Effects of Ibuprofen during Exertional Heat Stroke in Mice.

UNLABELLED: Intestinal injury is one of the most prominent features of organ damage in exertional heat stroke (EHS). However, whether damage to the intestine in this setting is exacerbated by ibuprofen (IBU), the most commonly used nonsteroidal anti-inflammatory drug in exercising populations, is not well understood. PURPOSE: We hypothesized that IBU would exacerbate intestinal injury, reduce exercise performance, and increase susceptibility to heat stroke. METHODS: To test this hypothesis, we administered IBU via diet to male and female C57/BL6J mice, over 48 h before EHS. Susceptibility to

Unusual manifestation of heat stroke: Isolated trochlear nerve palsy.

Heat stroke is a life-threatening disease characterized by hyperthermia and neurological dysfunction. The central nervous system is highly sensitive to hyperthermia, which causes neurological complications due to the involvement of the cerebellum, basal ganglia, anterior horn cells, and peripheral nerves. Several studies reported about clinical symptoms and brain image findings of heat stroke. Isolated cranial nerve dysfunction caused by lacunar infarction is an extremely rare condition in patient with heat stroke. We experienced a rare case of trochlear nerve palsy due to midbrain infarction

Controversies in exertional heat stroke diagnosis, prevention, and treatment.

During the past several decades, the incidence of exertional heat stroke (EHS) has increased dramatically. Despite an improved understanding of this syndrome, numerous controversies still exist within the scientific and health professions regarding diagnosis, pathophysiology, risk factors, treatment, and return to physical activity. This review examines the following eight controversies: 1) reliance on core temperature for diagnosing and assessing severity of EHS; 2) hypothalamic damage induces heat stroke and this mediates "thermoregulatory failure" during the immediate recovery period; 3) EH

Sex-dependent responses to exertional heat stroke in mice.

With increasing participation of females in endurance athletics and active military service, it is important to determine if there are inherent sex-dependent susceptibilities to exertional heat injury or heat stroke. In this study we compared responses of male and female adult mice to exertional heat stroke (EHS). All mice were instrumented for telemetry core temperature measurements and were exercise-trained for 3 wk before EHS. During EHS, environmental temperature was 37.5°C (35% RH) while the mice ran on a forced running wheel, using incremental increases in speed. The symptom-limited endp

Is muscle energy production disturbed in exertional heat stroke?

BACKGROUND: Exertional heat stroke (EHS) is a life-threatening disease that shares some clinical similarities with malignant hyperthermia (MH). By use of (31)Phosphorus magnetic resonance spectroscopy (MRS), EHS patients with MH susceptibility and MH patients shared common metabolic abnormalities. The aim of this study was to determine whether subjects who suffered from an EHS episode had disturbed muscle energetics. METHOD: This retrospective study was performed within the French database of military subjects that were explored from 2004 to 2010 after they suffered an EHS. All subjects had b

Exertional heat stroke, rhabdomyolysis and susceptibility to malignant hyperthermia.

Unexpectedly severe exertional heat stroke and rhabdomyolysis should prompt a clinician to look for susceptibility to malignant hyperthermia. We report a case of exertional heat stroke and rhabdomyolysis in a man later determined to have the malignant hyperthermia phenotype. We review the existing literature regarding this association and suggest future research that could address areas of remaining clinical uncertainty.

Long-term psychological and physiological effects of heat stroke.

Heat stroke leads only rarely to permanent neurological deficits and the convalescence is almost complete. There are, however, some sporadic descriptions of disturbances that lasted for up to 4 months. Little has been mentioned in the literature on residual changes in personality and late neurological side effects. The present study was conducted to follow systematically late personality and behavioral abnormalities in a population of heat stroke victims. This study analyzed 21 young subjects (age: 21 +/- 2 years), who were inflicted by heat stroke. They were invited for a physiological and ps

Chronic idiopathic anhydrosis--a rare cause of heat stroke.

