OBJECTIVE: To elucidate clinical antecedents of sensorineural hearing loss (SNHL) in very preterm infants.
DESIGN: Case-control study.
SUBJECTS: Fifteen children < 33 weeks' gestation with significant SNHL born between 1 January 1990 and 31 December 1994, detected within 9 months of birth, and 30 matched control children.
METHODOLOGY: Perinatal variables in the two groups were compared using non-parametric tests and conditional logistic regression (EGRET).

CT study of the middle ear and ossicular chain plays a major role in assessing the etiology of conductive hearing loss. A review is given on the way our knowledge of pathways of immune responses inside and in the immediate vicinity of the inner ear has gradually developed over the past two decades. Immune reactivity plays a more important role in the etiopathogenesis and natural course of various inner ear disorders than was thought originally. Hearing loss in infants and children may be sensorineural, conductive, or mixed. Severity varies from mild to profound. Educational initiatives aimed at children, parents, and primary health care providers could help prevent needless permanent hearing impairment. Effective programs aimed at education and hearing conservation among children and adolescents are overdue. The causes of sensorineural hearing loss, the concept of multidisciplinary team evaluation, and measurement of hearing are discussed. Advances in genetics of hearing loss are reviewed.

In order to determine the characteristics and incidence of hearing loss in Behçet's disease, 72 consecutive cases and 72 sex- and age-matched normal subjects were submitted to this study. Detailed audiologic tests were performed in all cases. A prospective hearing survey was performed in a sample of 102 diabetic patients. The hearing data were compared with the hearing thresholds of three control population groups. A significant difference was found in the average hearing thresholds between the diabetic patients and all of the three control populations. Several autoimmune disorders are associated with inner ear involvement. The resulting sensorineural loss of hearing may be improved by treatment with immunosuppressive agents. We describe a patient with oral ulcerations, systemic vasculitis, iridocyclitis and bilateral sudden deafness. After a course of systemic steroid treatment the hearing improved. Atypical Cogan's syndrome is the most likely diagnosis.

A report is presented on a man of Turkish origin, with Noonan Syndrome and unilateral conductive hearing loss since early childhood. There was no history of otitis media. At the age of 23, exploratory tympanotomy revealed a total absence of the long process of the incus and a normal-looking tympanic membrane. The position of the normal-shaped mobile stapes was just medial, and not posteromedial, to the malleus. A congenital ossicular chain anomaly was diagnosed. Sixty head injury patients were evaluated for hearing loss; ten underwent ABER testing. Forty percent of the cases had hearing loss of different degrees. The audiometric pattern was variable in the different types of injuries, although the incidence of conductive deafness was quite low (5%). In most of the cases of sensorineural hearing loss, the end organ was implicated. Recruitment was demonstrated in the ABERs, as well as in the retrocochlear lesions, by studying the IPLs present.

Eleven temporal bones from eight patients who had clinical histories of sudden hearing loss (SHL) were studied to assess the possible etiopathogenesis. Out of 360 children with hearing loss seen during 1 year in the Haringey Audiology Unit, 21 had unilateral, sensorineural hearing loss. Sixteen had previously been checked to have no loss. It is strongly suspected that the hearing loss resulted from mumps, and therefore the disability in such children could be prevented by introducing mumps vaccination into the immunisation programme.
