The national framework and consequence management guidance following a biological attack.

Consequence management following a release of aerosolized Bacillus anthracis spores requires a high level of technical understanding and direction. National policies and regulations address the topics of preparedness goals and organizational structure, but they do not tell responders how to perform remediation. Essential considerations include determining what must be cleaned, evaluating health risks, ascertaining the priority of cleanup, and selecting appropriate decontamination technologies to meet consensus and risk-derived clearance goals. This article highlights key features of a national

Living with chronic illness in the family setting.

This paper discusses the ways in which family practices and family relationships interact with the demands of diet-related chronic illness. It uses a study that examined the management of food and diet in families where one member had been diagnosed with one of two medical conditions: coronary heart disease (CHD) and coeliac disease (CD). The data revealed a propensity for family members, especially female partners/wives, to see the tasks and activities related to the special diet in terms of family practices rather than individual (patients') responsibilities. These family practices could be

The Effects of a Reproductive Health Voucher Program on Out-of-Pocket Family Planning and Safe Motherhood Service Expenses: A Yemeni Study.

Background/Objectives: Using healthcare vouchers mitigates the financial burdens of low-income individuals, therefore enhancing mothers' satisfaction and encouraging service utilization. In Yemen, reducing financial barriers results in marked improvement in reproductive health services utilization for mothers and their newborns. Such financial strain can be addressed through reproductive health vouchers, which reduce out-of-pocket expenses of family planning, pregnancy, birth, postnatal care and neonatal care. This study compares the Safe Motherhood and Family Planning Voucher Program in the L

Priority-setting in TB programme planning: guiding an equitable and effective response.

Despite being preventable and curable, TB remains a major infectious killer. Effective control requires prevention, early diagnosis, proper treatment, and the ability to address socio-economic risk factors. With limited resources, countries must prioritise interventions amid evolving epidemiology, technologies, and health system demands. To support this, the WHO's policy brief, 'Priority-setting in tuberculosis programme planning', outlines structured and participatory decision-making approaches. It frames priority-setting as an ongoing governance function, not a one-time response to scarcity,

Narratives and behavioral perspectives: the overlooked role of infertility in reproductive health.

Billions of dollars are spent on family planning in low- and middle-income countries each year. Despite this investment, about half of all pregnancies are still unintended. In recent years, there have been indications of progress against closing this gap, including meaningful commitments from country governments, increased investment in reproductive health supplies, and the development of new contraceptive technologies. There is, however, one issue that emerges in almost all research focused on women's needs and preferences related to reproductive health but has been woefully absent from the g

Addressing the health needs of adolescents transitioning out of foster care.

Adolescents in foster care experience complex health care needs and face multiple barriers in receiving the necessary and appropriate health care services. When the adolescent ages-out of foster care at 18 or 21 years-old they are expected to make a transition to independence with no financial resources, poor access to health care, few personal and family connections and little if any support from the foster care system. This places them at risk for poor physical and mental health status, poverty, unemployment, homelessness, and incarceration. No formalized system of transition planning has ye

Strengthening Government Leadership in Family Planning Programming in Senegal: From Proof of Concept to Proof of Implementation in 2 Districts.

Given Senegal's limited resources, the country receives substantial support from externally funded partner organizations to provide family planning and maternal and child health services. These organizations often take a strong and sometimes independent role in implementing interventions with their own structures and personnel, thereby bypassing the government district health system. This article presents findings from the Initiative Sénégalaise de Santé Urbaine (ISSU) (Senegal Urban Health Initiative) that assessed in 2 districts, Diamniadio and Rufisque, the extent to which it was feasible t

Polio transition in the African Region: progress, challenges and prospects for health systems integration.

BACKGROUND: The Global Polio Eradication Initiative (GPEI) has made substantial progress towards global eradication of polio. As regions, including Africa, approach the final stages of this goal, countries must ensure that essential polio functions are sustained and effectively integrated into national health systems. To support this transition, the World Health Organization(WHO), through its regional offices, has guided countries using the Polio Transition Global Vision and its Monitoring and Evaluation Framework. This paper reviews progress made in transition planning across 15 polio transit

Strengthening nationally led approaches to new product introduction for HIV prevention.

As the pipeline of new HIV prevention technologies (including long-acting injectables, vaginal rings, antibody infusions, and, potentially, vaccines) expands careful planning is needed to ensure these innovations are effectively and equitably introduced within public health systems. Such planning is particularly important because the introduction and scale-up of HIV prevention tools has historically been hampered by insufficient national ownership, implementation in pilot settings that do not reflect the realities of broader public health systems, insufficient community engagement, and fragmen

Developing a professional pathway in health equity to facilitate curricular transformation at the University of Michigan Medical School.

PROBLEM: Medical schools are challenged to realign curricula to address society's needs in a rapidly changing environment, and to support new instruction and assessment methods that require substantial faculty time. APPROACH: In 2010, the University of Michigan Medical school began planning the Global Health and Disparities Path of Excellence (GHD Path), an optional co-curriculum for students interested in health disparities, with explicit goals to (1) draw attention to the school's social mission; (2) test new, faculty-intensive methods of learning and assessment for all students; and (3) se

Contraceptive Use Among Women with Prediabetes and Diabetes in a US National Sample.

INTRODUCTION: Although elevated blood glucose is associated with adverse maternal and fetal health outcomes, evidence suggests that women with diabetes may not be receiving comprehensive reproductive health care, including family planning and preconception care. Using a population-based sample, we evaluated the relationship between contraceptive use and biomarker-identified diabetes. METHODS: This cross-sectional study used data from 5548 women in the nationally representative National Longitudinal Study of Adolescent to Adult Health (Add Health) from 2007 to 2009. Women were aged 24 to 32 ye

An Overview of Global Neurosurgery.

Until recently, surgery had been passed over in the domain of global health, historically being described as "the neglected stepchild of global health." Knowledge of the existing global disparities in neurosurgical care has led to neurosurgery capacity-building efforts especially in low-income and middle-income countries. While many global collaborative projects are currently undertaken with philanthropic support, sustainability and scalability are not likely without governmental adoption of neurosurgery-inclusive national surgical plans. Momentum grows for the global neurosurgery community to
