 15 minutes of EMS patient contact. METHODS: This was a retrosp

Community Resources and Hazards Across the Rural-Urban Continuum.

IMPORTANCE: Rural-urban differences in health outcomes have worsened over time, yet the variation in exposure to social determinants of health (SDOH), which are key drivers of these outcomes, has not been adequately quantified. OBJECTIVE: To explore variation in exposure to a sample of beneficial and adverse community-level SDOH across the rural-urban continuum. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study included all residential addresses in Maryland using GPS-Health (Geographic Patterns of SDOH), a 2025 statewide address-level dataset.
