The impact of pericardial disruption on heart function and remodeling after myocardial infarction in mice.

Myocardial infarction (MI) remains a leading cause of deaths globally. Thus, reliable and relevant animal MI models are crucial to understand underlying disease mechanisms and develop new therapies. Generally, MI in rodents is induced by ligation of the left anterior descending artery (LAD) leaving a ruptured pericardium. This contrasts MI in man, where the pericardium remains intact. Herein, we therefore determined in adult mice whether pericardial disruption along MI influences left ventricular (LV) heart function, remodeling, and scar formation. Baseline heart function was assessed in 30 C5

Characterizing the transition from immune response to tissue repair after myocardial infarction by multiparametric imaging.

Persistent inflammation following myocardial infarction (MI) precipitates adverse outcome including acute ventricular rupture and chronic heart failure. Molecular imaging allows longitudinal assessment of immune cell activity in the infarct territory and predicts severity of remodeling. We utilized a multiparametric imaging platform to assess the immune response and cardiac healing following MI in mice. Suppression of circulating macrophages prior to MI paradoxically resulted in higher total leukocyte content in the heart, demonstrated by increased CXC motif chemokine receptor 4 (CXCR4) positr

Post-traumatic left ventricular false aneurysm.

Most false aneurysms of the heart represent contained ventricular free wall ruptures after myocardial infarction. Post-traumatic aneurysms also may follow penetrating or non-penetrating trauma to the chest. Regardless of the origin of the false aneurysm there is a propensity for aneurysm rupture. We report a patient who developed a false aneurysm of her left ventricle that developed post-motor vehicle accident. Her orthopedic problems were the clinical problems identified and after a hospital admission of 10 days she was discharged home. Four weeks later she died suddenly from anterior left ve

Clinical Manifestation, Timing Course, Precipitating Factors, and Protective Factors of Ventricular Free Wall Rupture Following ST-Segment Elevation Myocardial Infarction.

Ventricular free wall rupture (FWR) is a catastrophic complication of that occurs after acute myocardial infarction (AMI), and at present, its clinical characteristics are unclear. We analyzed a total of 6,712 consecutive patients who presented with ST-segment elevation myocardial infarction (STEMI), and 78 patients with FWR after STEMI were enrolled in the study. Patients' demographic data, clinical manifestation, laboratory test results, and angiographic features were then collected and analyzed. FWR occurred in 78 cases (1.16%), and the inhospital mortality of FWR was up to 92.3%. Among the

Isolated right coronary lesion and anterolateral papillary muscle rupture--case report and review of the literature.

Ischemic rupture of the anterolateral papillary muscle is uncommon due to its dual blood supply. It usually follows an ischemic event involving branches of the left circumflex or left anterior descending arteries. We present a case of a patient admitted with an acute inferior wall myocardial infarction and an isolated distal right coronary artery occlusion. Acute mitral regurgitation with rupture of the anterolateral papillary muscle was diagnosed on the fifth post-infarction day. The patient underwent mitral valve replacement and coronary artery bypass grafting to the posterior descending art

Distance from the coronary ostium to the culprit lesion in acute ST-elevation myocardial infarction and its implications regarding the potential prevention of proximal plaque rupture.

BACKGROUND: Shorter distances from the coronary ostia to culprit lesions have been associated with a higher incidence of adverse outcomes in ST elevation acute myocardial infarction (STEMI). As drug-eluting stents are associated with low rates of restenosis and formation of a stable intima, we sought to develop a mathematical model to estimate how far down the coronary artery a drug-eluting stent would have to be placed to theoretically mitigate the risk of proximal plaque rupture. OBJECTIVES AND METHODS: Distances from the ostia to the end of the culprit lesions were planimetered in 1,914 pa

Posterior myocardial infarction complicated by rupture of the posteromedial papillary muscle.

A 61-year-old man was admitted with acute posterior myocardial infarction and, on physical examination, was shown to have a mitral regurgitation (MR) murmur. Transthoracic echocardiography (TTE) showed severe hypokinesis of the posterior wall and severe MR by color flow. Right heart catheterization with a balloon-tipped catheter revealed a pulmonary artery wedge pressure of 30 mmHg. No 'step-up' was seen in blood samples from the right atrium and right ventricle. On angiography, a subtotal occlusion of the mid circumflex artery was found which was angioplastied and stented. As the patient's cl

Breeding Strategy Determines Rupture Incidence in Post-Infarct Healing WARPing Cardiovascular Research.

