Is an implant removal after dorsal plating of distal radius fracture always needed?

PURPOSE: The purpose of this study was to evaluate the need for an implant removal and the frequency of hardware related complications in a group of distal radius fractures treated with dorsal double plating. METHODS: This retrospective study analyzed data from 37 patients in whom distal radius fracture surgery was performed through single dorsal approach with two low profile titanium plates during a period of 5 years. Objective and subjective parameters were evaluated, focusing on detection of potentially harmful complications and the need of hardware removal during the follow up at least 12

Systematic Review and Meta-Analysis of Hardware Failure in Surgical Stabilization of Rib Fractures: Who, What, When, Where, and Why?

BACKGROUND: Surgical stabilization of rib fractures (SSRF) is increasingly used to reduce pulmonary complications and death among patients with rib fractures. However, the five Ws of hardware failure -who, what, when, where, and why- remains unclear. We aimed to synthesize available evidence on the five Ws and outline future research agenda for mitigating hardware failure. METHODS: Experimental and observational studies published between 2009 and 2020 evaluating adults undergoing SSRF for traumatic rib fractures underwent evidence synthesis. We performed random effects meta-analysis of cohort

Broken Beath Pin Tip During Arthroscopic PCL Reconstruction: A Rare Complication.

Instrument breakage during arthroscopic procedures is rare but can pose notable challenges. We present a case of Beath pin breakage during posterior cruciate ligament (PCL) tunnel preparation, requiring a separate incision for retrieval. A 38-year-old man with a multiligamentous right knee injury (Schenck KDIIIL) and common peroneal nerve palsy underwent arthroscopic anterior cruciate ligament (ACL) and PCL reconstruction with lateral collateral ligament (LCL) reconstruction. During PCL tibial tunnel preparation, the Beath pin tip broke and could not be retrieved arthroscopically. Fluoroscopic

Fixation Failure After Healing of an Adolescent Posterior Wall Acetabular Fracture Without Growth Disturbance: A Case Report.

CASE: An 11-year-old boy sustained a right posterior wall acetabulum fracture treated with open reduction and internal fixation using a 2.7-mm buttress plate that spanned the triradiate cartilage with planned removal at 6 months. The patient was lost to follow-up and returned at age 14 with a normal, pain-free hip. Radiographs showed a healed fracture with no acetabular deformity or growth disturbance; however, his posterior wall plate screws were broken. CONCLUSION: In skeletally immature patients undergoing acetabulum fixation, retained physeal-spanning fixation constructs may fail secondar

Clinical and safety outcome profiles of resorbable fixation systems in pediatric facial fractures: A systematic review and meta-analysis.

The management of facial fractures in the pediatric population represents a unique challenge in facial fracture care, where fixation must strike a balance between stability and interference with growth and the need for secondary implant removal procedures. Bioresorbable fixation systems have been increasingly applied in this population; however, their pooled safety and performance data and clinical advantages over titanium fixation in the pediatric population are not well described. This systematic review and meta-analysis aimed to comprehensively assess the clinical outcomes and complication

Optimizing Fixation and Biologic Augmentation in Aseptic Femoral Nonunion Management: A Retrospective Case Series.

Nonunion poses significant difficulties for both patients and orthopedic surgeons, often requiring intricate reconstructive surgeries to achieve bone healing and eliminate infections. Surgeons must navigate numerous contributing factors to nonunion, and they also face challenging hardware issues during revision procedures. These issues can include infections, loose or failing hardware, misaligned components, or inappropriate hardware configurations. This case series includes five cases of nonunion femur fractures and the goal is to carefully analyze the best treatment option for treating nonun

Pain and infection are the primary reasons for elective retrieval of bone-anchored implants.

PURPOSE: Elective removal of bone-anchored hearing implants and epistheses is uncommon, limiting the understanding of the biological factors contributing to adverse reactions, implant success or failure. Therefore, the tissue-level response at the implant interface remains poorly understood. METHODS: This study examined 21 explanted bone-anchored implants and interfacial tissues from 19 patients using a controlled retrieval protocol. The clinical data were correlated with histological, immunohistochemical, microbiological, and structural analyses. RESULTS: Pain (12/19) and infection (7/19) wer

The fate of plates and screws after facial fracture reconstruction.

