A 5-year-old boy with stenosed left pulmonary artery following total correction of tetralogy of Fallot underwent surgical pulmonary arterioplasty through a left anterolateral thoracotomy on a normothermic perfused heart under cardiopulmonary bypass. We found this to be a convenient approach, and recommend it for correction of this lesion in the absence of gross pulmonary regurgitation and right ventricular outflow tract dilatation.

The removal of the patient's own diseased heart in order to perform a total cardiac transplantation has several disadvantages. A new technique for bypassing the patient's diseased left ventricle by using a cardiac allograft has been developed in our laboratory and applied clinically in 2 patients. This technique carried no direct surgical mortality, and no complications resulting from the patient's own heart being left in situ have been observed thus far. The presence of a second heart lying partly in the anterior mediastinum and partly in the right pleural space appeared to have no deleteriou

An effective method has been developed for prevention of air embolism in cardiopulmonary bypass surgery on the open heart without heart isolation from intergrown tissues. The method consists in filling and washing heart cavities with carbon dioxide before removing the clamps from the aorta. This method does not involve routine prophylactic measures such as heart dislocation and puncture of left-ventricular apex, heart ventricles massage to remove air, etc. Operation without heart isolation from intergrown tissues helps markedly reduce blood loss, make the operation less traumatic and shorter.

Cor triatriatum sinister is a rare congenital heart disease in which the left atrium is divided into 2 chambers by a membrane, causing resistance to the blood flow to the left ventricle. The onset of symptoms depends upon the effective size of the orifice in the membrane and the associated heart disease. Our case is a 67-year-old woman with cor triatriatum found by chance at the onset of myocardial infarction. Coronary artery bypass surgery and excision of the membrane were successfully performed. Preoperative left ventricular dysfunction was improved after surgery. Imaging examinations such a

Patients with porcelain aorta carry a high risk of cerebral as well as systemic embolism during cardiac surgery. Here we describe a case of severe aortic stenosis and coronary artery disease combined with the circumferentially calcified aorta. The patient was a 77-year-old man who successfully received four coronary artery bypass grafts with in situ arterial grafts without clamping the aorta and aortic valve replacement. Aortic valve replacement and two distal coronary artery anastomoses to the left circumflex artery and obtuse marginal branch were performed under cardiac arrest during hypothe

Descending thoracic aortic (DTA) aneurysms causing left main bronchus compression can be surgically repaired under left heart bypass (LHB). Safe LHB requires a competent aortic valve. Some patients present with concomitant DTA aneurysms and severe aortic regurgitation (AR), precluding LHB as an adjunct for aortic surgery. The authors present such a case and outline the management. AR can safely be addressed first in an immediate staged surgical approach, providing adequate left ventricular function.

OBJECTIVE: To determine whether changes in cardiac performance observed in infants early during extracorporeal membrane oxygenation are due to the underlying disease process, the partial bypass procedure, changes in cardiac loading conditions, or due to a direct myocardial effect of hypoxia-reoxygenation.
DESIGN: Consecutive case series.
PATIENTS: Infants with meconium aspiration syndrome were studied at the end of their extracorporeal membrane oxygenation course (veno-arterial, n = 12; veno-venous, n = 8), after lung compliance and pulmonary arterial pressure returned toward normal and they

PURPOSE: The conventional surgical treatment for postinfarction left ventricular aneurysm (LVA) is open-heart repair with cardiopulmonary bypass. However, the risk of the open-heart surgery under cardiopulmonary bypass may result in an unacceptable risk for many patients with multiple comorbidities. Here, we reported a new off-pump repair technique for postinfarction apical LVA.
METHODS: A new off-pump repair technique, circular banding and occlusion technique, was applied to repair the postinfarction apical LVA in 12 patients. Clinical data of all those 12 patients were retrospectively revie

Hemodynamic changes during pregnancy can result in cardiovascular decompensation in women with pre-existing cardiac diseases. Despite optimized medical treatment, some patients with severe structural cardiac abnormalities may need surgical intervention during pregnancy. We describe a woman who presented at 20 weeks of gestation with acute heart failure due to cor triatriatum, a rare form of congenital heart disease. This condition is characterized by a perforated fibromuscular membrane dividing the left atrium into two chambers. The clinical presentation varies from asymptomatic to acute heart