BACKGROUND: Von Willebrand A domain Related Protein (WARP), is a recently identified extracellular matrix protein. Based upon its involvement in matrix biology and its expression in the heart, we hypothesized that WARP regulates cardiac remodeling processes in the post-infarct healing process. METHODS AND RESULTS: In the mouse model of myocardial infarction (MI), WARP expression increased in the infarcted area 3-days post-MI. In the healthy myocardium WARP localized with perlecan in the basement membrane, which was disrupted upon injury. In vitro studies showed high expression of WARP by card

Modifying the mechanics of healing infarcts: Is better the enemy of good?

Myocardial infarction (MI) is a major source of morbidity and mortality worldwide, with over 7 million people suffering infarctions each year. Heart muscle damaged during MI is replaced by a collagenous scar over a period of several weeks, and the mechanical properties of that scar tissue are a key determinant of serious post-MI complications such as infarct rupture, depression of heart function, and progression to heart failure. Thus, there is increasing interest in developing therapies that modify the structure and mechanics of healing infarct scar. Yet most prior attempts at therapeutic sca

[Papillary muscle rupture in myocardial infarction].

We report nine patients with acute mitral regurgitation secondary to post-infarction papillary rupture operated between 1980 and 1992. Seven cases had posterior papillary muscle rupture. All patients were in critical conditions with pulmonary edema at the moment of surgery. In eight cases, mitral valve replacement was performed (4 with mechanical prostheses) and in one, the valve was repaired with papillary muscle reimplantation. Six cases were also subjected to myocardial revascularization with sapheneous vein grafts. Two patients (22%) died during the postoperative period and 4 had postopera

Post-Myocardial Infarction Ventricular Septal Defect Percutaneous Closure.

BACKGROUND: Ventricular septal rupture is an uncommon but serious complication of late post-myocardial infarctions (MI) with an exceedingly high mortality. The American College of Cardiology and American Heart Association still advise immediate surgical closure of ventricular septal rupture. CASE SUMMARY: We described a novel minimally invasive transcatheter approach to post-MI ventricular septal defect (PMIVSD) closure through a venovenous loop for an 82-year-old woman. DISCUSSION: A PMIVSD is typically treated surgically and offers reasonable outcomes in patients who survive an initial hea

The novel use of heart transplantation for the management of a case with multiple complications after acute myocardial infarction.

Rupture of the interventricular septum after myocardial infarction (MI) is an uncommon but serious complication, usually leading to congestive heart failure and cardiogenic shock. Surgical repair is usually the only definitive treatment for these patients because medical management is associated with a 30-day mortality approaching 100%. However with conventional surgical repair, operative mortality rates range from 33% to 53%. Furthermore, outcomes in patients with posterior ventricular septal defect (VSD) have been reported to have mortality rates up to 86%. Therefore, alternative treatment s

Double jeopardy: two major complications of a myocardial infarction in one patient.

A 62-year-old woman with no cardiac history was admitted to the hospital with an ambulant inferoposterolateral myocardial infarction. She experienced two major complications, namely, a free-wall rupture of the infarcted area, on which a bovine pericardial patch was glued surgically, and, a few days later, a posteromedial papillary muscle rupture, for which a mitral valve replacement was performed. She survived both complications and is now rehabilitating at home.

Postinfarct cardiac free wall rupture: the relationship of rupture site to papillary muscle insertion.

The objective of this study was to examine for any relationship between the sites of papillary muscle insertion in left ventricular free wall (LVFW) and the site of LVFW rupture postinfarct. Twenty-five consecutive patients with LVFW rupture (12 men and 13 women, mean age 72.3 yr, range = 48 to 93) at the University of Ottawa Heart Institute during the period of July 1, 1988 to 1992 were studied. Clinical charts were reviewed, and the Formalin-fixed hearts were re-examined grossly and microscopically. Each patient died of tamponade or after surgery for tamponade. Eight (32%) of the 25 infarcts

Different forms of cardiomyocyte death in post-myocardial infarction ventricular remodeling: mechanisms and therapeutic strategies.

Myocardial infarction (MI) is one of the leading causes of heart failure and cardiovascular mortality worldwide, with post-infarction ventricular remodeling serving as a central pathophysiological mechanism driving the progression toward heart failure, a process that markedly impairs long-term quality of life and contributes to adverse clinical outcomes. Recent studies have gradually elucidated the distinct and overlapping roles of multiple forms of cardiomyocyte death in this process, forming a complex network of programmed cell death; following MI, cardiomyocytes undergo various modes of dea
