The Importance of Mitochondrial Disease Testing in Young Adults With New Onset Sensorineural Hearing Loss.

OBJECTIVES: Sensorineural hearing loss (SNHL) occurs commonly as part of mitochondriopathies and varies in severity and onset. In this study, we characterized hearing with specific consideration for hearing loss as a potential early indicator of mitochondrial disease (MD). We hypothesize that genetic testing at the earliest detection of SNHL may lead to an earlier MD diagnosis. DESIGN: We reviewed the clinical and audiometric data of 49 patients undergoing genetic testing for MD. RESULTS: One-third of individuals with molecularly confirmed MD presented with SNHL. On average, patients had hea

Sensitivity of the antiphasic digits-in-noise test to simulated unilateral and bilateral conductive hearing loss.

OBJECTIVES: The objective of this study is (1) to assess whether the presentation level of the antiphasic digits-in-noise (DIN) test affects the speech recognition threshold (SRT), (2) to evaluate how accurately simulated unilateral and bilateral conductive hearing loss is detected (CHL) and (3) to determine whether increasing the presentation level normalises the antiphasic DIN SRT. DESIGN: Participants performed antiphasic and diotic DINs at different presentation levels with unilateral, bilateral or no earplugs. STUDY SAMPLE: Twenty-four and twelve normal hearing adults. RESULTS: Without

Racial and ethnic disparities in diagnostic efficacy of comprehensive genetic testing for sensorineural hearing loss.

Understanding racial and ethnic disparities in diagnostic rates of genetic testing is critical for health equity. We sought to understand the extent and cause of racial and ethnic disparities in diagnostic efficacy of comprehensive genetic testing (CGT) for sensorineural hearing loss (SNHL). We performed a retrospective cohort study at two tertiary children's hospitals on a diverse cohort of 240 consecutive pediatric patients (76% publicly insured, 82% non-White) with SNHL of unknown etiology who underwent CGT. Definite and possible genetic diagnoses were assigned for each patient, representin

Clinical Test Batteries to Diagnose Hearing Loss in Infants and Children: The Cross-Check Principle.

Frequency-specific audiometry and tympanometry are widely considered the foundation of the pediatric hearing evaluation. However, additional behavioral and objective measures are needed to cross-check findings and ensure an accurate and timely diagnosis. This is particularly important in the pediatric population, wherein a delay in diagnosis can have a significant impact on language development, learning, and educational outcomes. The following discussion emphasizes the importance of performing a comprehensive test battery in the pediatric audiology clinic through 5 unique case studies.

Significance of Vestibular Testing on Distinguishing the Nerve of Origin for Vestibular Schwannoma and Predicting the Preservation of Hearing.

BACKGROUND: Determining the nerve of origin for vestibular schwannoma (VS), as a method for predicting hearing prognosis, has not been systematically considered. The vestibular test can be used to investigate the function of the superior vestibular nerve (SVN) and the inferior vestibular nerve (IVN). This study aimed to preoperatively distinguish the nerve of origin for VS patients using the vestibular test, and determine if this correlated with hearing preservation. METHODS: A total of 106 patients with unilateral VS were enrolled in this study prospectively. Each patient received a caloric

Delayed evoked otoacoustic emissions--an ideal screening test for excluding hearing impairment in infants.

A screening test is badly needed with which cochlear hearing impairment in small children and newborns can be detected. Delayed evoked otoacoustic emissions (DEOAEs) have been found in laboratory research to be a very useful tool to test the normality of cochlear preprocessing. The characteristics of a simple apparatus for clinical use to measure DEOAEs are described together with typical examples of emissions. Many case studies have already demonstrated the clinical usefulness of DEOAEs in neonates and small children. Five cases are explained in detail and the advantages of such an early scre

Unveiling the potential of the Digits-in-Noise test as a hearing screening tool for older adults with cognitive impairment.

BackgroundThe Digits-in-Noise (DIN) test is recognized as a promising hearing screening tool due to its feasibility and reliability, particularly in noisy environments. Although endorsed by the World Health Organization for general population hearing screening, it has not been validated in older adults with cognitive impairment, such as mild cognitive impairment (MCI), and dementia, including Alzheimer's disease.ObjectiveTo address this gap, this study aimed to evaluate diagnostic accuracy of DIN compared to pure-tone audiometry, marking the first validation of the DIN test in this specific gr