AIMS: Cardiopulmonary bypass (CPB) is the standard approach for coronary artery bypass grafting (CABG) in advanced ischaemic cardiomyopathy. Microaxial pump support has been envisioned to allow for beating-heart CABG without CPB (MPCAB), thereby avoiding CPB-inherent complications. This study aims to compare the in-hospital and follow-up outcome of MPCAB versus CPB-CABG in patients with severely impaired left ventricular function.
METHODS AND RESULTS: Eleven patients suffering from three-vessel coronary artery disease with median ejection fraction of 27% and deemed appropriate for CABG accord

PurposeTo reduce and avoid the use of exogenous blood products for neonates and infants, our center limits the use of packed red blood cells and does not use fresh frozen plasma in our cardiopulmonary bypass (CPB) circuit primes. This practice has resulted in several bloodless neonatal open heart surgical procedures including the entire post operative hospital stays. This case report describes a patient with Hypoplastic Left Heart Syndrome who underwent the Norwood procedure without the use of exogenous blood products. No other report of a bloodless Norwood procedure could be found in the lite

Leriche syndrome usually occurs when atherosclerotic obstructions result in luminal narrowing of the abdominal aorta or iliac arteries and leads to thrombosis; it rarely causes heart or renal failure. We report the case of a 58-year-old Asian man with heart and renal failure as the dominant clinical manifestations of renovascular hypertension caused by Leriche syndrome. We performed an aorto-bifemoral bypass and unilateral renal artery stenting. Post-operative echocardiography showed improved cardiac function, with the left ventricular ejection fraction increasing from 30% before surgery to 54

We report the case of a 72-year-old woman who underwent urgent coronary bypass grafting (CABG). Intraoperatively a persistent left superior vena cava (PLSVC) draining into the left atrium was detected. Because of the resulting right-to-left shunt volume even after complete drainage and collapse of the right atrium, the heart was still ejecting in the absence of significant aortic insufficiency. Thorough examination revealed a PLSVC draining into the left atrium. After ligation of the PLSVC, the CABG could be performed as planned.

OBJECTIVE: This study aimed to demonstrate the early mortality results of the minimally invasive off-pump coronary artery bypass surgery.
METHODS: Fifteen patients (11 males, 4 females; mean age: 62 years) who underwent OPCAB between December 2021 and July 2024 were retrospectively analyzed. All patients had a left anterior descending artery (LAD) stent and in-stent stenosis or stent thrombosis. All patients underwent left internal mammary artery to LAD anastomosis on a beating heart without cardiopulmonary bypass via thoracotomy.
RESULTS: There was no early mortality. The mean duration of h

OBJECTIVE: The bidirectional Glenn procedure is a well-established procedure performed as part of the single-ventricle palliation pathway. Numerous studies have highlighted the potential benefits of an "early" BDG procedure. The ideal age to perform the BDG procedure, however, remains uncertain. We report our experience with the BDG procedure in patients younger than 3 months.
METHODS: One hundred sixty-nine consecutive patients from 1998 to 2007 undergoing the BDG procedure were divided into 2 groups: younger than 3 months (n = 20) and older than 3 months. The groups were compared for 26 var

BACKGROUND: Modified ultrafiltration has been shown to reverse haemodilution and improve ventricular function following cardiopulmonary bypass. There has been concern, however, about the safety and efficacy of modified ultrafiltration after the first stage of Norwood reconstruction for palliation of neonates with hypoplasia of the left heart and its variants.
METHODS: We reviewed the intraoperative course of all patients undergoing the first stage of Norwood reconstruction between September 1, 2000, and August 31, 2002.
RESULTS: The first stage of reconstruction was performed in 99 neonates,

A consecutive series of 98 patients ranging from 2 1/2 to 79 years of age underwent mitral valve replacement using the superior approach that entails an atriotomy done between the superior vena cava and the ascending aorta prolonging it into the left superior pulmonary vein. The technique opens the roof of the left atrium without dissection, frequently without the need for double cannulation of the right atrium. No mobilization of the heart is involved, which is left in its normal position. There were 62 patients undergoing replacement alone, 22 combined with coronary bypass surgery, 10 with s