Rigid plate and screw fixation is the mainstay of treatment for complex fractures of the facial skeleton. Complications of plate and screw fixation include prominence, infection, exposure, and migration. Five hundred and seven patients undergoing plate and screw fixation for facial fractures (1112 fractures) from 1983 to 1988 were followed for complications. Sixty-one patients (12 percent) required hardware removal. The location on the facial skeleton influenced symptoms and the rate of hardware removal. Infection and exposure may be decreased with antiseptic irrigations, avoiding mucosal dama

Management of complex fibular fractures: double plating of fibular fractures.

Complex fibular fractures as a result of either high-energy mechanisms or advanced age can be difficult to manage as significant comminution can preclude standard operative techniques. Furthermore, the maintenance of osseous reduction postoperatively throughout convalescence can present an equivalent challenge. Strict weightbearing restrictions in the elderly and the noncompliant postoperatively convey an additional risk of potential failure. We present a technique that has offered additional reassurance in these instances. We retrospectively evaluated 25 consecutive patients who had undergone

Removal of patient-specific reconstruction plates after mandible reconstruction with a fibula free flap: is the plate the problem?

Computer-aided microvascular mandible reconstruction is an increasingly common procedure in oral and maxillofacial surgery. The aim of this retrospective single-centre study was to evaluate the rate and specifics of hardware removal after fibula free flap (FFF) fixation with a patient-specific reconstruction plate. The study included patients who underwent hardware removal between April 2017 and October 2019. Statistical analyses were performed regarding the different indications for plate removal (dental implantation versus complication) and the surgical approach (intraoral versus extraoral).

Femoral nail impingement with the quadriceps: an ultrasound case report on orthopedic hardware conflict.

Femoral shaft fractures are commonly treated with intramedullary nailing; however, this procedure can lead to complications. This study explores the role of ultrasound in diagnosing femoral hardware impingement. We present the findings in two patients with persistent thigh pain after femoral intramedullary nailing. Ultrasound successfully identified hardware impingement. Removal of the offending screw led to symptom resolution, with a follow-up US confirming the absence of residual conflicts.

Variable Volumes of Resected Bone Resulting From Different Total Ankle Arthroplasty Systems.

BACKGROUND: The increased popularity and success of total ankle arthroplasty (TAA) has resulted in the development of varying TAA hardware designs, many of which include specific bone-sparing or bone-sacrificing features. The goal of this work was to determine differences in the volume of bone removed for implantation of different total ankle arthroplasty hardware systems. METHODS: Sixteen cadaveric specimens were computed tomography-scanned preoperatively and after total ankle arthroplasty using either an INBONE II, Salto Talaris, STAR, or Zimmer TMTA implant. Geometries of the talus and the

5- months exposed implant after osteosynthesis is not necessarily a problem: A case report of a distal tibia fracture.

CASE: A 66-year-old patient sustained a closed tibial fracture which was treated by minimal invasive plate osteosynthesis. She developed a skin necrosis at the contusion side one month after surgery. A free gracilis flap was performed for soft tissue coverage together with hardware replacement. The intraoperative histopathological examen showed no signs of infection. One out of four positive intraoperative cultures taken from bone samples was positive and was interpreted as contamination. That is why the prophylactic antibiotic therapy with Cefuroxime was stopped after 5 days. The gracilis fla

[Translated article] Long-term clinical and radiological outcomes of Mayo IIA olecranon fractures treated with high-strength transosseous suture.

INTRODUCTION: Olecranon fractures are common injuries, especially in the elderly population, in whom conventional osteosynthesis with tension band wiring (TBW) or plating (PF) is associated with high reoperation rates for hardware removal due to intolerance (up to 80% of surgically treated patients in the TBW group and 25% in those treated with PF). High-strength transosseous suture (TBS) has been proposed as an alternative to reduce these complications associated with conventional techniques, although few studies with long-term follow-up exist. The aim of this study was to assess, in the long

Screw Thread Configuration Has No Effect on Outcomes of In Situ Fixation for Stable Slipped Capital Femoral Epiphysis.

BACKGROUND: No consensus exists regarding the optimal surgical management of slipped capital femoral epiphysis (SCFE). Treatment goals include avoiding slip progression and sequelae such as avascular necrosis (AVN). Factors associated with surgical implants merit further research. This study investigates the effect of screw thread configuration and the number of screws on surgical outcomes. METHODS: A total of 152 patients undergoing cannulated, stainless steel, in situ screw fixation of SCFE between January 2005 and April 2018 were included. Procedure laterality, screw number and thread conf