A 27 year old man presented with heat stroke following exposure to a humid, hot environment in the absence of physical exertion. Investigation revealed the presence of generalized anhydrosis without evidence of an associated disease. Although chronic idiopathic anhydrosis is rare, this entity should be considered in cases of unexplained heat intolerance and heat stroke.

Field and clinical observations of exertional heat stroke patients.

Exertional heatstroke (EH) occurs when heat production, generated by muscular exercise, exceeds the body's heat dissipation capacities. This illness has been reported among young, active individuals, laborers, and religious pilgrims. Although EH includes a rectal temperature above 39.5 degrees C (depending on the timing of the measurement) and elevation of serum enzymes, altered mental status is the universally accepted sign which distinguishes EH from heat exhaustion. Once EH is suspected, cooling therapy should be initiated immediately and investigation of multiple-system involvement should

Antilipopolysaccharide improves survival in primates subjected to heat stroke.

Recent investigations have suggested that endotoxins or lipopolysaccharides (LPS) may play a role in heat stroke pathophysiology. In this study we wish to investigate whether prophylactic administration of anti-LPS hyperimmune plasma improves survival of experimental heat stroke in primates. Eleven monkeys were anesthetised and heat-stressed to a rectal temperature of 43.5 degrees C (group A) and then allowed to recover at room temperature. Five had received a prophylactic i.v. dose of equine anti-LPS (experimental group), while the other six (control group) received an equivalent i.v. dose of

[Heat stroke with anuria, muscular disorders and early hypercalcemia].

A case is reported of heat stroke associating dehydration, anuria, muscle disorders and early hypercalcemia. All disorders disappeared within 48 hours with rehydration. Early hypercalcemia differs from late hypercalcemia reported at resumption of diuresis. Early hypercalcemia might result from blood concentration with hyperproteinemia and release of bone calcium under the action of PTH. Its prognosis is good, which is not the case of late hypercalcemia.

Acute heat stroke. Epidemiologic, biochemical, renal, and coagulation studies.

Fifteen Marine recruits with acute heat stroke were examined for (1) predisposing factors, (2) blood coagulation disturbances, (3) renal function abnormalities, and (4) blood composition alterations. Epidemiologic data identified the following risk factors; previous residence in a temperate climate, first phase of training, fatigue, and strenuous exercise in hot, humid conditions. Results of blood coagulation studies disclosed an increase in prothrombin and partial thromboplastin times, with a decrease in platelet count, probably indicating a transient, low-grade consumptive process. Blood ure

Heat exhaustion and heat stroke among active component members of the U.S. Armed Forces, 2020-2024.

In 2024, the crude incidence rates of heat stroke and heat exhaustion were 36.4 and 183.9 cases per 100,000 person-years, respectively. After a period of decline in rates of incident heat stroke from 2020 through 2023, during the 2024 surveillance period an increase was observed. When considering only heat exhaustion, incident rates increased each year during the 5-year surveillance period, from 2020 through 2024. In 2024, higher rates of heat stroke were observed among male service members, when compared to their female counterparts, as well as among non-Hispanic White service members compare

Heat exhaustion and heat stroke among active component members of the U.S. Armed Forces, 2019-2023.

The most serious types of heat illnesses, heat exhaustion and heat stroke, are occupational hazards associated with many of the military's training and operational environments. These illnesses can typically be prevented by appropriate situational awareness, risk management strategies, along with effective countermeasures. In 2023, the crude incidence of heat stroke and heat exhaustion were 31.7 and 172.7 cases per 100,000 person-years, respectively. The rates of incident heat stroke declined during the 2019 to 2023 surveillance period, but rates of incident heat exhaustion increased over the

Gut microbiota modulation in the prevention and treatment of heat stroke.

In recent years, heat stroke (HS) have been reported with increasing frequency, and this trend is hard to separate from broader environmental changes, including climate change, recurrent extreme heat events, and air pollution. When people are exposed to high-temperature environments for a prolonged period, especially during intense physical activity, the condition may progress to HS. HS is an acute and potentially fatal disorder that can deteriorate rapidly if not treated in time. The intestine appears to be particularly vulnerable during HS. HS can disrupt intestinal tight junctions and weake
